Evaluation of Pre-Procedural Phone Call Effectiveness Before a Colonoscopy

Abstract Problem: Colorectal cancer is the third most diagnosed cancer in the United States (Gu et al., 2019). A colonoscopy procedure is the best diagnostic tool to evaluate the colon for pre-cancerous and cancerous polyps (Dang et al., 2020). Inadequate bowel preparation decreases colon visualization where adenomatous colon lesions may be missed. Methods: Using an observational, pre/post design, a chart review was conducted for this QI project. Quantitative data comparing Boston Bowel Preparation Scale (BBPS) scores was collected on patients who received nurse-led phone calls one month prior to a colonoscopy, and patients who received phone calls one week prior to a colonoscopy. 100 patients pre-implementation and 100 patients post-implementation were included. Additional data collected was completion of bowel preparation and adherence to clear liquids the day prior to procedure time as reported by patients. Results: An independent samples t-test was conducted to compare BBPS scores for the pre and post groups. There was a significant difference in the score with the pre group results as (M=7.2, SD=1.2) and the post group results as (M=7.6, SD=.83); t(198)= -2.66, p= .009. Implications for Practice: Pre-procedural phone calls made within one week of colonoscopy procedure in comparison with one month before procedure increases bowel preparation scores. Facilities providing colonoscopy services should implement this process into regular practice. Keywords: Colonoscopy, bowel preparation, BBPS score, nurse-led phone call

Novel drug candidates for the treatment of metastatic colorectal cancer through global inverse gene-expression profiling

Drug-induced gene-expression profiles that invert disease profiles have recently been illustrated to be a starting point for drug repositioning. In this study, we validate this approach and focus on prediction of novel drugs for colorectal cancer, for which there is a pressing need to find novel antimetastatic compounds. We computationally predicted three novel and still unknown compounds against colorectal cancer: citalopram (an antidepressant), troglitazone (an antidiabetic), and enilconazole (a fungicide). We verified the compounds by in vitro assays of clonogenic survival, proliferation, and migration and in a subcutaneous mouse model. We found evidence that the mode of action of these compounds may be through inhibition of TGF{beta} signaling. Furthermore, one compound, citalopram, reduced tumor size as well as the number of circulating tumor cells and metastases in an orthotopic mouse model of colorectal cancer. This study proposes citalopram as a potential therapeutic option for patients with colorectal cancer, illustrating the potential of systems pharmacology

Motility-related protein-1 (MRP-1/CD9) expression can predict disease-free survival in patients with squamous cell carcinoma of the head and neck

CD9 is a transmembrane protein that has been implicated in cell adhesion, motility and proliferation, and numerous studies have demonstrated the prognostic value of its expression in different solid tumours. The purpose of this study is to determine the predictive value of CD9 in squamous cell carcinoma (SCC) of the head and neck. A total of 153 cases were examined for CD9 expression using immunohistochemistry applied on formalin-fixed, paraffin-embedded tissue. Cases were stratified in two categories depending on CD9 expression, as positive (>/=50% positive cells) or reduced (<50%). In all, 108 cases were positive for CD9 (85 cases with membranous, and 23 with both membranous and cytoplasmic staining) and 45 reduced expression. Reduced CD9 expression was significantly associated with high grade (P=0.0007) and lower disease-free survival (DFS) (P=0.017). The latter retained its significance in the multivariate analysis. When the 23 cases with both membranous and cytoplasmic patterns were studied as a separate subgroup, there were significant associations between CD9 expression and tumour grade (P=0.025) (95% CI 11-68), tumour stage (P=0.08) (95% CI 3.5-86) and the occurrence of any failure (P=0.083) (95% CI -1.7-57). Immunohistochemical CD9 expression proved to be an independent prognostic factor in SCC of the head and neck, and it may detect patients at a high risk of recurrence. In addition, the cytoplasmic pattern seems to have an even more significant value. However, this finding is limited to the small number of cases with this pattern

(orig./DG)This thesis contains calculations to determine flow and efficiency. Various measurements were conducted at a radial turbine with adjustable guide blades (changes of engine pressures, speed and guide blade position). The great number of data obtained for each operating point made it possible to calculate the total losses and also to allocate those losses to the various components such as guide blades, rotor and diffusor.
