The diagnostic value of ultrasonography-derived edema of the temporal artery wall in giant cell arteritis: a second meta-analysis
<p>Abstract</p> <p>Background</p> <p>Ultrasonography of temporal arteries is not commonly used in the approach of patients with suspected giant cell arteritis (GCA) in clinical practice. A meta-analysis of primary studies available through April 2004 concluded that ultrasonography could indeed be helpful in diagnosing GCA. We specifically re-examined the diagnostic value of the ultrasonography-derived halo sign, a dark hypoechoic circumferential thickening around the artery lumen, indicating vasculitic wall edema, in GCA.</p> <p>Methods</p> <p>Original, prospective studies in patients with suspected GCA that examined ultrasonography findings of temporal arteries using the ACR 1990 classification criteria for GCA as reference standard, published through 2009, were identified. Only eight studies involving 575 patients, 204 of whom received the final diagnosis of GCA, fulfilled technical quality criteria for ultrasound. Weighted sensitivity and specificity estimates of the halo sign were assessed, their possible heterogeneity was investigated and pooled diagnostic odds ratio was determined.</p> <p>Results</p> <p>Unilateral halo sign achieved an overall sensitivity of 68% (95% CI, 0.61-0.74) and specificity of 91% (95% CI, 0.88-0.94) for GCA. The values of inconsistency coefficient (I<sup>2</sup>) of both sensitivity and specificity of the halo sign, showed significant heterogeneity concerning the results between studies. Pooled diagnostic odds ratio, expressing how much greater the odds of having GCA are for patients with halo sign than for those without, was 34 (95% CI, 8.21-138.23). Diagnostic odds ratio was further increased to 65 (95% CI, 17.86-236.82) when bilateral halo signs were present (sensitivity/specificity of 43% and 100%, respectively). In both cases, it was found that DOR was constant across studies.</p> <p>Conclusion</p> <p>Temporal artery edema demonstrated as halo sign should be always looked for in ultrasonography when GCA is suspected. Providing that currently accepted technical quality criteria are fulfilled, halo sign's sensitivity and specificity are comparable to those of autoantibodies used as diagnostic tests in rheumatology. Validation of revised GCA classification criteria which will include the halo sign may be warranted.</p
Plasma Homeostasis and Cloacal Urine Composition in Crocodylus porosus Caught Along a Salinity Gradient
Juveniles of the Estuarine or Saltwater Crocodile, Crocodylus porosus, maintain both osmotic pressure and plasma electrolyte homeostasis along a salinity gradient from fresh water to the sea. In fresh water (FW) the cloacal urine is a clear solution rich in ammonium and bicarbonate and containing small amounts of white precipitated solids with high concentrations of calcium and magnesium. In salt water (SW) the cloacal urine has a much higher proportion of solids, cream rather than white in colour, which are the major route for excretion of potassium in addition to calcium and magnesium. Neither liquid nor solid fractions of the cloacal urine represent a major route for excretion of sodium chloride. The solids are urates and uric acid, and their production probably constitutes an important strategy for water conservation by C. porosus in SW. These data, coupled with natural history observations and the recent identification of lingual salt glands, contribute to the conclusion that C. porosus is able to live and breed in either fresh or salt water and may be as euryhaline as any reptile
Parameterized tests of the strong-field dynamics of general relativity using gravitational wave signals from coalescing binary black holes: Fast likelihood calculations and sensitivity of the method
Thanks to the recent discoveries of gravitational wave signals from binary
black hole mergers by Advanced Laser Interferometer Gravitational Wave
Observatory and Advanced Virgo, the genuinely strong-field dynamics of
spacetime can now be probed, allowing for stringent tests of general relativity
(GR). One set of tests consists of allowing for parametrized deformations away
from GR in the template waveform models and then constraining the size of the
deviations, as was done for the detected signals in previous work. In this
paper, we construct reduced-order quadratures so as to speed up likelihood
calculations for parameter estimation on future events. Next, we explicitly
demonstrate the robustness of the parametrized tests by showing that they will
correctly indicate consistency with GR if the theory is valid. We also check to
what extent deviations from GR can be constrained as information from an
increasing number of detections is combined. Finally, we evaluate the

Intraocular pressure and ocular pulse amplitude using dynamic contour tonometry and contact lens tonometry
