    Health Workforce Remuneration: comparing wage levels, ranking and dispersion of 16 occupational groups in 20 countries

    BackgroundThis article represents the first attempt to explore remuneration in Human Resources for Health (HRH), comparing wage levels, ranking and dispersion of 16 HRH occupational groups in 20 countries (Argentina, Belarus, Belgium, Brazil, Chile, Colombia, the Czech Republic, Finland, Germany, India, Mexico, the Netherlands, Poland, Russian Federation, Republic of South Africa (RSA), Spain, Sweden, Ukraine, United Kingdom (UK), and United States of America (USA)). The main aim is to examine to what extent the wage rankings, standardized wage levels, and wage dispersion are similar between the 16 occupational groups and across the selected countries and what factors can be shown to be related to the differences that emerge. MethodThe pooled data from the continuous, worldwide, multilingual WageIndicator web survey between 2008 and 2011 (for selected HRH occupations, n=49,687) have been aggregated into a data file with median or mean remuneration values for 300 occupation/country cells. Hourly wages are expressed in standardized US Dollars (USD), all controlled for purchasing power parity (PPP) and indexed to 2011 levels. ResultsThe wage ranking of 16 HRH occupational groups is fairly similar across countries. Overall Medical Doctors have the highest and Personal Care Workers the lowest median wages. Wage levels of Nursing &amp; Midwifery Professionals vary largely. Health Care Managers have lower earnings than Medical Doctors in all except six of the 20 countries. The largest wage differences are found for the Medical Doctors earning 20 times less in Ukraine than in the US, and the Personal Care Workers, who earn nine times less in the Ukraine than in the Netherlands. No support is found for the assumption that the ratio across the highest and lowest earning HRH occupations is similar between countries: it varies from 2.0 in Sweden to 9.7 in Brazil. Moreover, an increase in the percentage of women in an occupation has a large downward effect on its wage rank. ConclusionsThis article breaks new ground by investigating for the first time the wage levels, ranking, and dispersion of occupational groups in the HRH workforce across countries. The explorative findings illustrate that the assumption of similarity in cross-country wage ranking holds, but that wage dispersion and wage levels are not similar. These findings might contribute to the policies for health workforce composition and the planning of healthcare provisions. <br/

    The evolving galaxy is considered as a system of baryonic fragments embedded into the static dark nonbaryonic (DH) and baryonic (BH) halo and subjected to gravitational and viscous interactions. Although the chemical evolution of each separate fragment is treated in the frame of one -- zone close box model with instantaneous recycling, its star formation (SF) activity is a function of mean local gas density and, therefore, is strongly influenced by other interacting fragments.     Fast and accurate spike sorting of high-channel count probes with KiloSort

    New silicon technology is enabling large-scale electrophysiological recordings in vivo from hundreds to thousands of channels. Interpreting these recordings requires scalable and accurate automated methods for spike sorting, which should minimize the time required for manual curation of the results. Here we introduce KiloSort, a new integrated spike sorting framework that uses template matching both during spike detection and during spike clustering. KiloSort models the electrical voltage as a sum of template waveforms triggered on the spike times, which allows overlapping spikes to be identified and resolved. Unlike previous algorithms that compress the data with PCA, KiloSort operates on the raw data which allows it to construct a more accurate model of the waveforms. Processing times are faster than in previous algorithms thanks to batch-based optimization on GPUs. We compare KiloSort to an established algorithm and show favorable performance, at much reduced processing times. A novel post-clustering merging step based on the continuity of the templates further reduced substantially the number of manual operations required on this data, for the neurons with near-zero error rates, paving the way for fully automated spike sorting of multichannel electrode recordings

    Outcomes of combined hip procedure with dual mobility cup versus osteosynthesis for acetabular fractures in elderly patients: a retrospective observational cohort study of fifty one patients.

    Acetabular fractures are more and more common in the elderly. Open reduction and internal fixation (ORIF) may lead to poor outcomes and high revision rates. Primary total hip arthroplasty (THA) combined with internal fixation, also known as the combined hip procedure (CHP), associated with dual mobility cup (DM-CHP) could be an efficient procedure in selected elderly patients. The aim of this study is to compare functional and radiological outcomes between ORIF and DM-CHP. Between 2007 and 2018, 51 patients older than 65 years were surgically treated for acetabular fractures. Twenty-six patients were treated by DM-CHP and 25 by ORIF. Each group was divided into two subgroups regarding a single or combined approach. Hospital stay, surgical time, intraoperative blood loss, and complications were documented. The Harris Hip Score (HHS) was used for measuring the functional outcome. Radiological analysis was used to assess the centre of rotation in the DM-CHP group. Median surgery time and intra-operative blood loss were higher in DM-CHP than those in ORIF. Early medical complication rate was higher for a combined approach as compared with a single posterior approach in DM-CHP (p = 0.003). Dislocation rate was 7.7% in DM-CHP. Revision rate was higher in ORIF (20% versus 7.7%). HHS was similar in both groups. DM-CHP leads to similar functional outcomes and less revision than ORIF. This study strengthens the practice of using only the posterior approach for primary THA in the elderly. Dual mobility is a valid therapeutic option for acetabular fractures in elderly patients

    The structure of dark matter halos in hierarchical clustering theories

    During hierarchical clustering, smaller masses generally collapse earlier than larger masses and so are denser on the average. The core of a small mass halo could be dense enough to resist disruption and survive undigested, when it is incorporated into a bigger object. We explore the possibility that a nested sequence of undigested cores in the center of the halo, which have survived the hierarchical, inhomogeneous collapse to form larger and larger objects, determines the halo structure in the inner regions. For a flat universe with P(k)knP(k) \propto k^n, scaling arguments then suggest that the core density profile is, ρrα\rho \propto r^{-\alpha} with α=(9+3n)/(5+n)\alpha = (9+3n)/(5+n). But whether such behaviour obtains depends on detailed dynamics. We first examine the dynamics using a fluid approach to the self-similar collapse solutions for the dark matter phase space density, including the effect of velocity dispersions. We highlight the importance of tangential velocity dispersions to obtain density profiles shallower than 1/r21/r^2 in the core regions. If tangential velocity dispersions in the core are constrained to be less than the radial dispersion, a cuspy core density profile shallower than 1/r cannot obtain, in self-similar collapse. We then briefly look at the profiles of the outer halos in low density cosmological models where the total halo mass is convergent. Finally, we analyze a suite of dark halo density and velocity dispersion profiles obtained in cosmological N-body simulations of models with n= 0, -1 and -2. We find that the core-density profiles of dark halos, show considerable scatter in their properties, but nevertheless do appear to reflect a memory of the initial power spectrum, with steeper initial spectra producing flatter core profiles.
