Atopiás dermatitis gyermekkorban. A családi struktúra, a lakókörnyezet, a táplálás és bakterialis superantigének szerepe. = Atopic dermatitis in childhood. The role of family structure, indoor environment, feeding and bacterial superantigens.
2002-ben az AD prevalencia Baranya megyei iskolás gyermekek körében 15,1%-nak, 2005-ben 16,1%-nak bizonyult; a 7-9 éves korosztályok adatainak összehasonlítása közel azonos eredményt adott: 17,0% (2002) és 17,1% (2005). Hazánkban az AD prevalencia emelkedése mérséklődik. A komplex kérdőíves felmérés eredményei az öröklött tényezők jelentőségét bizonyítják a betegség kialakulásában és igazolják, hogy a betegek környezetében sokkal inkább fellelhetők az ismert potenciális provokáló faktorok, mint a kontroll csoport tagjainál. Az AD-ben szenvedő betegek bőrfelszínén észlelt Staphylococcus aureus kolonizáció és a baktériumok által termelt szuperantigén tulajdonságokat mutató exotoxinok szerepe a betegség tüneteinek létrejöttében, az eredmények szerint az eddig ismert adatokhoz képest valószínűleg kisebb jelentőségű. A gondozott betegek adatai arra utalnak, hogy az ?extrinsic? és ?intrinsic? típusú AD a klinikai tünetek alapján egységes kórképnek tartható, a genetikai és a környezeti provokáló tényezőkre adott válasz tekintetében azonban különbség áll fenn közöttük. A felmérés tisztázta, hogy az AD nem tekinthető banális megbetegedésnek, igen jelentős pszichológiai, szociális és financiális terhet ró a családra és az egészségügyi ellátórendszerre. A 2003-ban és a 2004-ben az iskoláskorúak körében végzett prevalencia vizsgálatok szerint az orvos által megállapított asztma előfordulási gyakorisága 8,2%, az allergiás rhinitisé pedig 14%. | In 2002 and 2005 two surveys were conducted among schoolchildren in Baranya county to estimate the prevalence of atopic dermatitis (AD). The prevalence-rates proved to be 15,1% (2002) and 16,1% (2005). The data derived from the 7-9 year-old age group were nearly equal in the two surveys: 17,0% (2002) and 17,1% (2005). In Hungary the rate of elevation of the AD-prevalence is slowing down. The results from the complex questionnaire study represent the strong impact of inheritance on the development of AD and show the presence of some earlier proposed exogenous provocative factors more frequently in the environment of children suffering from AD than in the control group. From our investigations it can be concluded that the role of Staphylococcus aureus skin colonization and the production of staphylococcal endotoxins are not essential requirements for the development of atopic skin signs. The data of our patients suggest that the clinical symptoms of the extrinsic and intrinsic type of AD are identical but the answers given on environmental provocative factors could be different in the two subtypes. The survey revealed that AD could not be seen as a simple disease; it puts serious psychical, social and financial burden both on the family and the national health system. Our prevalence studies in 2003 and 2004 revealed 8.2% frequency for physician-diagnosed bronchial asthma while in the case of allergic rhinitis this value was 14%
A felnőttkori neuroinfekciók epidemiológiai és klinikai jellemzőinek vizsgálata a nyugat-dunántúli régióban = Evaluation of the epidemiological and clinical characteristics of central nervous system infections in adults in the West Pannonian region
Absztrakt:
Bevezetés: A diagnosztika fejlődése és az elérhető hatékony
antimikrobás szerek ellenére a neuroinfekciók továbbra is magas letalitással
járó kórképek. Célkitűzés: A retrospektív vizsgálat célja, hogy
felmérje a nyugat-dunántúli régióban 2010 és 2016 között otthon szerzett
neuroinfekciók epidemiológiai és klinikai jellemzőit. Módszer:
Életkor szerinti csoportosításban (65 év) elemeztük 176 beteg
adatait. Eredmények: 81 bakteriális, 91 virális, 1 parazitás és
3 kevert neuroinfekció fordult elő. A leggyakrabban izolált baktérium a
Streptococcus pneumoniae (20%) és a Borrelia
burgdorferi (16%) volt. A virális kórokozók közül a legtöbbször a
kullancsencephalitis-vírus (37%), majd a herpes simplex vírus (10%) és az
enterovírus (7%) fordult elő. Az összes eset 40%-ában az etiológia ismeretlen
maradt. Az incidencia átlagosan 9,8/100 000 fő/év, a letalitás 12% volt. Az
idősek körében szignifikánsan gyakrabban fordult elő az immunrendszert negatívan
befolyásoló tényező (p = 0,008), alacsony felvételi Glasgow Kóma Skála-érték (p
= 0,017) és tudatzavar (p = 0,050). Kedvezőtlen klinikai kimenetelt
prognosztizált az idős életkor (OR = 6,5 CI95%: 2,5–17,1;
p95%: 1,2–8,1; p =
0,019) és az alacsony felvételi Glasgow Kóma Skála-érték (OR = 1,6
CI95%: 1,3–1,9; p<0,001). Következtetések: A
