    A preliminary study: aspirin discontinuation before elective operations; when is the optimal timing?

    Purpose: To evaluate the optimum timing of aspirin cessation before noncardiac surgeries. We have conducted a pilot study to minimize the aspirin cessation time before various surgeries. Methods: Eighty patients who were taking regular aspirin for secondary prevention undergoing elective surgical operations were enrolled in the study. We separated the patients into two groups. The control group had 35 patients who stopped aspirin intake 10 days before surgery. The study group had 45 patients who stopped their aspirin intake and underwent surgery one day after arachidonic acid aggregation tests were within normal limits. Bleeding, blood loss, and transfusion requirements were assessed perioperatively. Results: The mean time between aspirin cessation and aspirin nonresponsiveness were found to be 4.2 days with a median value of 4 days. In addition, the mean time between aspirin cessation and operation day were found to be 5.5 days with a median value of 5 days. No perioperative bleeding, thromboembolic or cardiovascular complications were encountered. Conclusion: Reducing time of aspirin cessation from 7-10 days to 4-5 days is a possibility for patients using aspirin for secondary prevention without increased perioperative complications

    Association of total serum antioxidant capacity with the Tei index in echocardiography in patients with microvascular angina

    Objectives Cardiac syndrome X (CSX) is a conditioncharacterized by exercise-induced chest pain that occursconsidering a normal coronary angiogram. We aimed toinvestigate the total serum antioxidant capacity (TAC) andbiventricular global functions using echocardiography inpatients with CSX.Patients and methods The study population included 55patients with typical anginal symptoms and a positiveexercise stress test, or ischemia in myocardial perfusionscintigraphy and normal coronary arteries detectedangiographically, and 49 healthy volunteers with atypicalchest pain and a negative stress test. TAC was assessedfrom blood samples. Transthoracic echocardiography wasperformed for the entire study population. The Tei index wascalculated using the formula IVCT+IVRT/ET.Results TAC was found to be significantly lower in the CSXgroup compared with the control group (0.70 ± 0.37 vs.1.5± 0.30, respectively, P1.77 mmol/l), it was found that the Tei indexwas higher in the decreased TAC group compared with theother groups (0.66 ± 0.18 vs. 0.49 ± 0.10 and0.46± 0.13 mmol/l, P<0.001, respectively).Conclusion Our study suggested that TAC wassignificantly decreased in CSX patients and decreasedantioxidant levels were related to impaired Tei index inechocardiography in patients with microvascularangina.     The effects of Covid-19 on physical medicine and rehabilitation in Turkey in the first month of pandemic

    Objectives: The outbreak of novel coronavirus-2019 (COVID-19) has affected Turkey very seriously, as well as all around the world. Many urgent and radical measures were taken due to the high contagious risk and mortality rate of the outbreak. It is noteworthy that isolation recommendations and the provision of health services for pandemic have a negative impact on Physical Medicine and Rehabilitation (PMR) services. In this study, we aimed to evaluate the effects of COVID-19 on the PMR services and physiatrists immediately after the first month of pandemic in Turkey. Patients and methods: An online survey consisting of 45 items was sent to the members of the Turkish Society of Physical Medicine and Rehabilitation. The main goal of the survey was to evaluate the changes in the provided service of PMR and conditions of physiatrists one month after the first reported COVID-19 case in Turkey. Results: A total of 606 PMR specialists and residents responded to the survey. The mean number of the patients visited the outpatient clinics was 148.2 +/- 128.5 per week before the pandemic, it significantly decreased to 23.4 +/- 33.1 per week after the first month of the reported first COVID-19 case. Similarly, the mean number of the patients of inpatient service significantly decreased from 21.7 +/- 39.3 per week to 2.5 +/- 10.0 per week after the first month of the pandemic. Most of the residents (69%) reported that their training was seriously affected due to pandemic. From the economic aspect, 69.2% of the participants who were working at private hospitals reported a decrease in their monthly salary, and 21% of them were sent to an unpaid vacation. A total of 21.9% of private-practice institutions paused their services. During the first month, 46.9% of the participants were assigned to the different services such as COVID-19 inpatient service, emergency or COVID-19 outpatient clinics. According to the Republic of Turkey, Ministry of Health guideline and algorithm, 15.7% of the physicians were in the category of healthcare workers with suspected COVID-19. Conclusion: The COVID-19 pandemic affected seriously both the services and the PMR physicians as early as the first month. This effect is expected to become worse, when the duration of pandemic prolongs. Proper arrangements and measures should be planned to ameliorate the negative effects of the pandemic on the patients and PMR physicians

    Willis-Ekbom Disease Presenting with Genital Symptoms

    Restless legs syndrome, which recently was coined as Willis-Ekbom disease (WED) is a sleep related movement disorder characterized by an indefinable sense in legs. Rarely, genital restless symptoms may be seen as isolated or accompanied by leg symptoms. Herein, we share a WED case mild leg symptoms and with severe genital symptoms affecting her social life. Genital symptoms have showed improvement by orgasm and L-dopa treatment. Therefore, WED should be considered in differential diagnosis of patients with genital symptoms fulfilling diagnostic criteria of WED defined for restless legs
