Cold Atom Physics Using Ultra-Thin Optical Fibers: Light-Induced Dipole Forces and Surface Interactions
The strong evanescent field around ultra-thin unclad optical fibers bears a
high potential for detecting, trapping, and manipulating cold atoms.
Introducing such a fiber into a cold atom cloud, we investigate the interaction
of a small number of cold Caesium atoms with the guided fiber mode and with the
fiber surface. Using high resolution spectroscopy, we observe and analyze
light-induced dipole forces, van der Waals interaction, and a significant
enhancement of the spontaneous emission rate of the atoms. The latter can be
assigned to the modification of the vacuum modes by the fiber.Comment: 4 pages, 4 figure
Cervical visual inspection with acetic acid (VIA) and oncogenic human papillomavirus screening in rural indigenous guatemalan women: Time to rethink VIA
Single-visit âscreen-and-treatâ strategies using visual inspection with acetic acid (VIA) and cryotherapy (liquid nitrous oxide ablation) in low-resource settings are commonly used to de-tect and treat precancerous lesions for cervical cancer prevention. This study compared VIA sensi-tivity and specificity in rural indigenous Guatemalan communities, to that of oncogenic human papillomavirus (HPV) testing for detection of precancerous changes, using cytology as the reference standard. Between 3â8 September 2017, trained nurses examined 222 women aged 23â58 years with VIA. Specimens for liquid-based cytology and HPV testing were obtained prior to VIA with a cyto-brush and transported in PreservCyt to a US clinical laboratory. VIA and HPV test sensitivities were assessed as proportions of women with abnormal cytology that had abnormal VIA or HPV results, respectively, and specificities, as proportions with normal cytology with normal VIA or negative HPV tests. Of 222 women, 18 (8.1%) had abnormal cytology (1 carcinoma in a participant who received VIA-based cryotherapy in 2015, 4 high-and 5 low-grade squamous intraepithelial lesions, and 8 atypical squamous cells of undetermined significance (ASCUS)). Excluding ASCUS, sensitivities of VIA and HPV were 20.0% and 100%, respectively. VIA-based screening may not be accepta-ble for detecting precancerous lesions, and field cryotherapy for preventing malignancy. The World Health Organization recommended in 2021 ââŠusing HPV DNA detection as the primary screening test rather than VIA or cytology.â
Background-free detection of trapped ions
We demonstrate a Doppler cooling and detection scheme for ions with low-lying
D levels which almost entirely suppresses scattered laser light background,
while retaining a high fluorescence signal and efficient cooling. We cool a
single ion with a laser on the 2S1/2 to 2P1/2 transition as usual, but repump
via the 2P3/2 level. By filtering out light on the cooling transition and
detecting only the fluorescence from the 2P_3/2 to 2S1/2 decays, we suppress
the scattered laser light background count rate to 1 per second while
maintaining a signal of 29000 per second with moderate saturation of the
cooling transition. This scheme will be particularly useful for experiments
where ions are trapped in close proximity to surfaces, such as the trap
electrodes in microfabricated ion traps, which leads to high background scatter
from the cooling beam
Optical nanofibers and spectroscopy
We review our recent progress in the production and characterization of
tapered optical fibers with a sub-wavelength diameter waist. Such fibers
exhibit a pronounced evanescent field and are therefore a useful tool for
highly sensitive evanescent wave spectroscopy of adsorbates on the fiber waist
or of the medium surrounding. We use a carefully designed flame pulling process
that allows us to realize preset fiber diameter profiles. In order to determine
the waist diameter and to verify the fiber profile, we employ scanning electron
microscope measurements and a novel accurate in situ optical method based on
harmonic generation. We use our fibers for linear and non-linear absorption and
fluorescence spectroscopy of surface-adsorbed organic molecules and investigate
their agglomeration dynamics. Furthermore, we apply our spectroscopic method to
quantum dots on the surface of the fiber waist and to caesium vapor surrounding
the fiber. Finally, towards dispersive measurements, we present our first

Impact of previous tobacco use with or without cannabis on first psychotic experiences in patients with first-episode psychosis
Objective: There is high prevalence of cigarette smoking in individuals with first-episode psychosis (FEP) prior to psychosis onset. The purpose of the study was to determine the impact of previous tobacco use with or without cannabis on first psychotic experiences in FEP and the impact of this use on age of onset of symptoms, including prodromes. Methods: Retrospective analyses from the naturalistic, longitudinal, multicentre, âPhenotype-Genotype and Environmental Interaction. Application of a Predictive Model in First Psychotic Episodes (PEPs)â Study. The authors analysed sociodemographic/clinical data of 284 FEP patients and 231 matched healthy controls, and evaluated first psychotic experiences of patients using the Symptom Onset in Schizophrenia Inventory. Results: FEP patients had significantly higher prevalence of tobacco, cannabis, and cocaine use than controls. The FEP group with tobacco use only prior to onset (N = 56) had more sleep disturbances (42.9% vs 18.8%, P = 0.003) and lower prevalence of negative symptoms, specifically social withdrawal (33.9% vs 58%, P = 0.007) than FEP with no substance use (N = 70), as well as lower prevalence of ideas of reference (80.4% vs 92.4%, P = 0.015), perceptual abnormalities (46.4% vs 67.4%, P = 0.006), hallucinations (55.4% vs 71.5%, P = 0.029), and disorganised thinking (41.1% vs 61.1%, P = 0.010) than FEP group with previous tobacco and cannabis use (N = 144). FEP patients with cannabis and tobacco use had lower age at first prodromal or psychotic symptom (mean = 23.73 years [SD = 5.09]) versus those with tobacco use only (mean = 26.21 [SD = 4.80]) (P = 0.011). Conclusions: The use of tobacco alone was not related to earlier age of onset of a first psychotic experience, but the clinical profile of FEP patients is different depending on previous tobacco use with or without cannabis. © 2021 The Author
Prodromal symptoms and the duration of untreated psychosis in first episode of psychosis patients: what differences are there between early vs. adult onset and between schizophrenia vs. bipolar disorder?