bakteriális kórképek között továbbra is a S. pneumoniae a
vezető kórokozó. Régiónkban a kullancsencephalitis incidenciája jelentősen
magasabb az országos átlaghoz képest (2,3 versus 0,35/100 000
fő/év). Vizsgálatunk epidemiológiai és klinikai jellemzést tett lehetővé a

Abstract:
Introduction: Despite advancements in diagnostic capabilities
and the availability of effective antimicrobial agents, community-acquired
infections of the central nervous system (CNS) are still associated with high
mortality rates. Aim: To assess the epidemiological and
clinical characteristics of community-acquired CNS infections treated in the
West Pannonian region between 2010 and 2016. Method: Clinical
data of 176 patients were retrospectively analysed in two age cohorts: 15 to 65
and more than 65 years of age. Results: Neuroinfections were
found to be bacterial in 81, viral in 91, parasitic in 1 and mixed in 3 cases
during the observation period. The most frequent bacterial pathogens isolated
were Streptococcus pneumoniae (20%) and Borrelia
burgdorferi (16%). The most frequent viral pathogens isolated were
tick-borne encephalitis virus (37%), herpes simplex virus (10%) and enterovirus
(7%). Aetiology was unknown in 40 percent of all cases. The average incidence
rate was 9.8/100 000 person/year with a mortality rate of 12%. In the cohort of
patients aged >65 years, significantly higher frequencies of
immunocompromising factors, lower Glasgow Coma Scale values at admission and
confusion were observed (p-values: 0.008, 0.017, and 0.050, respectively).
Prognosis was negatively influenced by low Glasgow Coma Scale values at
admission (OR = 1.6 CI95%: 1.3–1.9; p<0.001), old age (OR = 6.5
CI95%: 2.5–17.1; p<0.001) and immunodeficiency (OR = 3.1
CI95%: 1.2–8.1; p = 0.019). Conclusions:
S. pneumoniae remains the most frequently observed causative
bacterial pathogen associated with community-acquired CNS infections. Incidence
of tick-borne encephalitis in our county is higher than the national average
(2.3 versus 0.35/100 000 person/year). Orv Hetil. 2019;

Identification and characterization of CTX-M-15 producing Klebsiella pneumoniae clone ST101 in a Hungarian university teaching hospital
We investigated the molecular epidemiology of extended spectrum β-lactamase (ESBL) producing Klebsiella pneumoniae isolates derived from the teaching hospitals of University of Pécs, Pécs, Hungary in the time period 2004–2008. Molecular typing, antimicrobial susceptibility testing, detection of common β-lactamase genes (blaCTX-M, blaTEM and blaSHV) and virulence associated traits (hypermucoviscosity, magA, k2a, rmpA, siderophores, type 1 and 3 fimbria, biofilm formation, serum resistance) were performed for 102 isolates. The results showed the presence of three major ciprofloxacin resistant CTX-M-15 producing clones (ST15 n = 69, ST101 n = 10, and ST147 n = 9), of which ST15 was predominant and universally widespread. Considering distribution in time and place, ST101 and ST147 were detected at fewer inpatient units and within a narrower time frame, as compared to ST15. Beside major clones, eleven minor clones were identified, and were shown to harbour the following β-lactamase genes: six clones carried blaCTX-M, four clones harboured blaSHV-5 and one clone possessed both blaCTX-M and ESBL type blaSHV. Among the SHV-5 producing K. pneumoniae clones a novel sequence type was found, namely ST1193, which harboured a unique infB allele. Different virulence factor content and peculiar antimicrobial susceptibility profile were characteristic for each clone. In contrast to major clone isolates, which showed high level resistance to ciprofloxacin, minor clone isolates displayed significantly lower MIC values for ciprofloxacin suggesting a role for fluoroquinolones in the dissemination of the major K. pneumoniae clones. Survival Comes at a Cost: A Coevolution of Phage and Its Host Leads to Phage Resistance and Antibiotic Sensitivity of Pseudomonas aeruginosa Multidrug Resistant Strains
