Impacted Central Incisors in the Upper Jaw in an Adolescent Patient: Orthodontic-Surgical TreatmentâA Case Report
The inclusion of both maxillary permanent central incisors is uncommon. This condition compromises face aesthetics, phonation andmasticatory function. Therefore, early diagnosis is essential to avoid complications and failures. There are various reasons for inclusion, but supernumerary teeth are the leading cause. Early causes of removal and rapid expansion of the palate determine a high probability of success with the spontaneous eruption of the impacted elements. However, it is often necessary to proceed with a surgicalâorthodontic treatment. The inclination of teeth in relation to the midline and the root maturation degree determine prognosis and therapeutic timing. In this case report, the orthopantomogram (OPG) X-ray of a 9-year-old boy revealed two impacted supernumerary teeth in the anterior maxillary region, preventing the eruption of the permanent upper central incisors. The impacted supernumerary teeth were surgically removed at different times. A straight wire multibrackets technique associated with a fixed palatal appliance was used. The palatal appliance featured an osteomucous resin support at the level of the retroincisal papilla. Subsequently, surgical exposure was carried out using the closed eruption technique and elastictraction, bringing 11 and 21 back into the arch
COVID-19 Infection in Children and Infants: Current Status on Therapies and Vaccines
Since the beginning in December 2019, the SARS-CoV-2 outbreak appeared to affect mostly the adult population, sparing the vast majority of children who only showed mild symptoms. The purpose of this investigation is to assess the status on the mechanisms that give children and infants this variation in epidemiology compared to the adult population and its impact on therapies and vaccines that are aimed towards them. A literature review, including in vitro studies, reviews, published guidelines and clinical trials was performed. Clinical trials concerned topics that allowed a descriptive synthesis to be produced. Four underlying mechanisms were found that may play a key role in providing COVID-19 protection in babies. No guidelines are available yet for therapy due to insufficient data; support therapy remains the most used. Only two vaccines are approved by the World Health Organization to be used in children from 12 years of age, and there are currently no efficacy or safety data for children below the age of 12 years. The COVID-19 clinical frame infection is milder in children and adolescents. This section of the population can act as vectors and reservoirs and play a key role in the transmission of the infection; therefore, vaccines are paramount. More evidence is required to guide safely the vaccination campaign
Benefits and Implications of Resveratrol Supplementation on Microbiota Modulations: A Systematic Review of the Literature
Abstract: Resveratrol is a polyphenol that has been shown to possess many applications in different
fields of medicine. This systematic review has drawn attention to the axis between resveratrol and
human microbiota, which plays a key role in maintaining an adequate immune response that can
lead to different diseases when compromised. Resveratrol can also be an asset in new technologies,such as gene therapy. PubMed, Cochrane Library, Scopus, Web of Science, and Google Scholar were
searched to find papers that matched our topic dating from 1 January 2017 up to 18 January 2022,
with Englishâlanguage restriction using the following Boolean keywords: (âresveratrolâ AND
âmicrobio*â). Eighteen studies were included as relevant papers matching the purpose of our
investigation. Immune response, prevention of thrombotic complications, microbiota, gene therapy,
and bone regeneration were retrieved as the main topics. The analyzed studies mostly involved
resveratrol supplementation and its effects on human microbiota by trials in vitro, in vivo, and ex
vivo. The beneficial activity of resveratrol is evident by analyzing the changes in the hostâs genetic
expression and the gastrointestinal microbial community with its administration. The possibility of
identifying individual microbial families may allow to tailor therapeutic plans with targeted
polyphenolic diets when associated with microbial dysbiosis, such as inflammatory diseases of the
gastrointestinal tract, degenerative diseases, tumors, obesity, diabetes, bone tissue regeneration, and
metabolic syndrome
The Effects of Periodontal Treatment on Rheumatoid Arthritis and of Anti-Rheumatic Drugs on Periodontitis: A Systematic Review
Rheumatoid arthritis (RA) and periodontitis are chronic inflammatory diseases that widely spread and share the same patterns of pro-inflammatory cytokines. This systematic review aims to evaluate the effects of non-surgical periodontal treatment (NSPT) on RA and, conversely, the impact of disease-modifying anti-rheumatic drugs (DMARDs) on periodontitis. PubMed, Embase, and Web of Science were searched using the MESH terms âperiodontitisâ and ârheumatoid arthritisâ from January 2012 to September 2023. A total of 49 articles was included in the final analysis, 10 of which were randomized controlled trials. A total of 31 records concerns the effect of NSPT on parameters of RA disease activity, including a 28-joint disease activity score, anti-citrullinated protein antibodies, rheumatoid factor, C reactive protein, erythrocyte sedimentation rate, pro-inflammatory cytokines and acute phase proteins in serum, saliva, gingival crevicular fluid, and synovial fluid. A total of 18 articles investigated the effect of DMARDs on periodontal indexes and on specific cytokine levels. A quality assessment and risk-of-bias of the studies were also performed. Despite some conflicting results, there is evidence that RA patients and periodontitis patients benefit from NSPT and DMARDs, respectively. The limitations of the studies examined are the small samples and the short follow-up (usually 6 months). Further research is mandatory to evaluate if screening and treatment of periodontitis should be performed systematically in RA patients, and if the administration of DMARDs is useful in reducing the production of cytokines in the periodontium
Conservative Treatment of Dental Non-Carious Cervical Lesions: A Scoping Review
