    Selection of Elements at the Dissolution of Heat Resistant Nickel Alloys in Mineral Acid Solutions

    Thermodynamic modeling of the interaction of a heat-resistant nickel alloy containing refractory rare metals with hydrochloric, sulfuric and nitric acids with a temperature change in the range of 20-100 ∘ C and a concentration of 50 to 150 g / dm3 at a pressure of 1 atm was carried out. The thermodynamic assessment data were confirmed in experiments on the anodic dissolution of the alloy in sulfate solutions. According to the results of x-ray phase analysis of the sludge, it was revealed that tungsten in the form of a solid solution in nickel passes to this product. Also, tantalum and niobium pass into the slurry in the form of oxides. Mostly, Re, Co, Cr passed into the electrolyte solution. The results of the study can serve as a scientific basis for the development of promising technologies for processing metal waste from heat-resistant alloys.     Pyrimidine-Based Push–Pull Systems with a New Anchoring Amide Group for Dye-Sensitized Solar Cells

    New donor–π–acceptor pyrimidine-based dyes comprising an amide moiety as an anchoring group have been designed. The dyes were synthesized by sequential procedures based on the microwave-assisted Suzuki cross-coupling and bromination reactions. The influence of the dye structure and length of π-linker on the photophysical and electrochemical properties and on the photovoltaic effectiveness of dye-sensitized solar cells was investigated. An increase in efficiency with a decrease in the length of π-linker was revealed.     Development of minimallyinvasive outpatient phlebology in Ekaterinburg

    In this article we have studied the ratio of open phlebectomy and minimally invasive procedures in the treatment of varicose veins disease of the lower extremities in Ekaterinburg over the past 5 years. It is revealed that in the last 5 years in the structure of phlebological care of the city out-patient endovascular and minimally invasive procedures on the trunks of subcutaneous veins prevail. They are 2.5 times more likely than the classic phlebectomy. The range of medical services in phlebology has significantly expanded, which made the approach to the patient more individual, improved the quality of care and increased its safety. Expenses of the budget from the introduction of minimally invasive technologies (only due to the cost savings of the hospital) decreased by 187,44 million rubles /year.В настоящей статье произведено изучение соотношения открытых флебэктомий и миниивазивных процедур в лечении варикозной болезни нижних конечностей в городе Екатеринбурге за последние 5 лет. Выявлено, что в последнее 5-летие в структуре флебологической помощи города преобладают амбулаторные эндоваскулярные и миниинвазивные процедуры на стволах подкожных вен. Они выполняются в 2,5 раза чаще, чем классические флебэктомии. Значимо расширился спектр медицинских услуг во флебологии, что сделало подход к пациенту более индивидуальным, улучшило качество помощи и повысило ее безопасность. Затраты бюджета от внедрения миниинвазивных технологий (только за счет экономии затрат стационара) уменьшились на 187,44 млн.руб /год

    Thromboelastography in the practice of hematologist in monitoring pregnant women with disorders of hemostasis

    The article analyses the opportunities thromboelastography for monitoring of hemostasis in pregnant women with thrombophilia. Using the thromboelastography assessed the State of the plasma parameters of hemostasis, fibrinolysis system, and adjust the amount of assigned patients to drugs (anticoagulants, antiagregantov, inhibitors of fibrinolysis). Performance of thromboelastography in dynamics made it possible to assess the effectiveness of the molecular heparin therapy during pregnancy and to identify appropriate ways of its correction in preparation for childbirth to prevent bleeding. Conducting a functional test with heparinaza enhances diagnostic validity thromboelastography.В статье проанализированы возможности тромбозластографии для контроля за состоянием гемостаза у беременных женщин с генетически обусловленной тромбофилией и гемостазиологическими нарушениями при подготовке к родам. С помощью тромбозластографии оценивали состояние плазменного, тромбоцитарного гемостаза и системы фибринолиза, корректировали объем назначенных пациентам препаратов (антикоагулянтов, антиагрегантов, ингибиторов фибринолиза). Выполнение тромбозластографии в динамике позволило оценить эффективность терапии низкомолекулярным гепарином в течение беременности и наметить адекватные пути ее коррекции при подготовке к родам для профилактики кровотечений. Проведение функциональной пробы с гепариназой расширяет диагностическую значимость тромбозластографии

    Новая мутация в гене TRIP4, ассоциированная с фенотипом врожденной мышечной дистрофии типа Давиньон–Шове (клинический случай)

    Congenital muscular dystrophies are heterogeneous groups of neuromuscular diseases leading to hypotonia, progressive muscle weakness and dystrophic or structural signs in muscle biopsy. At the present time, 34 genes associated with congenital muscular dystrophy have been described. The clinical case of a rare form of congenital muscular dystrophia associated with a homozygous mutation in the TRIP4 gene in a patient with respiratory failure requiring respiratory support, neurological symptoms, muscular hypotonia, and multiple congenital malformations of skeletal system is presented for the first time in Russia. The undescribed pathogenic homozygous variant of the nucleotide sequence in the TRIP4 gene (chr15:64686179, c.136C>T, p.Arg46Ter, 2 exon, NM_016213.4) was detected by whole exome sequencing. The mutation in the TRIP4 gene was validated by Sanger sequencing in a child and its origin was investigated. The mother and father of the girl are carriers of the heterozygous variant in the TRIP4 gene. Identification of the genetic cause of a rare form of neuromuscular disease is important for determining the tactics of patient management and medical and genetic counseling of the family, as well as clarifying the pathogenesis of a rare pathology. Врожденные мышечные дистрофии и врожденные миопатии представляют собой гетерогенную группу нервно-мышечных заболеваний, приводящих к гипотонии, прогрессирующей мышечной слабости и дистрофическим или структурным признакам при мышечной биопсии. В настоящее время описано 34 гена, связанных с врожденной мышечной дистрофией. Впервые в России представляется клинический случай редкой формы врожденной мышечной дистрофии, обусловленной гомозиготной мутацией в гене TRIP4, у пациента с дыхательной недостаточностью, требующей респираторной поддержки, неврологической симптоматикой, мышечной гипотонией, множественными врожденными пороками развития опорно-двигательной системы. В результате проведенного полноэкзомного секвенирования выявлен ранее не описанный патогенный вариант нуклеотидной последовательности в гене TRIP4 в гомозиготном состоянии, приводящий к остановке синтеза полнофункционального белка (chr15:64686179, c.136C>T, p.Arg46Ter, 2 й экзон, NM_016213.4). Мутация в гене TRIP4 была валидирована методом секвенирования по Сэнгеру у ребенка, и исследовано ее происхождение. Мать и отец девочки являются носителями гетерозиготного варианта в гене TRIP4. Выявление генетической причины редкой формы нервно-мышечного заболевания важно для определения тактики ведения пациента и медико-генетического консультирования семьи, а также уточнения патогенеза редкой патологии
