    Study prevalence of peripheral arterial disease in diabetics with coronary artery disease at a large tertiary care teaching hospital in North India

    Background: Peripheral arterial disease and coronary artery disease have similar factors. The extent and severity of PAD is significantly associated with presence and severity of CAD.Methods: Patients were interviewed and clinical profile of patients including risk factors of coronary artery disease like diabetes, hypertension, smoking, dyslipidemia, obesity and family history of coronary artery disease were assessed and recorded in the proforma. Information regarding demographics, co morbidity, past history and family history was collected. Physical examination of each patient was carried and basic anthropometric data: height, weight, BMI and blood pressure, peripheral pulses calculated. Investigations both non- invasive and invasive including peripheral angiography were recorded.Results: In this prospective study fifty (50) type 2 diabetes patients admitted with CAD were studied whose mean age was 55.6+8.2 and mean duration of diabetes was 6.8+8.4. In this study none of the patients with PAD had single atherosclerosis risk factors including diabetes. 30% patients were having two risk factors. PAD in in patients with CAD is particularly enhanced by the concomitant occurrence of two or more of these risk factors (p=0.016). Hypertension as a predictor of PAD was statistically significant (p=0.0037). In this study the duration of diabetes was 10 years in 40% of patients with angiographically proven PAD.Conclusions: It was observed that presence and severity CAD was significantly associated with PAD

    A trial sequential meta-analysis of TNF-α –308G\u3eA (rs800629) gene polymorphism and susceptibility to colorectal cancer

    © 2019 The Author(s). Purpose: Tumor necrosis factor-α (TNF-α), secreted by the activated macrophages, may participate in the onset and progression of colorectal cancer (CRC). The association of TNF-α –308 G\u3eA (rs1800629) single-nucleotide polymorphism (SNP) with CRC risk has been investigated by many studies but the results are inconclusive. A trial sequential meta-analysis was performed for precise estimation of the relationship between TNF-α –308 G\u3eA gene polymorphism with CRC risk. Methods: Medline (PubMed), EMBASE (Excerpta-Medica) and Google Scholar were mined for relevant articles. Odds ratios (ORs) and 95% confidence intervals (CIs) were calculated to estimate the significance of association. Results: The pooled analysis indicated no risk associated with TNF-α –308 G\u3eA SNP and overall CRC risk in five genetic comparison models, i.e. allelic (A vs. G: P = 0.524; OR = 1.074, 95% CI = 0.863–1.335), homozygous (AA vs. GG: P = 0.489; OR = 1.227, 95% CI = 0.688–2.188), heterozygous (AG vs. GG: P = 0.811; OR = 1.024, 95% CI = 0.843–1.244), dominant (AA+AG vs. GG: P = 0.630; OR = 1.055, 95% CI = 0.849–1.311) and recessive (AA vs. AG+GG: P = 0.549; OR = 1.181, 95% CI = 0.686–2.033). Subgroup analysis revealed that TNF-α –308 G\u3eA SNP is associated with reduced risk of CRC in Asian ethnicity. The study showed no publication bias. Conclusions: No association of TNF-α –308 G\u3eA SNP with overall CRC risk was found. This SNP is likely to be protective against CRC in Asian population when compared with Caucasian population. Larger prospective-epidemiological studies are warranted to elucidate the roles of TNF-α –308 G\u3eA SNP in the etiology of CRC and to endorse the present findings

    Association of MBL2 gene polymorphisms with pulmonary tuberculosis susceptibility: Trial sequence meta-analysis as evidence