The increasing ineffectiveness of traditional antibiotics and the rise of multidrug resistant (MDR) bacteria have necessitated the revival of bacteriophage (phage) therapy. However, bacteria might also evolve resistance against phages. Phages and their bacterial hosts coexist in nature, resulting in a continuous coevolutionary competition for survival. We have isolated several clinical strains of Pseudomonas aeruginosa and phages that infect them. Among these, the PIAS (Phage Induced Antibiotic Sensitivity) phage belonging to the Myoviridae family can induce multistep genomic deletion in drug-resistant clinical strains of P. aeruginosa, producing a compromised drug efflux system in the bacterial host. We identified two types of mutant lines in the process: green mutants with SNPs (single nucleotide polymorphisms) and smaller deletions and brown mutants with large (∼250 kbp) genomic deletion. We demonstrated that PIAS used the MexXY-OprM system to initiate the infection. P. aeruginosa clogged PIAS phage infection by either modifying or deleting these receptors. The green mutant gaining phage resistance by SNPs could be overcome by evolved PIASs (E-PIASs) with a mutation in its tail-fiber protein. Characterization of the mutant phages will provide a deeper understanding of phage-host interaction. The coevolutionary process continued with large deletions in the same regions of the bacterial genomes to block the (E-)PIAS infection. These mutants gained phage resistance via either complete loss or substantial modifications of the phage receptor, MexXY-OprM, negating its essential role in antibiotic resistance. In vitro and in vivo studies indicated that combined use of PIAS and antibiotics could effectively inhibit P. aeruginosa growth. The phage can either eradicate bacteria or induce antibiotic sensitivity in MDR-resistant clinical strains. A hepatitis C-vírus-fertőzés kezelése a rendelkezésre álló gyógyszereken és
finanszírozási lehetőségeken alapuló, félévenként megújított hazai
konszenzusajánlás alapján, átláthatóságot biztosító szervezeti körülmények
között zajlik Magyarországon. Ebben az aktualizált ajánlásban új, speciális
szempontok jelennek meg, egyebek között a szűrés, a diagnosztika, a legújabban
elfogadott terápiák és allokációjuk vonatkozásában. A kezelés indikációja
(ellenjavallat hiányában) a vírusszaporodás és az ezzel kapcsolatos
májsejtkárosodás és/vagy fibrosis kimutatásán alapul, amelynek kimutatására a
nem invazív (elasztográfiás vagy biokémiai) fibrosis módszerek kerültek
előtérbe. A betegség kezelésére fordítható terápiás keret limitált. Emiatt az
új, direkt ható antivirális kombinációk első kezelésként csak olyan betegek
esetében alkalmazhatók, akiknél a korlátozás nélkül elérhető, de kevésbé hatásos
és biztonságos pegilált interferonból és ribavirinből álló terápia
mellékhatásának lehetősége magas és/vagy a tartós vírusmentesség elérésének
esélye előre láthatóan alacsony. A kezelésbe vonás sorrendjét a fibrosis
stádiumán alapuló prioritási pontrendszer határozza meg, amely figyelembe veszi
a májelégtelenséget vagy egyéb komplikációkat, illetve ezek közvetlen veszélyét,
a májkárosodás aktivitását és progresszióját, az átvitel kockázatát, valamint
más meghatározott speciális szempontokat is. A kezelést végzők, a finanszírozó
Országos Egészségbiztosítási Pénztár és a betegszervezetek konszenzusával a
betegcsoportonként választható gyógyszereket azok költséghatékonysága határozza
meg, az egy vírusmentessé vált betegre jutó gyógyszerköltség és a vírusmentessé
nem vált betegekre fordítandó várható többletköltség mérlegelésével. Szakmai
szempontból előnyt élveznek az interferonmentes kezelések, valamint a rövidebb

Abstract
Treatment of hepatitis C is based on a national consensus guideline updated
six-monthly according to local availability and affordability of approved
therapies through a transparent allocation system in Hungary. This updated
guideline incorporates some special new aspects, including recommendations for
screening, diagnostics, use and allocation of novel direct acting antiviral
agents. Indication of therapy in patients with no contraindication is based on
demonstration of viral replication with consequent inflammation and/or fibrosis
in the liver. Non-invasive methods (elastographies and biochemical methods) are
preferred for liver fibrosis staging. The budget allocated for these patients is
limited. Therefore, expensive novel direct acting antiviral combinations as
first line treatment are reimbursed only, if the freely available, but less
effective and more toxic pegylated interferon plus ribavirin dual therapy deemed
to prone high chance of adverse events and/or low chance of cure. Priority is
given to those with urgent need based on a pre-defined scoring system reflecting
mainly the stage of the liver disease, but considering also additional factors,
i.e., hepatic decompensation, other complications, activity and progression of
liver disease, risk of transmission and other special issues. Approved
treatments are restricted to the most cost-effective combinations based on the
cost per sustained virological response value in different patient categories
with consensus amongst treating physicians, the National Health Insurance Fund
and patient’s organizations. Interferon-free treatments and shorter therapy
