    Outcomes of outpatient ureteral stenting without fluoroscopy at Groote Schuur Hospital, Cape Town, South Africa

    Background. Ureteral stenting is generally a theatre-based procedure that requires a multidisciplinary team and on-table imaging. Limited hospital bed numbers and theatre time in our centre in Cape Town, South Africa, have led us to explore an alternative approach.Objectives. To see whether outpatient insertion of ureteric stents under local anaesthesia without fluoroscopy was a possible and acceptable alternative to theatre-based ureteral stenting.Methods. Ureteral stenting (double-J stents and ureteric catheters) was performed with flexible cystoscopy under local anaesthesia and chemoprophylaxis, but without fluoroscopic guidance, in an outpatient setting. Every patient had an abdominal radiograph and an ultrasound scan of the kidney after the procedure to confirm stent position.Results. Three hundred and sixteen procedures (276 double-J stents and 40 ureteric catheters) were performed in 161 men and 155 women. The overall success rate for the procedures was 85.4%, independent of gender (p=0.87), age (p=0.13), type of device inserted (p=0.81) or unilateral/bilateral nature of the procedure (p=1.0). Procedures with a successful outcome were performed in a significantly (p<0.0001) shorter median time (10 minutes (interquartile range (IQR) 5 - 15)) than failed procedures (20 minutes (IQR 10 - 30)). Patients with a pain score of >5 experienced a significantly (p=0.02) greater proportion of failure (27.3%) than patients with a pain score of ≤5 (12.5%). Difficulties were encountered in 23.7% of procedures, with a significantly higher proportion being registered in failed interventions compared with successful ones (82.6% v. 13.7%; p<0.0001).Conclusions. The procedure was easily mastered and technically simple, and represents savings in cost, time and human resources in our setting.

    Microbiological assessment of food crops irrigated with domestic greywater

    Two of the challenges facing Africa in the 21stcentury are effective use of restricted water resources and ensuring food security. The eThekwini Municipality is not exempt from these challenges and is investigating ways in which they can be addressed in the local context. One of the aims of national government has been to ensure the supply of clean drinking water and suitable sanitation to every household. As part of its efforts in this regard, eThekwini Municipality has introduced a multi-tiered water supply programme ranging from full-pressure water systems and flush toilets to stand pipes and dry toilets. In the dry sanitation systems, there is no real provision for the disposal of domestic greywater and, bearing in mind that South Africa is a water scarce country, the municipality is investigating ways of using this water as a resource rather than viewing it as a waste. A preliminary trial was set up in 2003 using this water to irrigate vegetables to be used for home consumption. The microbiological safety of this practice needed to be examined to ensure that it would indeed help to uplift communities by the provision of healthy food as well as provide an ecologically sound use and reuse of available water. The initial proposal was to investigate total coliforms, E. coli and Enterococcus as the normal faecal indicators for health purposes as well as Staphylococcus as a human skin surface micro-organism and coliphage as a viral indicator. The first controlled field trial was put in at the test site at the University of KwaZulu-Natal in 2005 using several crop types and three irrigation regimes. The vegetable samples were examined both internally and externally for the selected micro-organisms. It was found that very few E. coli were detected and in later crops the micro-organism list was expanded and further identification of the total coliform organisms detected is being addressed.     Inflammatory metabolic profile of South African patients with prostate cancer.

    Men with African ancestry are more likely to develop aggressive prostate cancer (PCa) and to die from this disease. The study of PCa in the South African population represents an opportunity for biomedical research due to the high prevalence of aggressive PCa. While inflammation is known to play a significant role in PCa progression, its association with tumor stage in populations of African descent has not been explored in detail. Identification of new metabolic biomarkers of inflammation may improve diagnosis of patients with aggressive PCa. Plasma samples were profiled from 41 South African men with PCa using nuclear magnetic resonance (NMR) spectroscopy. A total of 41 features, including metabolites, lipid classes, total protein, and the inflammatory NMR markers, GlycA, and GlycB, were quantified from each NMR spectrum. The Bruker's B.I.-LISA protocols were used to characterize 114 parameters related to the lipoproteins. The unsupervised KODAMA method was used to stratify the patients of our cohort based on their metabolic profile. We found that the plasma of patients with very high risk, aggressive PCa and high level of C-reactive protein have a peculiar metabolic phenotype (metabotype) characterized by extremely high levels of GlycA and GlycB. The inflammatory processes linked to the higher level of GlycA and GlycB are characterized by a deep change of the plasma metabolome that may be used to improve the stratification of patients with PCa. We also identified a not previously known relationship between high values of VLDL and low level of GlycB in a different metabotype of patients characterized by lower-risk PCa. For the first time, a portrait of the metabolic changes in African men with PCa has been delineated indicating a strong association between inflammation and metabolic profiles. Our findings indicate how the metabolic profile could be used to identify those patients with high level of inflammation, characterized by aggressive PCa and short life expectancy. Integrating a metabolomic analysis as a tool for patient stratification could be important for opening the door to the development of new therapies. Further investigations are needed to understand the prevalence of an inflammatory metabotype in patients with aggressive PCa
