    Effect of vitamin C and vitamin E on lung contusion: A randomized clinical trial study

    ABSTRACT There is association between lung contusion (lC) and a progressive in fl ammatory response. The protective effect of vitamin C and vitamin E, as strong free radical scavengers on favourite outcome of (LC) in animal models,has been confirmed. Design: to evaluate the effect of vitamins, E and C on arterial blood gas (ABG) and ICU stay, in (LC), with injury severity score (ISS) 18 ± 2, due to blunt chest trauma. Methods: This study was a randomized, double-blind, placebo controlled clinical trial. Patients with (ISS)18 ± 2 blunt chest trauma, who meet criteria, participated in the study. A total of 80 patients from Feb 2015 to Jun2018and were randomly divided into 4 groups. Patients received intravenous vitamin E (1000IU mg), was (group I);intravenous vitamin C (500) (group II). Vitamin C + vitamin E = (group III), and intravenous distilled water = (control group) or (group IV). ABG, serum cortisol, and CRP levels were determined at baseline, 24 h and 48 h after the intervention. Results: a significant decrease in ICU stay in group III compared to other groups (p < 0.001). Co-administration of vitamin C and vitamin E showed significant increases pH (values to reference range from acidemia”), oxygen pressure, and oxygen saturation in group III compared to other groups (p <0.001). A significant decrease in carbon dioxide pressure was also detected after receiving vitamin C and vitamin E in group III, compared to other groups (p < 0.001).     A Rare Case of HIV-Induced Inflammatory Demyelinating Polyneuropathy.

    Background:Acute inflammatory demyelinating polyneuropathy (AIDP) is an uncommon form of neuropathy in HIV-infected patients that can cause pain, sensory disturbance, and motor weakness. Case presentation:A 23-year-old African American male with past medical history of Guillain-Barre Syndrome (GBS), Lyme disease, and sexually transmitted infections including syphilis and chlamydia presented with acute back pain radiating to bilateral lower extremities with worsening right foot weakness for four days. Cerebrospinal fluid (CSF) studies including meningoencephalitis panel were negative as well as blood tests for Lyme disease and HIV antibody testing. Patient was initially treated with penicillin for positive treponemal serology but without improvement in lower extremity weakness. Electromyogram showed evidence of early demyelinating motor polyneuropathy. Four days after presentation, repeat HIV antibody testing returned positive. Recurrent AIDP in this case was suspected to be secondary to acute HIV infection, and highly active antiretroviral therapy (HAART) was administered along with intravenous immunoglobulin (IVIG). Muscle strength improved with therapy and patient was expected to have continued improvement with intensive rehabilitation after discharge. Conclusion:Acute inflammatory demyelinating polyneuropathy (AIDP) tends to present early in course of HIV infection. Therefore, HIV testing should be obtained in individuals presenting with new neurological deficits. Our patient received HAART therapy, in addition to the traditional modalities to manage AIDP, which led to a substantial recovery of his sensorimotor function

    An Agent-based Architecture for AI-Enhanced Automated Testing for XR Systems, a Short Paper

    This short paper presents an architectural overview of an agent-based framework called iv4XR for automated testing that is currently under development by an H2020 project with the same name. The framework's intended main use case of is testing the family of Extended Reality (XR) based systems (e.g. 3D games, VR sytems, AR systems), though the approach can indeed be adapted to target other types of interactive systems. The framework is unique in that it is an agent-based system. Agents are inherently reactive, and therefore are arguably a natural match to deal with interactive systems. Moreover, it is also a natural vessel for mounting and combining different AI capabilities, e.g. reasoning, navigation, and learning

    Mid-term effects of carotid endarterectomy on cognition and white matter status evaluated by whole brain diffusion tensor imaging metrics: A preliminary analysis

    Purpose: To analyze the mid-term (12 months) effects of carotid endarterectomy (CEA) on cognition and on the microstructural properties of the whole brain white matter in terms of derived diffusion Tensor imaging (DTI) metrics. Methods: We analyzed a population of 19 asymptomatic patients with extra-cranial internal carotid artery stenosis (eICA) eligible for CEA. All patients underwent cognitive evaluation with the Italian version of the Mini-Mental State Examination corrected for age and schooling, and with a Magnetic Resonance Imaging (MRI) investigation on a 1.5 Tesla MRI scanner, that included a 34-directions Diffusion Weighted Imaging (DWI) sequence for DTI metrics analysis. The global fractional anisotropy (gFA), global mean diffusivity (gMD), global radial diffusivity (gRD) and global axial diffusivity (gAD) were calculated for each patient. Both the cognitive and the imaging evaluation were performed at baseline (PRE-CEA) and 12 months after CEA (POST-CEA). Two-tailed Paerson's correlation test and paired samples t-test were used for evaluating the correlation between PRE-CEA and POST-CEA values, adopting a p-value of &lt;0.05 as statistically significant. Results: A statistically significant increase of the MMSE scores (p &lt; 0.0001), as well as for gFA (p &lt; 0.0001), and a statistically significant reduction of gMD (p = 0.027) and gRD (p = 0.0005) was observed 12 months following uncomplicated CEA. Conclusions: These findings suggest that CEA is associated with a general improvement of the WM microstructure of the whole brain