This scoping review aims to evaluate methods of conservative reconstruction of dental enamel lesions resulting from abrasions and evaluate the effect of diode laser in reducing the symptoms of tooth sensitivity. The cementoenamel junction is more prone to substance loss because the enamel thickness is substantially decreased, resulting in a much weaker enamelâdentin bond. Methods: Dental abrasion was examined in the mechanical cause alone. Pubmed, Scopus, and Web of Science were used to discover publications that matched our topic from 1 January 2018 to 20 March 2023. A comparison of various non-carious cervical lesion (NCCL) restoration treatments was generated mostly by mechanical considerations. Results: A final number of 11 clinical trials and randomized controlled trials were included in the review for qualitative analysis. Composite resins performed well in clinical trials for the restoration of NCCLs. Conclusions: Composite, in its different forms of filling and consistency, preceded by the use of adhesives, is an efficient and effective material for the treatment of NCCLs. Diode laser use prior to NCCL restoration of teeth does not diminish restoration retention rate, may lessen hypersensitivity, and may affect restoration success
Analysis of Dental Enamel Remineralization: A Systematic Review of Technique Comparisons
The demineralization process conditions the structure of the enamel and begins with a superficial decalcification procedure that makes the enamel surface porous and gives it a chalky appearance. White spot lesions (WSLs) are the first clinical sign that can be appreciated before caries evolves into cavitated lesions. The years of research have led to the testing of several remineralization techniques. This studyâs objective is to investigate and assess the various methods for remineralizing enamel. The dental enamel remineralization techniques have been evaluated. A literature search on PubMed, Scopus, and Web of Science was performed. After screening, identification, and eligibility processes 17 papers were selected for the qualitative analysis. This systematic review identified several materials that, whether used singly or in combination, can be effective in the process of remineralizing enamel. All methods have a potential for remineralization when they come into contact with tooth enamel surfaces that have early-stage caries (white spot lesions). From the studies conducted in the test, all of the substances used to which fluoride has been added contribute to remineralization. It is believed that by developing and researching new remineralization techniques, this process might develop even more successfully
Needle Fracture in Pterygomandibular Space during Loco-Regional Dental Anesthesia: Systematic Review
Background: Dentists administer hundreds of thousands of injections every day without particular concern, but the administration of local anesthetics can cause problems. One event, fortunately uncommon, that is discussed accurately but can cause significant concern is needle breakage. The purpose of this article is to review what has been reported in the literature on this topic to learn about patient symptomatology, management and possible complications. In addition, the case of a 34-year-old patient with needle persistence in the pterygoid space due to accidental rupture during inferior alveolar nerve block (IAN) was reported, for whom non-removal and monitoring over time was chosen. Materials and Methods: A literature search of the PubMed, Scopus, Web of Science and ScienceDirect databases was conducted analyzing anesthetic needle rupture during dental surgery. Results: At the end of the selection process, 17 articles resulted. Asymptomatic subjects were found, as well as those who had several symptoms. No issues were recorded by any of the authors, whether they withdrew the needle or simply monitored the patient. Conclusions: The literature on needle rupture during dental local anesthesia is scarce, and studies report conflicting results on treatment options. Most authors reported removing the fragment; however, others preferred conservative management limited to patient monitoring. No complications were reported in any study
Precision Medicine on the Effects of Microbiota on HeadâNeck Diseases and Biomarkers Diagnosis
Precision medicine using highly precise technologies and big data has produced personalised medicine with rapid and reliable diagnoses and targeted therapies. The most recent studies have directed precision medicine into the study of tumours. The application of precision medicine in the oral microbiota can be used both in the field of prevention and treatment in the strictly dental field. This article aims to evaluate the interaction between microbiota and oral cancer and the presence of biomarkers as risk predictors. Materials and Methods: A literature search of PubMed, Scopus, and Web of Science was performed analysing the various interactions between microorganisms, biomarkers, and oral cancer. Results: After screening processes, 21 articles were selected for qualitative analysis. Conclusion: The correlation between oral diseases/cancers and changes in the microbiota explains the increasing utility of precision medicine in enhancing diagnosis and adapting treatment on the individual components of the microbiota. Diagnosing and treating oral diseases and cancers through precision medicine gives, as well as economic advantages to the health care system, predictable and rapid management of the patient
Efficacy of Carbamide and Hydrogen Peroxide Tooth Bleaching Techniques in Orthodontic and Restorative Dentistry Patients: A Scoping Review
Background: Dental aesthetic procedures aim to rectify flaws in dental elementsâ shape, alignment, or overly dark color. One of the most common dental aesthetic procedures is tooth whitening. This study attempts to introduce key aspects of bleaching and describe the different techniques and the mechanism of action of bleaching agents, focusing on the clinical implications in orthodontic and on composite restorations. Methods: The research was performed on PubMed, Web of Science, and Science Direct databases for articles on our topic published between 2017 and 2023, and we found a total of 1512 studies. In total, 57 papers were considered for the qualitative analysis in the review. Results: This study found that both carbamide peroxide and hydrogen peroxide were clinically effective, although patientsâ level of tooth sensitivity seemed to be lessened by the latter. However, the latter appears to be more effective at reducing patient-experienced tooth sensitivity. Conclusion: Carbamide and hydrogen peroxide based whitening techniques were shown to be equally successful at treating tooth discoloration after bracket composite removal, with no discernible differences between them. To increase the effectiveness of whitening on composites, more research is needed. To achieve the desired results and avoid the negative effects of whitening gels on teeth and soft tissue, a patient-specific approach is advised
