    Antenatal depression and offspring health outcomes

    Background: Depression is the most common mental disorder during pregnancy, with prevalence rates between 4% and 20%. The objective of this review was to synthesize the literature on the association between antenatal depression and offspring birth outcomes, as well as developmental, behavioral, and psychiatric outcomes. Methods: A search of PubMed, Cochrane, and Medline databases was conducted for articles published until December 2017. Articles focusing on the effects of antenatal depression on the offspring were selected to be reviewed. Reference lists of all studies were examined for any missed articles. A total of 32 articles were included in this review. Results: Antenatal depression is associated with preterm birth, excessive infant crying, and offspring mental health problems. Untreated antenatal depression is strongly associated with adverse effects on the infant nervous system. Conclusion: Antenatal depression increases the likelihood of poor offspring health outcomes. Research should investigate whether medication use confounds this relationship

    Correlates of Lifetime and Past Month Vape Use in a Sample of Canadian University Students

    Introduction: This study examines correlates of lifetime (ie, ever vaped) and past month vape use among students aged 17 to 19 years, 20 to 25 years, and 26+ years from Western University in London, Ontario. The secondary objective is to assess lifetime and past month vape use among students not of legal age to purchase vaping products in Ontario (ie, those 17 to 18 years only). Methods: Using Qualtrics, a cross-sectional survey was sent via email to all students at Western University (N = 38 442), assessing their current and past risk-taking behaviors, mental health, sociodemographic characteristics, as well as questions pertaining to their family structure and socioeconomic background during childhood. Students were asked if they had ever vaped in their lifetime and about past month vape use. Logistic regression models were used to assess correlates of lifetime and past month vape use. Results: The sample consisted of 2626 university students. Compared to those ages 26+ years, teenage university students were 10 times more likely to have vaped in the past month and males were twice as likely as females to have vaped. Alcohol use doubled the odds of vaping in the past month, whereas cigarette, cannabis, or cocaine use approximately tripled the odds. Students who reported many sexual partners were far more likely to have vaped in the past month. Conclusion: Vape use is more common among teenage university students, males, those who drink alcohol, smoke, use cannabis or cocaine, and who report many sexual partners

    Process evaluation of fruit and vegetables distribution interventions in school-based settings: A systematic review

    Despite the importance of process evaluation in program evaluations, research has focused primarily on the effectiveness of fruit and vegetables (FVs) distribution interventions on children\u27s consumption, with little attention given to how these interventions achieve their outcomes. Five bibliographic databases (Embase, PubMed, ProQuest, Scopus, and Web of Science Core Collection) were searched in June 2019 for studies of interventions where the main focus was the implementation of distributed FVs to school-aged children as a snack. The Critical Appraisal Skills Programme (CASP) tool was used to appraise the risk of bias within included studies. Data were extracted based on study characteristics and findings. Results identified 24 studies reporting on 11 interventions and 1 policy. The findings of this systematic review indicate that the majority of the studies included limited references to implementation research. Recurring limitations include an absence of an evaluation theoretical framework and the data collection methods used. Also, several factors were identified as informing the success of snack-based FVs distribution programs, including participation of the school community, school characteristics, background knowledge, and parental engagement. Lack of timely FVs delivery, limited funding, inadequate awareness about the program, insufficient teachers’ time, and food waste were identified as challenges to successful programming. Findings indicate that distributing FVs to school-aged children as a snack can increase their consumption, but only with proper implementation. Further evaluative research is required to better inform future implementation of snack-based FV distribution interventions in school settings

    Variations in the management of pneumonia in pediatric emergency departments: Compliance with the guidelines

