Esophageal squamous papilloma is a benign squamous epithelial polypoid tumor and is usually seen as a solitary lesion of the lower esophagus. They have a low prevalence, and about 250 cases have been reported in the literature since 1959. Chronic mucosal irritation and infection with human papilloma virus are proposed etiologies. There are no pathognomonic symptoms for the typical esophageal squamous papilloma. Our patient was a 51-yearold man who had chronic dyspeptic complaints and endoscopically detected reflux esophagitis. A 3-4 mm sized polypoid formation was seen at the 25’th cm of his esophagus during the endoscopic examination. Pathologic inspection of the biopsied specimen taken from this part of his esophagus indicated esophageal squamous papilloma

Provider selection and task allocation issues in networks with different QoS levels and all you can send pricing

We investigate an optimization problem a firmfaces when acquiring network capacity from multiple providers. We define two types of tasks the firm performs using data networks, and show that the time, bandwidth and quality requirements of each type are quite different. We formulate the associated problem as a cost minimization problem subject to quality and capacity requirements and offer multiple solution approaches. We analyze how different prices, quality and task distribution affect the optimal behavior of the firm.We also implement Generalized Bender's Decomposition to solve a relaxation of this problem in order to obtain a tight lower bound

We define two types of tasks a firm performs using data networks. We show that the demands each type of task imposes on the network and a provider’s ability to meet these demands can be quite different. We formulate the associated problem as a cost minimization problem subject to quality and capacity requirements and offer multiple solutions. We also discuss how Bender’s Decomposition can solve a relaxation of this problem

Subacutely Formed Bezoar Resulting from Accidentally Ingested Industrial Material

Bezoars are the most common foreign bodies of the gastrointestinal tract. Clinical manifestations vary depending on the location of the bezoar, from no symptoms to acute abdominal syndrome. The ingestion of cling film, which is used for preserving food, may lead to a mechanical obstruction of the gut, especially at the second portion of the duodenal segment, and could manifest with abdominal pain, epigastric distress, nausea, vomiting, and fullness. We report the case of a 72-year-old man who presented with gastric outlet obstruction after accidentally ingesting cling film. He completely recovered after it was endoscopically removed. Cling film is not toxic but has erosive effects. Endoscopic removal of such material is recommended. Moreover, psychiatric intervention and management is imperative to prevent recurrence in such cases