BACKGROUND: The new Ocular Dynamic Contour Tonometer (DCT), investigational device supplied by SMT (Swiss Microtechnology AG, Switzerland) allows simultaneous recording of intraocular pressure (IOP) and ocular pulse amplitude (OPA). It was the aim of this study to compare the IOP results of this new device with Goldmann tonometry. Furthermore, IOP and OPA measured with the new slitlamp-mounted DCT were compared to the IOP and OPA measured with the hand-held SmartLens(®), a gonioscopic contact lens tonometer (ODC Ophthalmic Development Company AG, Switzerland). METHODS: Nineteen healthy subjects were included in this study. IOP was determined by three consecutive measurements with each of the DCT, SmartLens(®), and Goldmann tonometer. Furthermore, OPA was measured three times consecutively by DCT and SmartLens(®). RESULTS: No difference (P = 0.09) was found between the IOP values by means of DCT (mean: 16.6 mm Hg, median: 15.33 mm Hg, SD: +/- 4.04 mm Hg) and Goldmann tonometry (mean: 16.17 mm Hg, median: 15.33 mm Hg, SD: +/- 4.03 mm Hg). The IOP values of SmartLens(® )(mean: 20.25 mm Hg, median: 19.00 mm Hg, SD: +/- 4.96 mm Hg) were significantly higher (P = 0.0008) both from Goldmann tonometry and DCT. The OPA values of the DCT (mean: 3.08 mm Hg, SD: +/- 0.92 mm Hg) were significantly lower (P = 0.0003) than those obtained by SmartLens(® )(mean: 3.92 mm Hg, SD: +/- 0.83 mm Hg). CONCLUSIONS: DCT was equivalent to Goldmann applanation tonometry in measurement of IOP in a small group of normal subjects. In contrast, SmartLens(® )(contact lens tonometry) gave IOP readings that were significantly higher compared with Goldmann applanation tonometer readings. Both devices, DCT and SmartLens(® )provide the measurement of OPA which could be helpful e.g. for the management of glaucoma
Profilin modulates sarcomeric organization and mediates cardiomyocyte hypertrophy
Aims: Heart failure is often preceded by cardiac hypertrophy, which is characterized by increased cell size, altered protein abundance, and actin-cytoskeletal reorganization. Profilin is a well-conserved, ubiquitously expressed, multi-functional actin-binding protein, whose role in cardiomyocytes is largely unknown. Given its involvement in vascular hypertrophy, we aimed to test the hypothesis that profilin-1 is a key mediator of cardiomyocyte-specific hypertrophic remodeling. Methods and Results: Profilin-1 was elevated in multiple mouse models of hypertrophy, and a cardiomyocyte-specific increase of profilin in Drosophila resulted in significantly larger heart tube dimensions. Moreover, adenovirus-mediated overexpression of profilin-1 in neonatal rat ventricular myocytes (NRVMs) induced a hypertrophic response, measured by increased myocyte size and gene expression. Profilin-1 silencing suppressed the response in NRVMs stimulated with phenylephrine or endothelin-1. Mechanistically, we found that profilin-1 regulates hypertrophy, in part, through activation of the ERK1/2 signaling cascade. Confocal microscopy showed that profilin localized to the Z-line of Drosophila myofibrils under normal conditions and accumulated near the M-line when overexpressed. Elevated profilin levels resulted in elongated sarcomeres, myofibrillar disorganization, and sarcomeric disarray, which correlated with impaired muscle function. Conclusion: Our results identify novel roles for profilin as an important mediator of cardiomyocyte hypertrophy. We show that overexpression of profilin is sufficient to induce cardiomyocyte hypertrophy and sarcomeric remodeling, and silencing of profilin attenuates the hypertrophic response
SN 2005hj: Evidence for Two Classes of Normal-Bright SNe Ia and Implications for Cosmology
HET Optical spectra covering the evolution from about 6 days before to about
5 weeks after maximum light and the ROTSE-IIIb unfiltered light curve of the
"Branch-normal" Type Ia Supernova SN 2005hj are presented. The host galaxy
shows HII region lines at redshift of z=0.0574, which puts the peak unfiltered
absolute magnitude at a somewhat over-luminous -19.6. The spectra show weak and
narrow SiII lines, and for a period of at least 10 days beginning around
maximum light these profiles do not change in width or depth and they indicate
a constant expansion velocity of ~10,600 km/s. We analyzed the observations
based on detailed radiation dynamical models in the literature. Whereas delayed
detonation and deflagration models have been used to explain the majority of
SNe Ia, they do not predict a long velocity plateau in the SiII minimum with an
unvarying line profile. Pulsating delayed detonations and merger scenarios form
shell-like density structures with properties mostly related to the mass of the
shell, M_shell, and we discuss how these models may explain the observed SiII
line evolution; however, these models are based on spherical calculations and
other possibilities may exist. SN 2005hj is consistent with respect to the
onset, duration, and velocity of the plateau, the peak luminosity and, within
the uncertainties, with the intrinsic colors for models with M_shell=0.2 M_sun.
Our analysis suggests a distinct class of events hidden within the
Branch-normal SNe Ia. If the predicted relations between observables are
confirmed, they may provide a way to separate these two groups. We discuss the
implications of two distinct progenitor classes on cosmological studies
employing SNe Ia, including possible differences in the peak luminosity to

Do Search for Dibaryonic De - Excitations in Relativistic Nuclear Reactions
Some odd characteristics are observed in the single particle distributions
obtained from interactions at momenta which are
explained as the manifestation of a new mechanism of strangeness production via
dibaryonic de-excitations. A signature of the formation of hadronic and
baryonic clusters is also reported. The di-pionic signals of the dibaryonic
orbital de-excitations are analyzed in the frame of the MIT - bag Model and a
Monte Carlo simulation.The role played by the dibaryonic resonances in
relativistic nuclear collisions could be a significant one.