To assess the role of age (early onset psychosis-EOPâ<â18 years vs. adult onset psychosis-AOP) and diagnosis (schizophrenia spectrum disorders-SSD vs. bipolar disorders-BD) on the duration of untreated psychosis (DUP) and prodromal symptoms in a sample of patients with a first episode of psychosis. 331 patients with a first episode of psychosis (7â35 years old) were recruited and 174 (52.6%) diagnosed with SSD or BD at one-year follow-up through a multicenter longitudinal study. The Symptom Onset in Schizophrenia (SOS) inventory, the Positive and Negative Syndrome Scale and the structured clinical interviews for DSM-IV diagnoses were administered. Generalized linear models compared the main effects and group interaction. 273 AOP (25.2â±â5.1 years; 66.5% male) and 58 EOP patients (15.5â±â1.8 years; 70.7% male) were included. EOP patients had significantly more prodromal symptoms with a higher frequency of trouble with thinking, avolition and hallucinations than AOP patients, and significantly different median DUP (91 [33â177] vs. 58 [21â140] days; Zâ=ââ 2.006, pâ=â0.045). This was also significantly longer in SSD vs. BD patients (90 [31â155] vs. 30 [7â66] days; Zâ=ââ 2.916, pâ=â0.004) who, moreover had different profiles of prodromal symptoms. When assessing the interaction between age at onset (EOP/AOP) and type of diagnosis (SSD/BD), avolition was significantly higher (Wald statisticâ=â3.945; pâ=â0.047), in AOP patients with SSD compared to AOP BD patients (pâ=â0.004). Awareness of differences in length of DUP and prodromal symptoms in EOP vs. AOP and SSD vs. BD patients could help improve the early detection of psychosis among minors
Cost-effectiveness of novel vaccines for tuberculosis control: a decision analysis study
<p>Abstract</p> <p>Background</p> <p>The development of a successful new tuberculosis (TB) vaccine would circumvent many limitations of current diagnostic and treatment practices. However, vaccine development is complex and costly. We aimed to assess the potential cost effectiveness of novel vaccines for TB control in a sub-Saharan African country - Zambia - relative to the existing strategy of directly observed treatment, short course (DOTS) and current level of bacille Calmette-Guérin (BCG) vaccination coverage.</p> <p>Methods</p> <p>We conducted a decision analysis model-based simulation from the societal perspective, with a 3% discount rate and all costs expressed in 2007 US dollars. Health outcomes and costs were projected over a 30-year period, for persons born in Zambia (population 11,478,000 in 2005) in year 1. Initial development costs for single vaccination and prime-boost strategies were prorated to the Zambian share (0.398%) of global BCG vaccine coverage for newborns. Main outcome measures were TB-related morbidity, mortality, and costs over a range of potential scenarios for vaccine efficacy.</p> <p>Results</p> <p>Relative to the status quo strategy, a BCG replacement vaccine administered at birth, with 70% efficacy in preventing rapid progression to TB disease after initial infection, is estimated to avert 932 TB cases and 422 TB-related deaths (prevention of 199 cases/100,000 vaccinated, and 90 deaths/100,000 vaccinated). This would result in estimated net savings of 5.6 million compared to the status quo, averting 1,863 TB cases and 1,011 TB-related deaths (prevention of 398 cases/100,000 vaccinated, and of 216 deaths/100,000 vaccinated). With vaccination at birth alone, net savings would be realized within 1 year, whereas the prime-boost strategy would require an additional 5 years to realize savings, reflecting a greater initial development cost.</p> <p>Conclusions</p> <p>Investment in an improved TB vaccine is predicted to result in considerable cost savings, as well as a reduction in TB morbidity and TB-related mortality, when added to existing control strategies. For a vaccine with waning efficacy, a prime-boost strategy is more cost-effective in the long term.</p
