    Fungal carriage on healthcare workers’ hands, clothing, stethoscopes and electronic devices during routine patient care: a study from a tertiary care center

    Background – Invasive fungal infections are a constant threat to immunocompromised and critically ill patients. Healthcare workers caring for such patients act as conduits of transmission through their contaminated hands and belongings. Although bacterial contamination of healthcare workers is known, our knowledge about fungal carriage is sparse. Aim– To study the prevalence and type of fungal carriage on healthcare workers hands, aprons/hospital scrubs, electronic devices and stethoscopes. Methods– Healthcare workers working in Medicine ward and ICU during November and December 2019 were sampled. Hand washes were collected in Brain Heart Infusion (BHI) broth with gentamycin. Direct impression smears on blood agar were taken from aprons/hospital scrubs. Electronic devices and stethoscopes were sampled using moist cotton swabs. Subculture and plating was done on Sabarouds Dextrose Agar (SDA). Yeasts were identified using Matrix Assisted Laser Desorption Ionisation Time of Flight (MALDI TOF) and moulds were identified using microscopy. Findings – Out of 60 health care workers, 20 (33.3%) had fungal carriage. Aprons/hospital scrubs and hands were contaminated in 17 (28.3%) and 3 (5%) respectively. Aprons/hospital scrubs mainly constituted moulds belonging to species of Aspergillus. Hands were contaminated with Candida tropicalis, Candida parapsilosis and Candida auris. Electronic devices and stethoscopes had no fungal contamination. Sex (p=0.77), designation (p=0.32) and unit of surveillance (p=0.06) were not significantly associated with fungal isolation from health care workers. Conclusion – Active fungal surveillance provides prevalent carriage rates and serve as a feedback to improve our disinfection and hand hygiene practices. It also aids in identification of potential source of hospital outbreaks

The evolution of steganography has been paralleled by the development of steganalysis. In this game of hide and seek, the two player’s steganography and steganalysis always want to break the other down. Over the past three decades, research has produced a plethora of remarkable image steganography techniques (ISTs). The major challenge for most of these ISTs is to achieve a fair balance between the metrics such as high hiding capacity (HC), better imperceptibility, and improved security. This study aims to present an exhaustive scrutiny of various ISTs from the classical to recent developments in the spatial domain, with respect to various image steganographic metrics. Further, the current status, recent developments, open challenges, and promising directions in this field are also highlighted

    Association of three H - Hookworm, hemosuccus pancreaticus, and hypertension (portal) in a patient with melena

    Hookworm infestations, endemic in India, are a common cause of iron deficiency anemia. Hemosuccus pancreaticus, a rare clinical condition, is due to passage of blood into the pancreatic duct possibly through a route between an aneurysm of an artery close to the pancreas and/or pancreatic duct, leading to gastrointestinal (GI) bleeding. Portal hypertensive upper GI bleed is also known since long. We report a case of a 38-year-old male with a history of alcoholism who was being investigated for GI bleeding who had concomitant hookworm infestation, hemosuccus pancreaticus as well as portal hypertension. To the best of our knowledge, this is the first report of common occurrence of hemosuccus pancreaticus and portal hypertension with hookworm infection. This case signifies the importance of infectious causes of GI bleeding to be considered even in cases where anatomic malformations or pathophysiological alterations are predominant

    A Novel and Secure Fake-Modulus Based Rabin-Ó  Cryptosystem

    Electronic commerce(E-commerce) transactions require secure communication to protect sensitive information such as credit card numbers, personal identification, and financial data from unauthorized access and fraud. Encryption using public key cryptography is essential to ensure secure electronic commerce transactions. RSA and Rabin cryptosystem algorithms are widely used public key cryptography techniques, and their security is based on the assumption that it is computationally infeasible to factorize the product of two large prime numbers into its constituent primes. However, existing variants of RSA and Rabin cryptosystems suffer from issues like high computational complexity, low speed, and vulnerability to factorization attacks. To overcome the issue, this article proposes a new method that introduces the concept of fake-modulus during encryption. The proposed method aims to increase the security of the Rabin cryptosystem by introducing a fake-modulus during encryption, which is used to confuse attackers who attempt to factorize the public key. The fake-modulus is added to the original modulus during encryption, and the attacker is unable to distinguish between the two. As a result, the attacker is unable to factorize the public key and cannot access the sensitive information transmitted during electronic commerce transactions. The proposed method’s performance is evaluated using qualitative and quantitative measures. Qualitative measures such as visual analysis and histogram analysis are used to evaluate the proposed system’s quality. To quantify the performance of the proposed method, the entropy of a number of occurrences for the pixels of cipher text and differential analysis of plaintext and cipher text is used. When the proposed method’s complexity is compared to a recent variant of the Rabin cryptosystem, it can be seen that it is more complex to break the proposed method—represented as O(ɲ× τ) which is higher than Rabin-P (O(ɲ)) algorithms

    Mo(VI) Potential Metallodrugs : Explaining the Transport and Cytotoxicity by Chemical Transformations