    We have studied atomic diffraction at normal incidence from an evanescent standing wave with a high resolution using velocity selective Raman transitions. We have observed up to 3 resolved orders of diffraction, which are well accounted for by a scalar diffraction theory. In our experiment the transverse coherence length of the source is greater than the period of the diffraction grating.Comment: 8 pages, 4 figure

    Cold atoms in videotape micro-traps

    We describe an array of microscopic atom traps formed by a pattern of magnetisation on a piece of videotape. We describe the way in which cold atoms are loaded into one of these micro-traps and how the trapped atom cloud is used to explore the properties of the trap. Evaporative cooling in the micro-trap down to a temperature of 1 microkelvin allows us to probe the smoothness of the trapping potential and reveals some inhomogeneity produced by the magnetic film.     Combined microwave ablation and osteosynthesis for long bone metastases

    Background and Objectives: The purpose of this study was to evaluate the feasibility, safety and efficacy of microwave ablation (MWA) in combination with open surgery nail positioning for the treatment of fractures or impending fractures of long bone metastases. Material and Methods: Eleven patients (four men, seven women) with painful bone metastases of the humerus, femur or tibia with non-displaced fractures (one case) or impending fractures (10 cases) underwent open MWA in combination with osteosynthesis by locked nail positioning. Pain intensity was measured using a VAS score before and after treatment. CT or MRI were acquired at one month before and 1, 3, 6, 12 and 18 months after treatment. Results: All procedures were successfully completed without major complications. The level of pain was significantly reduced one month after treatment. For the patients with humerus metastases, the complete recovery of arm use took 8 weeks, while for the patients with femoral metastases the complete recovery of walking capacity took 11 weeks. The VAS score ranged from 7 (4–9) before treatment to 1.5 (0–2.5) after treatment. During a mid-term follow-up of 18 months (range 4–29 months), none of the patients showed tumor relapse or new fractures in the treated site. Two patients died due to tumor disease progression. Conclusion: Results of this preliminary study suggest that combined MWA and surgical osteosynthesis with locked nails is a safe and effective treatment for pathological fractures or malignant impending fractures of long bone metastases of the humerus, femur and tibia. Further analyses with larger cohorts are warranted to confirm these findings

    Spring bloom dynamics and zooplankton biomass response on the US Northeast Continental Shelf

The definitive version was published in Continental Shelf Research 102 (2015): 47-61, doi:10.1016/j.csr.2015.04.005.The spring phytoplankton bloom on the US Northeast Continental Shelf is a feature of the ecosystem production cycle that varies annually in timing, spatial extent, and magnitude. To quantify this variability, we analyzed remotely-sensed ocean color data at two spatial scales, one based on ecologically defined sub-units of the ecosystem (production units) and the other on a regular grid (0.5°). Five units were defined: Gulf of Maine East and West, Georges Bank, and Middle Atlantic Bight North and South. The units averaged 47×103 km2 in size. The initiation and termination of the spring bloom were determined using change-point analysis with constraints on what was identified as a bloom based on climatological bloom patterns. A discrete spring bloom was detected in most years over much of the western Gulf of Maine production unit. However, bloom frequency declined in the eastern Gulf of Maine and transitioned to frequencies as low as 50% along the southern flank of the Georges Bank production unit. Detectable spring blooms were episodic in the Middle Atlantic Bight production units. In the western Gulf of Maine, bloom duration was inversely related to bloom start day; thus, early blooms tended to be longer lasting and larger magnitude blooms. We view this as a phenological mismatch between bloom timing and the “top-down” grazing pressure that terminates a bloom. Estimates of secondary production were available from plankton surveys that provided spring indices of zooplankton biovolume. Winter chlorophyll biomass had little effect on spring zooplankton biovolume, whereas spring chlorophyll biomass had mixed effects on biovolume. There was evidence of a “bottom up” response seen on Georges Bank where spring zooplankton biovolume was positively correlated with the concentration of chlorophyll. However, in the western Gulf of Maine, biovolume was uncorrelated with chlorophyll concentration, but was positively correlated with bloom start and negatively correlated with magnitude. This observation is consistent with both a “top-down” mechanism of control of the bloom and a “bottom-up” effect of bloom timing on zooplankton grazing. Our inability to form a consistent model of these relationships across adjacent systems underscores the need for further research
