    Angiographic determinants for side branch treatment in distal unprotected left main coronary artery bifurcation percutaneous coronary intervention with single stent crossover technique

     Background: The aim of this study was to analyze angiographic parameters such as bifurcation angle, diameter at the polygon of confluence (POC) and SYNTAX score in predicting the need for side branch treatment with single-stent crossover technique from distal unprotected left main coronary artery (ULMCA) to the major side branch.Methods: This was a retrospective and observational study. A total of 83 patients with distal ULMCA lesions were enrolled. Patients who underwent provisional single-stent crossover technique were compared with patients that required side branch treatment though plain old balloon angioplasty (POBA) 5 (6.0%) or stenting 7 (8.4%). Angiographic parameters, bifurcation angle, diameter at the POC and the SYNTAX score were analyzed for their ability to predict the need for side branch treatment.Results: Mean age of patients was 58.2±5.3 years. Males constituted 61 (73.5%) patients. Post main branch stenting 5 (6.0%) and 7 (8.4%) patients required side branch treatment with POBA and stent implantation, respectively. Binary regression revealed bifurcation angle (Z: 1.15, OR: 0.99, 95% CI: 0.97-1.01, p=0.25), diameter at the POC (Z: 1.1, OR: 0.82, 95% CI: 0.32-2.13, p=0.272) and SYNTAX score (Z: 1.51, OR: 1.18, 95% CI: 0.95-1.45, p=0.132) did not correctly predict the requirement of side branch treatment.Conclusion: Left main coronary artery to main branch stenting can be performed safely with single-stent crossover technique, irrespective of presence of side branch disease across a wide range of bifurcation angles, diameters at the POC and SYNTAX scores.

    AIM OF THE STUDY: I. Demographic analysis of brachial plexus injury regarding 1. Age, 2. Gender, 3. Various Levels of Injury, 4. Side of Injury, 5. Mode of Injury, 6. Occupation, 7. Association Injuries and, 8. Comparative Study of Total B.P.I and upper B.P.I. II. To analyze the functional outcome of 20 upper brachial plexus injury patients out of 35 total brachial plexus injury cases assessed during the period of 4 months and followed up for nearly 10 months. MATERIALS AND METHODS: We have clinically assessed 35 patients of brachial plexus injury who presented at the Institute of Hand and Reconstructive Microsurgery and Department of Plastic Surgery from 1.5.2010 to 30.9.2010 and were followed for nearly 10 months. Assessment of each patient made and recording made in the proforma shown in subsequent pages. After the history recording Inspection finding recorded and main examination is to exclude the involvement of root. Examination of suprascapular muscle and Rhomboides muscle are important. How to examine the muscle demonstrated in the picture. We can see the contraction of Rhomboidus Muscle on adduction of scapula and we need to palpate the contraction of the muscle adduction of scapula against resistance. Same way Latismus dorsi muscle also seen contracting on coughing we need to palpate contraction of the muscle when shoulder is extended and adducted against resistance. Pectoralis major is having clavicular and sternal origin should be examined individually and recorded. CONCLUSION: The study revealed that the productive age group of 21 to 30 years was commonly injured with brachial plexus injury. It was the males who were mostly involved. Total brachial plexus injury formed a large chunk of the patients with brachial plexus injuries, but the second commonest involvement was the upper trunk lesion of C5,6 or C5,6,7. It was commonly the right side that was involved and thus involved the dominant hand. Road traffic accidents with two wheelers formed the majority of cases with brachial plexus injuries. The occupation of the patient did not show any significant difference whether student, manual labourer of sedentary labourer was concerned. The demographic pattern of injuries of brachial plexus was almost similar when the total plexus injury and the upper plexus injury were concerned, except in the presence of associated injuries, where, upper lesions appeared to have more percentage of associated injuries. Outcome analysis revealed good results in surgical treatment of upper brachial plexus lesions, except in the Oberlin procedure of nerve transfer where the results were comparatively poorer. Management of the patient is difficult pre and post operatively. A correct evaluation of the patient pre operatively, and planning and execution of the correct surgical procedure and post surgical rehabilitation are essential. Nevertheless, with a highly motivated patient and a dedicated and specialized surgical team, the prognosis for functional recovery is good, especially in upper brachial plexus injuries, and these patients can still lead productive and satisfying lives. Education of public regarding speed control, obeying traffic rules should be done especially for college students for prevention of brachial plexus injuries, which is much easier than curing these problems

    The study has been carried out to investigate the effects of single factors such as temperature, extraction time, concentration of ethanol, material ratio and no. of extractions on the contents of flavonoids present in the leaves of Polyalthia longifolia. On this basis, an L16 orthogonal design of experiment was used to determine the optimal conditions for the extraction of flavonoids. The amount of flavonoids extracted reached its maxima at 65 ºC for 2 hrs by using 75% ethanol (modifier) with a material ratio of 1:10 and 2 times ofextraction.     Research productivity on COVID-19: A Bibliometric approach

    The study aimed to explore the perspective of COVID-19 publications across the globe. Further, attempts were also made to find out the most productive country, author, and institute in publishing literature on COVID-19. The data related study was retrieved from SCOPUS database. Various search strategies were used to retrieve publications on COVID-19 published between 1st December 2019 and 22nd June 2020. A total of 19,991 publications on COVID-19 were retrieved from the Scopus database. Further, it was found that Huazhong University of Science and Technology, China has produced 422 publications and received 5624 citations with an average citation of 13.327 per publication. BMJ Clinical Research Ed published 534 (1.61%) articles. Among the most productive authors, Wiwanitkit, Viroj from Hainan Medical University, China stands in the first place with 73 publications followed by Mahase, E. from the British Medical Journal, UK (52 publications) and Iacobucci, Gareth (48 publications). The study provided a relatively objective reference for peer scientists, national regimes, and the global health system. The findings of the study will definitely help the institutions as well as authors to get an opportunity to collaborate with regional, national and international research institutions and scientists

    Novel titanium - hydroxyapatite biocomposites By mechanical milling

    Titanium with nano hydroxyapatite (nHA) as well as with micron size hydroxyapatite was ball milled for various time intervals. The milled powders were characterized using X-Ray diffraction (XRD), scanning electron microscope (SEM) and transmission electron microscope (TEM) techniques. The XRD results show peak broadening with increase in milling time indicating the formation of nanocomposite with titanium and HA phases. Relatively, titanium was found to be more reduced in size with increase in milling time than HA in the composite. The EDX results indicated the presence of Ca and P in the titanium matrix. elemental mapping showed homogeneous distribution of Ti, Ca and P after 16 h of ball milling in all the composites. TEM micrographs also revealed the presence of nanocrystals. The powders were compacted, sintered and immersed in simulated body fluid (SBF), which has the same ion concentration as that of the human body plasma, to study bioactivity under physiological condition. After immersion in SBF for 1 week, the compacts were observed in SEM and all the composites were found to be bioactive with the formation of bone like apatite layer on the surface. The morphology of the apatite formed was globular and more dense for the composite prepared using nano HA compared to micron size HA