    Objective: We sought to assess compliance with evidence-based guidelines for the management of pediatric pneumonia, including the variations in tests ordered and antimicrobials prescribed. Our primary hypothesis was that compliance with the treatment recommendations from the most current guidelines would be low for antimicrobial prescriptions. Methods: We conducted a chart review at the Children\u27s Hospital in London, Ont., to assess variation in the management of pediatric pneumonia. All patients aged 3 months to 18 years seen at the pediatric emergency department between Apr. 1, 2006, and Mar. 31, 2007, with a diagnosis of pneumonia were eligible for inclusion in the study. Results: Compliance with management guidelines was 59.7% (95% confidence interval [CI] 53%-66%, n = 211) in children 5-18 years old and 83.0% (95% CI 80%-86%, n = 605) in children 3 months to 5 years old. Significant variation existed in the choice of antimicrobial agent for children with pneumonia, with nonrecommended agents frequently prescribed. Conclusion: Significant variation existed in the management of pediatric pneumonia, and adherence to guidelines was low for the group of patients aged 5-18 years. Future studies should attempt to provide guidance to distinguish between viral and bacterial etiology to allow judicious use of antimicrobials

    Food for thought: Comparing self-reported versus curbside measurements of household food wasting behavior and the predictive capacity of behavioral determinants

    A survey, based on an expanded Theory of Planned Behavior (TPB), was used to indirectly measure self-reported food wasting and its behavioral determinants. This was complemented with directly and objectively measured food waste in curbside garbage samples. Households (n = 189) reported throwing out avoidable food waste a mean of 5.48 times (SD = 5.58) and 6.63 portions (SD = 6.61) the week prior to completing the survey. These same households threw out a mean of 2,783 g/week of food waste (SD = 2,664) in a curbside garbage sample, with 63.27% of this consisting of avoidable food waste. There were weak to fair correlations between self-reported and curbside food waste samples. The direction and level of significance of all correlations of TPB behavioral determinants with self-reported and curbside food waste samples were similar, although the correlation coefficients were higher for self-reported food wasting. A linear regression (R2 = 0.34, p \u3c 0.001) on self-reported avoidable food waste frequency demonstrated that it was significantly (p \u3c 0.05) associated with perceived behavioral control, personal attitude, number of people per household, gender and employment status. This was contrasted with a linear regression (R2 = 0.19, p \u3c 0.001) on curbside avoidable food waste which was also significantly (p \u3c 0.05) associated with perceived behavioral control and number of people per household, but also housing tenure type (owner-occupancy vs tenancy) and the good provider identity. In general, self-reported results should be used with caution as they may underestimate food waste disposal and consideration should be given to supplement, if not replace, them with direct measurement of food waste disposal

    Reduce Food Waste, Save Money”: Testing a Novel Intervention to Reduce Household Food Waste

    An intervention, which used elements of the theory of planned behavior, was developed and tested in a randomized control trial (RCT) involving households in the city of London, Ontario, Canada. A bespoke methodology involving the direct collection and measurement of food waste within curbside garbage samples of control (n = 58) and treatment households (n = 54) was used to evaluate the effectiveness of the intervention. A comparison of garbage samples before and after the intervention revealed that total food waste in treatment households decreased by 31% after the intervention and the decrease was significantly greater (p = .02) than for control households. Similarly, avoidable food waste decreased by 30% in treatment households and was also significantly greater (p = .05) than for control households. Key determinants of treatment household avoidable food waste reduction included personal attitudes, perceived behavioral control, the number of people in a household, and the amount of garbage set out

    The quantity of food waste in the garbage stream of southern Ontario, Canada households

    There is little consensus on the amount of worldwide food waste generation because many current estimates are indirect and link back to the same limited primary datasets, with much of the data originating from fieldwork undertaken in the 1970s and 1980s. Direct measurement of waste streams, through waste composition studies, can be used to develop accurate estimates of food waste disposal. In Ontario, Canada, municipalities that undertake household waste composition studies all use a common direct measurement methodology that includes a broad range of waste categories, including food waste. The purpose of this research was to estimate the quantity of food waste disposed, in the garbage stream, by households in southern Ontario, Canada, and determine if this common methodology could be expanded and serve as the basis of a standardized and rigorous household food waste measurement methodology. Household waste composition study data (2012–2015), including a single “food waste” category, were gathered from 9 Ontario municipalities, aggregated and analyzed to develop estimates of food waste in the garbage stream. On average, households disposed 2.40 kg/week of food waste in the garbage, which comprised 35.4% of this waste stream. This does not include any food waste otherwise disposed (e.g., sink) or recycled (e.g., composted). Urban households disposed significantly greater amounts of food waste compared to rural households in the spring (p = 0.01) and summer (p = 0.02). Households with access to a green bin program disposed significantly less food waste than those with no access to a green bin program in the spring (p = 0.03) and summer (p\u3c0.01). The common methodology used to develop these estimates shows promise as the basis of a household food waste measurement methodology. This future methodology would include dividing food waste into avoidable and unavoidable food waste categories, as well as adding subcategories (e.g., avoidable fruits and vegetables)