    The transport and cytotoxicity of molybdenum-based drugs have been explained with the concept of chemical transformation, a very important idea in inorganic medicinal chemistry that is often overlooked in the interpretation of the biological activity of metal-containing systems. Two monomeric, [MoO2(L1)(MeOH)] (1) and [MoO2(L2)(EtOH)] (2), and two mixed-ligand dimeric MoVIO2 species, [{MoO2(L1-2)}2(ÎĽ-4,4'-bipy)] (3-4), were synthesized and characterized. The structures of the solid complexes were solved through SC-XRD, while their transformation in water was clarified by UV-vis, ESI-MS, and DFT. In aqueous solution, 1-4 lead to the penta-coordinated [MoO2(L1-2)] active species after the release of the solvent molecule (1 and 2) or removal of the 4,4'-bipy bridge (3 and 4). [MoO2(L1-2)] are stable in solution and react with neither serum bioligand nor cellular reductants. The binding affinity of 1-4 toward HSA and DNA were evaluated through analytical and computational methods and in both cases a non-covalent interaction is expected. Furthermore, the in vitro cytotoxicity of the complexes was also determined and flow cytometry analysis showed the apoptotic death of the cancer cells. Interestingly, ÎĽ-4,4'-bipy bridged complexes 3 and 4 were found to be more active than monomeric 1 and 2, due to the mixture of species generated, that is [MoO2(L1-2)] and the cytotoxic 4,4'-bipy released after their dissociation. Since in the cytosol neither the reduction of MoVI to MoV/IV takes place nor the production of reactive oxygen species (ROS) through Fenton-like reactions of 1-4 with H2O2 occurs, the mechanism of cytotoxicity should be attributable to the direct interaction with DNA that happens with a minor-groove binding which results in cell death through an apoptotic mechanism

    Background – Healthcare workers’ (HCW) hands and personnel belongings are vehicles of transmission of nosocomial infections. Knowledge, attitude and practice of hand hygiene have been extensively studied suggesting adequate knowledge but poor compliance. Similar data on aprons, mobile phone and stethoscope disinfection practices are lacking. This becomes extensively important topic of discussion in current COVID 19 pandemic where inadequacy in hygiene practices is devastating. Aim– To study the knowledge, attitude and infection prevention practices of HCWs aprons, electronic devices, stethoscopes, and hands. Methods– A cross sectional questionnaire-based survey was conducted among HCWs of Medicine ward and ICU. Findings – Sixty-six HCWs responded to the survey. Awareness that hands, aprons, mobile phones, stethoscopes could cause cross transmission and knowledge of correct practices was present in majority of the respondents. Hand hygiene was performed by 65.2% of the respondents before touching a patient and 54.5% after touching the patient surroundings while 13.6% performed only when it was visibly soiled. Mobile phones and stethoscopes were disinfected by 13.6% and 30.3% of the respondents after each patient encounter, respectively. Aprons were washed after using them at a stretch for a median duration of 5 days (1 – 30 days). Forgetfulness, lack of reinforcement, lack of time, inadequate awareness on standard disinfection practices and fear of damaging electronic devices from disinfectants use were reasons for poor compliance. Conclusion –There is an urgent need to spread awareness and formulate standard guidelines on disinfection practices especially for mobile phones, stethoscopes, and aprons in addition to reinforcing hand hygiene practices

    Assessment of the recurrent mutagenesis efficacy of sesame crosses followed by isolation and evaluation of promising genetic resources for use in future breeding programs

    The primary causes of low sesame production are a lack of high-yielding varieties, an inability to adjust to environmental challenges, and a poor response to enhanced crop management. The potential for improving sesame has been investigated using a variety of breeding approaches, including mutation and recombination breeding; however, these methods have only produced a minor increase in productivity. Recombination and mutant breeding have recently been proposed as novel, cutting-edge approaches to overcome the limitations of conventional breeding practices. This study was conducted to assess the recurrent mutagenesis efficacy of sesame crosses, followed by the isolation and evaluation of superior breeding lines that could be released as varieties in the future. In the F4M4 generation, the populations were grown in a randomized block design (RBD), and the efficacy of recurrent mutagenesis was assessed in terms of variability parameters such as the mean, standard deviation, and coefficient of variation. During the F5M5 generation, the materials were grown in three separate trials in Compact Family Block Design to evaluate the between-family and within-family variations, followed by the isolation of improved breeding lines based on per se performance. The improved breeding lines were further advanced to the F6M6 generation for evaluation in three separate trials in an RBD. In the F6M6 generation, ANOVA, correlation, regression, path coefficient, and Mahalanobis’s D 2 analysis were computed, followed by the construction of selection indices to select superior lines. The results showed the importance of three or four EMS treatment cycles for sesame improvement in the F4M4 generation. Twenty of the top lines based on per se performance (with a selection intensity of 27%) from each category in the F5M5 generation were carried forward to the F6M6 generation. In the F6M6 generation, the top six breeding lines (with a selection intensity of 30%) from each category, for a total of 18 breeding lines, were chosen based on these index scores. These high-yielding micromutant lines could be subsequently released as varieties through multiplication trials with standard checks. After a thorough evaluation, these lines have the potential to replace the current varieties and increase production, productivity, and adoption of sesame in India
