Endometrial Stromal Sarcoma Presenting As Puberty Menorrhagia

Endometrial stromal sarcomas are rare uterine tumours usually seen in perimenopausal females. We report here a case of low grade malignant endometrial stromal sarcoma in an adolescent girl, presenting as puberty menorrhagia. She underwent total hysterectomy with bilateral salpingo-oophorectomy and pelvic node sampling. She also received adjuvant chemotherapy and radiotherapy. She is disease free at completion of one year of follow-up

Reconsidering Economic Relations and Political Citizenship in the New Iberia of the New Europe: Some Lessons from the Fifteenth Anniversary of the Accession of Portugal and Spain to the European Union. CES Working Paper, no. 94, 2003

The purpose of this paper is to use the fifteenth anniversary of the accession of Portugal and Spain to the European Union as an opportunity to reflect on what has happened to both countries since 1986. It examines the integration process and how it has affected political, economic and social developments in Portugal and in Spain over the last fifteen years. In our view, and on balance, Spain and Portugal have benefited from accession. Since the last century, the obsession of Spanish and Portuguese reformists has been to make up the lost ground with modernized Europe. EU membership has been a critical step in this direction. The record of the past fifteen years is that this dream is becoming an economic reality. Despite impressive achievements, however, namely, since 1986, Portugal’s average per capita income has grown from 56 percent of the EU average to about 74 percent, whereas Spain’s has grown to 83 percent—both Iberian countries still have a long way to go to reach the EU average wealth. In addition, the question of Iberian and/or European citizenship, and its impact on the Portuguese and Spanish, remains open

The Effect of Pre-Primary Education on Primary School Performance

Although the theoretical case for universal pre-primary education is strong, the empirical foundation is weak. In this paper, we contribute to the empirical case by investigating the effect of a large expansion of universal pre-primary education on subsequent primary school performance in Argentina. We estimate that one year of preprimary school increases average third grade test scores by 8 percent of a mean or by 23 percent of the standard deviation of the distribution of test scores. The origin of compression influences geometric instabilities in bilayers

Geometric instabilities in bilayered structures control the surface morphology in a wide range of biological and technical systems. Depending on the application, different mechanisms induce compressive stresses in the bilayer. However, the impact of the chosen origin of compression on the critical conditions, post-buckling evolution and higher-order pattern selection remains insufficiently understood. Here, we conduct a numerical study on a finite-element set-up and systematically vary well-known factors contributing to pattern selection under the four main origins of compression: film growth, substrate shrinkage and whole-domain compression with and without pre-stretch. We find that the origin of compression determines the substrate stretch state at the primary instability point and thus significantly affects the critical buckling conditions. Similarly, it leads to different post-buckling evolutions and secondary instability patterns when the load further increases. Our results emphasize that future phase diagrams of geometric instabilities should incorporate not only the film thickness but also the origin of compression. Thoroughly understanding the influence of the origin of compression on geometric instabilities is crucial to solving real-life problems such as the engineering of smart surfaces or the diagnosis of neuronal disorders, which typically involve temporally or spatially combined origins of compression

Housing, Health and Happiness

Despite the importance of housing for people’s well-being, there is little evidence on the causal impact of housing and housing improvement programs on health and welfare. In this paper, we help to fill this gap by investigating the impact of a large-scale effort by the Mexican Government to replace dirt floors with cement floors on child health and adult happiness. We find that replacing dirt floors with cement floors significantly improves the health of young children. Specifically, we find significant decreases in the incidence of parasitic infestations, diarrhea, and the prevalence of anemia, and an improvement in children’s cognitive development. Additionally, we find that replacing dirt floors by cement floors significantly improves adult welfare, as measured by increased satisfaction with their housing and quality of life, as well as by lower scores on depression and perceived stress scales.

Fulfillment of administrative and professional obligations of hospitals and mission motivation of physicians

Background: To be successful, hospitals must increasingly collaborate with their medical staff. One strategic tool that plays an important role is the mission statement of hospitals. The goal of this research was to study the relationship between the fulfillment of administrative and professional obligations of hospitals on physicians' motivation to contribute to the mission of the hospital. Furthermore the mediating role of the physicians' emotional attachment to the hospital and moderation effect of the exchange with the head physicians were considered. Methods: Self-employed physicians of six hospitals participated in a survey. Descriptive analyses and linear regression were used to analyse the data. Results: The results indicate that affective commitment mediated the relationship between psychological contract fulfillment and mission statement motivation. In addition, the quality of exchange with the Chief Medical Officer moderated the relationship between the fulfillment of administrative obligations and affective commitment positively. Conclusion: This study extends our understanding of social exchange processes and mission statement motivation of physicians. We showed that when physicians perceive a high level of fulfillment of their psychological contract they are more committed and more motivated to contribute to the mission statement. A high quality relationship between physician and Chief Medical Officer can enhance this reciprocity dynamic
