Farmersâ acceptance of insects as an alternative protein source in poultry feeds
The research aimed at assessing the perceptions and willingness of poultry farmers, feed traders and processors to use insects as a source of protein ingredient in poultry feed. The research used a cross-sectional design and a structured questionnaire to collect quantitative data from 287 poultry farmers and 71 feed traders from 3 culturally diverse regions in Uganda. The study findings revealed that majority of the farmers mixed their own poultry feed. Willingness to use insects in poultry feeds was expressed by over 70% of the farmers, feed traders and processors, indicating a strong potential demand for insect-based feeds. However, some poultry farmers doubted the possibility of acquiring insects (rearing/harvesting) in large enough quantities and the consumersâ acceptance of poultry products from birds raised on insect-based feed. Nonetheless, there is a high potential for adoption of insects for use as poultry feed if they can be produced in sustainable quantities that ensure the viability of poultry farming and the feed processing businesses.

Bridging the gap: decomposing sources of gender yield gaps in Uganda groundnut production
Female plot managers in Sub-Saharan Africa often realize significantly lower crop yields than their male counterparts. Even for legumes, which are often referred to as âwomenâs cropsâ, yields are significantly lower. This study investigated the underlying causes of this gender yield gap in groundnut production. The analysis is based on survey data from 228 farm households from two groundnut growing regions in Uganda. We used the Blinder-Oaxaca model to decompose factors that contribute to this yield gap. Results show 63% and 44% gender yield gaps for improved and local varieties, respectively, with female plot managers realizing less than their male counterparts. Improved groundnut seeds increase female plot managerâs yields but not the yields of male plot managers. Male advantage and female disadvantage combined account for more than 70% of the yield gap in both improved and local groundnut variety production and exceed pure productivity differences. Labor use differences between female and male plot managers and variety types explain the observed yield gap. Interventions and policies that increase womenâs access to productive inputs including improved seed will significantly contribute to closing the yield gap, and thereby increase crop production, food security, as well as womenâs incomes

Impact of the Introduction of a Package of Diagnostic Tools, Diagnostic Algorithm, and Training and Communication on Outpatient Acute Fever Case Management at 3 Diverse Sites in Uganda: Results of a Randomized Controlled Trial.
BACKGROUND: Increasing trends of antimicrobial resistance are observed around the world, driven in part by excessive use of antimicrobials. Limited access to diagnostics, particularly in low- and middle-income countries, contributes to diagnostic uncertainty, which may promote unnecessary antibiotic use. We investigated whether introducing a package of diagnostic tools, clinical algorithm, and training-and-communication messages could safely reduce antibiotic prescribing compared with current standard-of-care for febrile patients presenting to outpatient clinics in Uganda. METHODS: This was an open-label, multicenter, 2-arm randomized controlled trial conducted at 3 public health facilities (Aduku, Nagongera, and Kihihi health center IVs) comparing the proportions of antibiotic prescriptions and clinical outcomes for febrile outpatients aged â„1 year. The intervention arm included a package of point-of-care tests, a diagnostic and treatment algorithm, and training-and-communication messages. Standard-of-care was provided to patients in the control arm. RESULTS: A total of 2400 patients were enrolled, with 49.5% in the intervention arm. Overall, there was no significant difference in antibiotic prescriptions between the study arms (relative risk [RR]: 1.03; 95% CI: .96-1.11). In the intervention arm, patients with positive malaria test results (313/500 [62.6%] vs 170/473 [35.9%]) had a higher RR of being prescribed antibiotics (1.74; 1.52-2.00), while those with negative malaria results (348/688 [50.6%] vs 376/508 [74.0%]) had a lower RR (.68; .63-.75). There was no significant difference in clinical outcomes. CONCLUSIONS: This study found that a diagnostic intervention for management of febrile outpatients did not achieve the desired impact on antibiotic prescribing at 3 diverse and representative health facility sites in Uganda

Takayasu's arteritis (pulseless disease) in Uganda
We report herein the case of a 23 year old woman who was referred to Mulago National Referral and Faculty of Medicine Makerere University Teaching Hospital because of sudden collapse, left sided weakness and headache for management. The patient underwent a battery of investigations but died five days after admission. The post mortem findings were extensive infarction the right cerebrum extending from parietal to occipital lobes. There was thickening of the wall and complete obliteration of right common carotid artery. The left common carotid artery was severely stenosed with marked thickening of the wall. The subclavian artery was thickened and completely obliterated. Microscopically there was intimal thickening by fibrous connective tissue and infiltrate of chronic inflammatory cells in the walls of the three affected branches of the oarta. These gross microscopic features were compatible with Takayasu's arteritis (TA).

Hypertensive disorders in women with peripartum cardiomyopathy: insights from the ESC EORP PPCM Registry
Aims:
Hypertensive disorders occur in women with peripartum cardiomyopathy (PPCM). How often hypertensive disorders co-exist, and to what extent they impact outcomes, is less clear. We describe differences in phenotype and outcomes in women with PPCM with and without hypertensive disorders during pregnancy.
Methods:
The European Society of Cardiology PPCM Registry enrolled women with PPCM from 2012-2018. Three groups were examined: 1) women without hypertension (âPPCM-noHTNâ); 2) women with hypertension but without pre-eclampsia (âPPCM-HTNâ); 3) women with pre-eclampsia (âPPCM-PEâ). Maternal (6-month) and neonatal outcomes were compared.
Results:
Of 735 women included, 452 (61.5%) had PPCM-noHTN, 99 (13.5%) had PPCM-HTN and 184 (25.0%) had PPCM-PE. Compared to women with PPCM-noHTN, women with PPCM-PE had more severe symptoms (NYHA IV in 44.4% and 29.9%, p<0.001), more frequent signs of heart failure (pulmonary rales in 70.7% and 55.4%, p=0.002), higher baseline LVEF (32.7% and 30.7%, p=0.005) and smaller left ventricular end diastolic diameter (57.4mm [±6.7] and 59.8mm [±8.1], p<0.001). There were no differences in the frequencies of death from any cause, re-hospitalization for any cause, stroke, or thromboembolic events. Compared to women with PPCM-noHTN, women with PPCM-PE had a greater likelihood of left ventricular recovery (LVEFâ„50%) (adjusted OR 2.08 95% CI 1.21-3.57) and an adverse neonatal outcome (composite of termination, miscarriage, low birth weight or neonatal death) (adjusted OR 2.84 95% CI 1.66-4.87).
Conclusion:
Differences exist in phenotype, recovery of cardiac function and neonatal outcomes according to hypertensive status in women with PPCM
