    Use of Intradialytic Parenteral Nutrition in Patients Undergoing Hemodialysis

    Intradialytic parenteral nutrition (IDPN) is a form of supplemental nutrition used to treat patients with malnutrition who receive hemodialysis. Once the diagnosis of malnutrition is made in such patients, encouragement of oral intake is the first‐line treatment. If this fails, then enteral or parenteral nutrition may be needed. This review examines the literature on the use of IDPN and summarizes the current recommendations. There is considerable controversy over indications and benefits of IDPN, and well‐controlled, long‐term studies are needed to help tease out these issues. In the interim, clinical judgment should be used when considering IDPN for individual patients

    Today, the treatment of osteoarthritis in the rotator cuff-deficient population is largely dominated by reverse shoulder arthroplasty (RSA). Despite the popularity of and increased familiarity with this procedure, the complication rate of RSA remains significant. An extended humeral head hemiarthroplasty may provide a less invasive alternative for select patients with cuff tear arthropathy (CTA) and preserved glenohumeral active elevation. With the indications for reverse arthroplasty expanding to younger patients, there are concerns about the longevity of this implant, as well as the associated revision burden. In the setting of failed RSA, the bone stock available for glenosphere baseplate fixation can be inadequate for reimplantation. The treatment strategies for complex shoulder deformities and failed RSA are limited by patient-specific issues, such as anatomy and risk factors. In this review, we discuss the potential role of extended humeral head hemiarthroplasty (CTA hemiarthroplasty) as a primary surgical option in select patients (1) who have preserved elevation \u3e 90°, (2) who have maintained stability (intact coracoacromial ligament), and (3) who desire to circumvent the complications associated with RSA. Furthermore, CTA hemiarthroplasty may be used for severe glenoid erosion, for a fragmented acromion, and in the revision setting for failed RSA aimed at a reliable salvage procedure

    The contemporary world requires consolidated organizations and institutions with the capacity to adequately fulfill its mission and vision, in which human resource plays a crucial role. Each organization has their uniqueness in organizational culture, it can be managed according to the strategic plan of the same. It is noteworthy that the organizational culture is not formed through speeches, but through strong actions. Similarly, the level of satisfaction of the people who work at the institution, public or private, is an aspect not missed in modern management, while its action depends on the achievement of organizational objectives. The study addressed the relationship between organizational culture and job satisfaction of teachers establishment of the Faculty of Mechanical Engineering of San Marcos, the same which looked upon the recognition of the state of research related to the problem of art, theoretical elaboration of each of the variables , arriving at their instruments measuring constructs. A direct relationship between the two variables was found.El mundo contemporáneo requiere de organizaciones e instituciones consolidadas, con capacidad para alcanzar adecuadamente su misión y visión, en donde el recurso humano tiene un rol fundamental. Cada organización presenta singularidad en su cultura organizacional, la misma que puede ser gestionada en función del plan estratégico de la misma. Cabe mencionar que la cultura organizacional no se forma mediante discursos, sino mediante acciones sólidas. De igual manera, el nivel de satisfacción de las personas que laboran en un institución, pública o privada, es un aspecto que no se soslaya en la administración moderna, en tanto depende de su acción el logro de los objetivos organizacionales. El estudio abordó la relación entre cultura organizacional y satisfacción laboral en el estamento docente de la facultad de ingeniería industrial de la UNMSM, el mismo que contempló el reconocimiento del estado del arte relacionado al problema de investigación, la elaboración teórica de cada una de las variables, hasta arribar a sendos instrumentos de medición de los constructos.     Evaluating Molecular Mechanical Potentials for Helical Peptides and Proteins

    Multiple variants of the AMBER all-atom force field were quantitatively evaluated with respect to their ability to accurately characterize helix-coil equilibria in explicit solvent simulations. Using a global distributed computing network, absolute conformational convergence was achieved for large ensembles of the capped A21 and Fs helical peptides. Further assessment of these AMBER variants was conducted via simulations of a flexible 164-residue five-helix-bundle protein, apolipophorin-III, on the 100 ns timescale. Of the contemporary potentials that had not been assessed previously, the AMBER-99SB force field showed significant helix-destabilizing tendencies, with beta bridge formation occurring in helical peptides, and unfolding of apolipophorin-III occurring on the tens of nanoseconds timescale. The AMBER-03 force field, while showing adequate helical propensities for both peptides and stabilizing apolipophorin-III, (i) predicts an unexpected decrease in helicity with ALA→ARG+ substitution, (ii) lacks experimentally observed 310 helical content, and (iii) deviates strongly from average apolipophorin-III NMR structural properties. As is observed for AMBER-99SB, AMBER-03 significantly overweighs the contribution of extended and polyproline backbone configurations to the conformational equilibrium. In contrast, the AMBER-99φ force field, which was previously shown to best reproduce experimental measurements of the helix-coil transition in model helical peptides, adequately stabilizes apolipophorin-III and yields both an average gyration radius and polar solvent exposed surface area that are in excellent agreement with the NMR ensemble

    The neonatal Fc receptor (FcRn), also known as the Brambell receptor and encoded by Fcgrt, is a MHC class I like molecule that functions to protect IgG and albumin from catabolism, mediates transport of IgG across epithelial cells, and is involved in antigen presentation by professional antigen presenting cells. Its function is evident in early life in the transport of IgG from mother to fetus and neonate for passive immunity and later in the development of adaptive immunity and other functions throughout life. The unique ability of this receptor to prolong the half-life of IgG and albumin has guided engineering of novel therapeutics. Here, we aim to summarize the basic understanding of FcRn biology, its functions in various organs, and the therapeutic design of antibody- and albumin-based therapeutics in light of their interactions with FcRn
