Evaluating Compassion-Mindfulness Therapy for Recurrent Anxiety and Depression: A Randomized Control Trial
Objective: To evaluate the effects of compassion–mindfulness therapy (C-MT), an adapted version of mindfulness-based cognitive therapy that integrates compassion training.Method: Individuals aged 17–69 with recurrent depressive and anxiety symptoms were recruited from a community mental health service unit. Half of the participants were randomized to an 8-week C-MT program (n = 41) and the other half to a wait-list control condition (n = 41).Results: Intent-to-treat analyses showed significant improvements in all measures in the treatment group. The effect sizes for depression and anxiety were 1.11 and 1.10, respectively, and those for physical distress, daily functioning, positive affect, and negative affect ranged from 0.71 to 1.04. All improvements were sustained at the 3-month follow-up.Conclusions: The results provide preliminary support for C-MT as a viable treatment option for individuals with recurrent depression and anxiety symptoms. Time-limited treatments such as C-MT should be promoted in social work practice.postprin

The regulation of pulmonary vascular tone by neuropeptides and the implications for pulmonary hypertension
© 2018 Lo, Moosavi and Bubb. Pulmonary hypertension (PH) is an incurable, chronic disease of small pulmonary vessels. Progressive remodeling of the pulmonary vasculature results in increased pulmonary vascular resistance (PVR). This causes secondary right heart failure. PVR is tightly regulated by a range of pulmonary vasodilators and constrictors. Endothelium-derived substances form the basis of most current PH treatments. This is particularly the case for pulmonary arterial hypertension. The major limitation of current treatments is their inability to reverse morphological changes. Thus, there is an unmet need for novel therapies to reduce the morbidity and mortality in PH. Microvessels in the lungs are highly innervated by sensory C fibers. Substance P and calcitonin gene-related peptide (CGRP) are released from C-fiber nerve endings. These neuropeptides can directly regulate vascular tone. Substance P tends to act as a vasoconstrictor in the pulmonary circulation and it increases in the lungs during experimental PH. The receptor for substance P, neurokinin 1 (NK1R), mediates increased pulmonary pressure. Deactivation of NK1R with antagonists, or depletion of substance P prevents PH development. CGRP is a potent pulmonary vasodilator. CGRP receptor antagonists cause elevated pulmonary pressure. Thus, the balance of these peptides is crucial within the pulmonary circulation (Graphical Abstract). Limited progress has been made in understanding their impact on pulmonary pathophysiology. This is an intriguing area of investigation to pursue. It may lead to promising new candidate therapies to combat this fatal disease. Epac2-deficiency leads to more severe retinal swelling, glial reactivity and oxidative stress in transient middle cerebral artery occlusion induced ischemic retinopathy
Ischemia occurs in diabetic retinopathy with neuronal loss, edema, glial cell reactivity and oxidative stress. Epacs, consisting of Epac1 and Epac2, are cAMP mediators playing important roles in maintenance of endothelial barrier and neuronal functions. To investigate the roles of Epacs in the pathogenesis of ischemic retinopathy, transient middle cerebral artery occlusion (tMCAO) was performed on Epac1-deficient (Epac1) mice, Epac2-deficient (Epac2) mice, and their wild type counterparts (Epac1+/+ and Epac2+/+). Two-hour occlusion and 22-hour reperfusion were conducted to induce ischemia/reperfusion injury to the retina. After tMCAO, the contralateral retinae displayed similar morphology between different genotypes. Neuronal loss, retinal edema and increase in immunoreactivity for aquaporin 4 (AQP4), glial fibrillary acidic protein (GFAP), peroxiredoxin 6 (Prx6) were observed in ipsilateral retinae. Epac2 ipsilateral retinae showed more neuronal loss in retinal ganglion cell layer, increased retinal thickness and stronger immunostaining of AQP4, GFAP, and Prx6 than those of Epac2+/+. However, Epac1 ipsilateral retinae displayed similar pathology as those in Epac1+/+ mice. Our observations suggest that Epac2-deficiency led to more severe ischemic retinopathy after retinal ischemia/reperfusion injury.published_or_final_versio
Antinociceptive and anti-inflammatory activities of the aqueous extract of the root bark of Combretum sericeum in rodents
Combretum sericeum (Combretacae) is used traditionally in Northern Nigeria for the treatment of fever. In this study, the activities of the aqueous extract of the root bark of this plant against nociception andinflammation were investigated in mice and rats. The studies were carried out using acetic acid writhing, hot plate algesia and carrageenan induced inflammation in rats. The results showed that the extractexhibited significant (P < 0.001) anti-nociceptive and anti-inflammatory activities in all the models used. Phytochemical screening of the extract revealed the presence of tannins, flavonoids, glycosides alkaloids and anthraquinones. The intraperitoneal median lethal dose (LD50) of the extract was found to be 177.48 mg/kg in mice. The observed activities might be the scientific basis for the traditional use of the plant in the treatment of fever. This study also paves way for the possible development of the plant extract as a phytodrug against pain and inflammatory conditions.Keywords: Combretum sericeum, anti-nociceptive, anti-inflammatory, hot plate algesi
SDB: A Secure Query Processing System with Data Interoperability