    The Impact of Canadian School Food Programs on Children’s Nutrition and Health: A Systematic Review

    The quality of children’s diets has declined over the past few decades, giving rise to a variety of health-related consequences. In response to this trend, school food programs have become an increasingly effective method to support nutrition and lifelong healthy eating habits. This systematic review synthesizes current academic literature pertaining to school nutrition programs in Canada to identify existing interventions and their impacts on children’s nutritional knowledge, dietary behaviour, and food intake. The review was conducted through a search of the following databases: ERIC, Education Source, CINAHL, PubMed, SagePub, SCOPUS, EMBASE, and CBCA. Information extracted from the articles included the program objectives, intervention design and components, research evaluation, and primary outcomes. A total of 11 articles evaluating Canadian school nutrition programs were identified. The programs incorporated a variety of intervention components including policy, education, family and community involvement, and/or food provision. These multi-component interventions were positively associated with children’s development of nutrition knowledge, dietary behaviour changes, and intake of healthy foods; however, barriers associated with intervention duration, intensity, and availability of resources may have influenced the extent to which these programs impacted children’s diets and overall health

    Once Were Farmers: Occupation, Social Mobility, and Mortality during Industrialization in Saguenay-Lac-Saint-Jean, Quebec 1840-1971

    We study men’s adult mortality and longevity by socio-occupational status during industrialization in Saguenay-Lac-Saint-Jean, Quebec. Data were extracted from the BALSAC database (UniversitĂ© du QuĂ©bec Ă  Chicoutimi), which comprehensively traces the demographic history of the region since the beginning of the French Canadian settlement in 1840 up to the early 1970s. Using five occupational classes and controlling for year, age at marriage, urbanrural residence, and literacy, we found no evidence for the emergence of socioeconomic differentials in mortality. At least until the early 1970’s, mortality in the region appears to be driven by ‘occupational risk’ rather than ‘fundamental social causes’

    Food for naught: Using the theory of planned behaviour to better understand household food wasting behaviour

    To better understand food wasting behaviour, the theory of planned behaviour was used to inform the development of a survey which was administered to households in London, Ontario, Canada. Respondent households (n Π1,263) threw out avoidable food waste 4.77 times/week (SD Π4.81, Mdn Π4.0) and 5.89 food portions/week (SD Π5.66, Mdn Π4.0). When asked to choose one of three possible motivators to reduce food wasting behaviour, 58.9% selected reducing monetary loss as their first choice and this was significantly (p \u3c 0.001) higher than both reducing environmental impact (23.9%) and reducing social impacts (17.2%). A linear hierarchical regression analysis (R2Π0.30, p \u3c 0.001) on intention to avoid food waste demonstrated that perceived behavioural control (p \u3c 0.001) and personal norms (p \u3c 0.001) had the greatest positive impact on intention. A linear hierarchical regression analysis (R2Π0.32, p \u3c 0.001) on self-reported food wasting behaviour showed that perceived behavioural control (p \u3c 0.001) and personal attitudes (p \u3c 0.01) resulted in less food wasting behaviour, while more children in a household (p \u3c 0.01) resulted in more food wasting behaviour. Interventions that seek to strengthen perceived behavioural control and convey the monetary impact of food waste could help reduce its disposal
