Correlation of Atrial Fibrillation with Left Atrial Volume in Patients with Mitral Stenosis. a Single Centre Study From Pakistan
Background: Rheumatic heart disease has a strong association with mitral valve stenosis. Atrial fibrillation is one of the most common complications of this condition and is a poor prognostic factor. Early detection and prompt management of atrial fibrillation can help to improve the quality of life and increase the life expectancy of the patients. We carried out this study to investigate the significance of left atrial volumetric changes in mitral stenosis and its correlation with atrial fibrillation.
Methodology: We audited the data of 60 patients of rheumatic heart disease who had mitral valve stenosis. The patients were randomized into atrial fibrillation (Group A) and normal sinus rhythm (Group B). We conducted this cross-sectional analytical study at Cardiology Department, Mayo Hospital, Lahore, from 1st February 2017 to 31st January 2018. We only included those patients who consented to be a part of this study and fulfilled our predefined inclusion criteria. Left atrial volume was measured by prolate ellipse method and biplane methods on echocardiography. The Data was analyzed on SPSS v20.
Results: Sixty patients were included in the study. Among the subjects, thirty-six (60%) were males, and twenty-four (40%) were females. Atrial fibrillation was noted in 43.33% of the patients of mitral valve stenosis. There was a marked difference in the mean volume of the left atrium among the two groups. We observed that the mean area of the mitral valve for Group A patients was larger than that of patients in Group B. Our study showed an inverse correlation between left atrial volume and mitral valve area among Group A patients.
Conclusion: Patients of mitral stenosis are at an increased risk of developing atrial fibrillation if the left atrial volume is increasing. All patients with mitral stenosis should have routine echocardiography & measurement of left atrial volumes, so that proper treatment can be started if the left atrial volume is increasing, to prevent atrial fibrillation
NMR and SR detection of unconventional spin dynamics in Er(trensal) and Dy(trensal) molecular magnets
Measurements of proton Nuclear Magnetic Resonance (1H NMR) spectra and
relaxation and of Muon Spin Relaxation (SR) have been performed as a
function of temperature and external magnetic field on two isostructural
lanthanide complexes, Er(trensal) and Dy(trensal) featuring
crystallographically imposed trigonal symmetry. Both the nuclear 1/T1 and muon
longitudinal relaxation rates, LRR, exhibit a peak for temperatures T
lower than 30K, associated to the slowing down of the spin dynamics, and the
width of the NMR absorption spectra starts to increase significantly at T ca.
50K, a temperature sizably higher than the one of the LRR peaks. The LRR peaks
have a field and temperature dependence different from those previously
reported for all Molecular Nanomagnets. They do not follow the
Bloembergen-Purcell-Pound scaling of the amplitude and position in temperature
and field and thus cannot be explained in terms of a single dominating
correlation time c determined by the spin slowing down at low
temperature. Further, for T lower than 50K the spectral width does not follow
the temperature behavior of the magnetic susceptibility chi. We suggest, using
simple qualitative considerations, that the observed behavior is due to a
combination of two different relaxation processes characterized by the
correlation times LT and HT, dominating for T lower than 30K and T
higher than 50K, respectively. Finally, the observed flattening of LRR for T
lower than 5K is suggested to have a quantum origin

The Inflationary Perturbation Spectrum
Motivated by the prospect of testing inflation from precision cosmic
microwave background observations, we present analytic results for scalar and
tensor perturbations in single-field inflation models based on the application
of uniform approximations. This technique is systematically improvable,
possesses controlled error bounds, and does not rely on assuming the slow-roll
parameters to be constant. We provide closed-form expressions for the power
spectra and the corresponding scalar and tensor spectral indices.Comment: 4 pages, 1 figur
Nanoscale layering of antiferromagnetic and superconducting phases in Rb2Fe4Se5
We studied phase separation in a single-crystalline antiferromagnetic
superconductor Rb2Fe4Se5 (RFS) using a combination of scattering-type scanning
near-field optical microscopy (s-SNOM) and low-energy muon spin rotation
(LE-\mu SR). We demonstrate that the antiferromagnetic and superconducting
phases segregate into nanometer-thick layers perpendicular to the iron-selenide
planes, while the characteristic in-plane size of the metallic domains reaches
10 \mu m. By means of LE-\mu SR we further show that in a 40-nm thick surface
layer the ordered antiferromagnetic moment is drastically reduced, while the
volume fraction of the paramagnetic phase is significantly enhanced over its
bulk value. Self-organization into a quasiregular heterostructure indicates an
intimate connection between the modulated superconducting and antiferromagnetic
phases.Comment: 5 pages, 2 figures. Updated version published in Phys. Rev. Lett. on