Demo 3: Systems, User Interfaces, and VisualizationWe address security issues in a cloud database system which em- ploys the DBaaS model — a data owner (DO) exports data to a cloud database service provider (SP). To provide data security, sensitive data is encrypted by the DO before it is uploaded to the SP. Compared to existing secure query processing systems like CryptDB [7] and MONOMI [8], in which data operations (e.g., comparison or addition) are supported by specialized encryption schemes, our demo system, SDB, is implemented based on a set of data- interoperable secure operators, i.e., the output of an operator can be used as input of another operator. As a result, SDB can sup- port a wide range of complex queries (e.g., all TPC-H queries) efficiently. In this demonstration, we show how our SDB prototype supports secure query processing on complex workload like TPC-H. We also demonstrate how our system protects sensitive in- formation from malicious attackers.published_or_final_versio
Ensemble of Hankel Matrices for Face Emotion Recognition
In this paper, a face emotion is considered as the result of the composition
of multiple concurrent signals, each corresponding to the movements of a
specific facial muscle. These concurrent signals are represented by means of a
set of multi-scale appearance features that might be correlated with one or
more concurrent signals. The extraction of these appearance features from a
sequence of face images yields to a set of time series. This paper proposes to
use the dynamics regulating each appearance feature time series to recognize
among different face emotions. To this purpose, an ensemble of Hankel matrices
corresponding to the extracted time series is used for emotion classification
within a framework that combines nearest neighbor and a majority vote schema.
Experimental results on a public available dataset shows that the adopted
representation is promising and yields state-of-the-art accuracy in emotion

Tai-chi for residential patients with schizophrenia on movement coordination, negative symptoms, and functioning: a pilot randomized controlled trial
Objective. Patients with schizophrenia residing at institutions often suffer from negative symptoms, motor, and functional impairments more severe than their noninstitutionalized counterparts. Tai-chi emphasizes body relaxation, alertness, and movement coordination with benefits to balance, focus, and stress relief. This pilot study explored the efficacy of Tai-chi on movement coordination, negative symptoms, and functioning disabilities towards schizophrenia. Methods. A randomized waitlist control design was adopted, where participants were randomized to receive either the 6-week Tai-chi program and standard residential care or only the latter. 30 Chinese patients with schizophrenia were recruited from a rehabilitation residency. All were assessed on movement coordination, negative symptoms, and functional disabilities at baseline, following intervention and 6 weeks after intervention. Results. Tai-chi buffered from deteriorations in movement coordination and interpersonal functioning, the latter with sustained effectiveness 6 weeks after the class was ended. Controls showed marked deteriorations in those areas. The Tai-chi group also experienced fewer disruptions to life activities at the 6-week maintenance. There was no significant improvement in negative symptoms after Tai-chi. Conclusions. This study demonstrated encouraging benefits of Tai-chi in preventing deteriorations in movement coordination and interpersonal functioning for residential patients with schizophrenia. Having a Family Doctor is Associated with Some Better Patient-Reported Outcomes of Primary Care Consultations
Background: Hong Kong (HK) has pluralistic primary care that is provided by a variety of doctors. The aim of our study was to assess patient-reported outcomes of primary care consultations in HK and whether having a family doctor (FD) made any difference. Methods: We interviewed by telephone 3148 subjects from 5174 contacted households (response rate 60.8%) randomly selected from the general population of HK about the experience of their last primary care consultations in September 2007 and April 2008. We compared the patient-reported outcomes (PRO) and patient-centered process of care in those with a FD, those with other types of regular primary care doctors (ORD) and those without any regular primary care doctor (NRD). PRO included patient enablement, global improvement in health, overall satisfaction, and likelihood of recommending their doctors to family and friends. Patient-centered process of care indicators was explanations about the illness, and address of patient’s concerns. Results: One thousand one hundred fifty, 746, and 1157 reported to have FD, ORD, and NRD, respectively. Over 80% of those with FD consulted their usual primary care doctors in the last consultation compared with 27% of those with NRD. Compared with subjects having ORD or NRD, subjects with FD reported being more enabled after the consultation and were more likely to recommend their doctors to family and friends. Subjects with FD and ORD were more likely than those having NRD to report a global improvement in health and satisfaction. FD group was more likely than the other two groups to report receiving an explanation on the diagnosis, nature, and expected course of the illness, and having their concerns addressed. Patient enablement was associated with explanation of diagnosis, nature, and expected course of illness, and address of patient’s concerns. Conclusion: People with a regular FD were more likely to feel being enabled and to experience patient-centered care in consultations.published_or_final_versio