Background: Mannose-binding lectin (MBL) or mannose-binding protein (MBP), encoded by MBL2 gene and secreted by the liver, activates complement system through lectin pathway in innate immunity against the host’s infection. Conflictingly, a number of MBL2 variants, rs1800450 (A\u3eB), rs1800451 (A\u3eC), rs5030737 (A\u3eD), rs7096206 (Y\u3eX), rs11003125 (H\u3eL), and rs7095891 (P\u3eQ) allele, have been found to be associated with compromised serum levels and pulmonary tuberculosis (PTB) susceptibility. The present meta-analysis study was performed to evaluate the potential association of these MBL2 gene variants with PTB susceptibility. Materials and methods: A quantitative synthesis was performed on PubMed (Medline), EMBASE, and Google Scholar web database searches. A meta-analysis was performed to calculate the pooled odds ratios and 95% CIs for all the genetic models. Results: A total of 14 eligible studies were included to analyze their pooled data for associations between alleles, genotypes, and minor allele carriers. The statistical analysis revealed the significant reduced PTB risk with homozygous variant genotype of rs1800451 polymorphism (CC vs AA: P=0.043; OR =0.828, 95% CI =0.689–0.994). Contrary to this, the variant allele of rs5030737 polymorphism showed association with increased PTB risk (D vs A: P=0.026; OR =1.563, 95% CI =1.054–2.317). However, the other genetic models of rs1800450 (A\u3eB), rs7096206 (Y\u3eX), and rs11003125 (H\u3eL) MBL2 gene polymorphisms did not divulge any association with PTB susceptibility. Conclusion: The current meta-analysis concludes that rs1800451 (A\u3eC) and rs5030737 (A\u3eD) polymorphisms of MBL2 gene play a significant role in PTB susceptibility. Further, well-designed epidemiological studies with larger sample size including consideration of environmental factors are warranted for the future

    A study of outcome of ossiculoplasty using autologous cartilage, refashioned incus, and polytetrafluoroethylene (Teflon) prosthesis in patients of chronic suppurative otitis media

    Abstract Aim To find out the mean air conduction thresholds (ACT) and air–bone gap (ABG) closure across the treatment groups at the end of 3 and 6 months of follow-up. Material and methods Sixty patients diagnosed with COM with conductive hearing loss were included in the study. Air conduction threshold (ACT) and air–bone gap were calculated and recorded pre-operatively. Surgery was done with clearance of disease followed by reconstruction of hearing in single-stage operation using autologous conchal cartilage, refashioned incus, and polytetrafluoroethylene (Teflon) prosthesis (PORP, TORP) depending upon the intraoperative findings during surgery. Patients were followed for up to 6 months for assessing the hearing outcome in terms of the mean air conduction threshold and mean air–bone gap closure for each group separately. Results The outcome of each ossiculoplasty material was calculated in terms of mean air conduction threshold and mean AB gap closure. Preoperative and postoperative air conduction threshold (ACT) at 3 months and 6 months follow-up of each group was as follows: for the autologous conchal cartilage group, 41.3 (± SD 6.69), 29.2 (± SD 5.39), and 21 (± SD 4.66); for autologous refashioned incus group, 40.4 (± SD 5.43), 28.4 (± SD 6.73), and 20.8 (± SD 4.33); for the Teflon PORP group, 42.9 (± SD 5.68), 31.4 (± SD 6.86), and 34.9 (± SD 6.37); and for the Teflon TORP group, 43.1 (± SD 5.40), 32.5 (± SD 5.91), and 36.2 (± SD 5.31). The mean air–bone gap preoperatively and postoperatively at 3 months and 6 months respectively were as follows: for autologous conchal cartilage, 40.6 (± SD 4.57), 23.7 (± SD 4.48), and 20 (± SD 5.28); for autologous refashioned incus, 39.3(± SD 4.92), 21.9 (± SD 5.61), and 19.4 (± SD 5.82); for Teflon PORP 43.0 (± SD 4.48), 32.8 (± SD 4.84), and 36.3 (± SD 5.56); and for Teflon TORP, 44.5 (± SD 5.56), 33.2 (± SD 5.53), and 35.2 (± SD 5.10). Conclusion The hearing outcome of ossiculoplasty varies with the type of ossiculoplasty material used. Most favorable results were obtained with refashioned autologous incus followed by autologous conchal cartilage. Teflon prosthesis has a significant improvement in hearing outcomes although the results are less favorable

    Three cytoplasmic male sterile (CMS) lines with Wild Abortive (WA) CMS source were crossed with 9 rice genotypes to assess their restorer/maintainer behavior during 2011-12. Out of 27 test crosses evaluated, 10 restorers, 8 partial restorers and 9 partial maintainer cross combinations were categorized on the basis of pollen fertility and spikelet sterility. Three genotypes K-08-61, K-08-60 and Pusa Sughand-5 were found as effective restorers for all the three CMS lines with spikelet fertility above 83% in F1s for data pooled over environments, besides one more genotype K-08-59 depicted restoration only with the introduced CMS line IR-68888A
