    Correlation of adrenomedullin gene expression in peripheral blood leukocytes with severity of ischemic stroke

    Human adrenomedullin (ADM), a 52-amino acid peptide, belongs to the calcitonin/calcitonin gene-related peptide (CGRP)/amylin peptide family. ADM acts as a multifunctional regulatory peptide and is upregulated in response to hypoxia. Previous microarray studies have found increased ADM gene (ADM) expression in peripheral blood cells of patients with stroke, however, it is unknown if an increased ADM level is correlated with severity of human ischemic stroke. This study investigated ADM expression in peripheral blood leukocytes (PBL) of healthy controls and subjects at day 1, week 1 and week 3 postacute ischemic stroke using rtPCR methodology. We found that ADM expression was significantly upregulated on the first day of stroke compared to the healthy subjects and the disease controls; the levels remained elevated for up to week 3. Further, ADM expression at day 1 was correlated with stroke severity measured by the National Institute of Healthy Stroke Scale (NIHSS), the modified Barthel Index (mBI) and the modified Rankin Scale (mRS). This could indicate that ADM expression level is related to the severity of tissue damage. We suggest that increased ADM expression in PBL after acute ischemic stroke is most likely to indicate that these cells have been subjected to hypoxia and that the magnitude of expression is likely to be related to the volume of hypoxic tissue. Hypoxia can affect lymphocytes function and could affect the immune response to stroke. The correlation of ADM expression level with the measures of stroke severity implicates ADM - a potential blood bio-marker in studies of ischemic stroke

    Combination therapy in hypertension: An update

    Meticulous control of blood pressure is required in patients with hypertension to produce the maximum reduction in clinical cardiovascular end points, especially in patients with comorbidities like diabetes mellitus where more aggressive blood pressure lowering might be beneficial. Recent clinical trials suggest that the approach of using monotherapy for the control of hypertension is not likely to be successful in most patients. Combination therapy may be theoretically favored by the fact that multiple factors contribute to hypertension, and achieving control of blood pressure with single agent acting through one particular mechanism may not be possible. Regimens can either be fixed dose combinations or drugs added sequentially one after other. Combining the drugs makes them available in a convenient dosing format, lower the dose of individual component, thus, reducing the side effects and improving compliance. Classes of antihypertensive agents which have been commonly used are angiotensin receptor blockers, thiazide diuretics, beta and alpha blockers, calcium antagonists and angiotensin-converting enzyme inhibitors. Thiazide diuretics and calcium channel blockers are effective, as well as combinations that include renin-angiotensin-aldosterone system blockers, in reducing BP. The majority of currently available fixed-dose combinations are diuretic-based. Combinations may be individualized according to the presence of comorbidities like diabetes mellitus, chronic renal failure, heart failure, thyroid disorders and for special population groups like elderly and pregnant females

    The term vascular remodeling is commonly used to define the structural changes in blood vessel geometry that occur in response to long-term physiologic alterations in blood flow or in response to vessel wall injury brought about by trauma or underlying cardiovascular diseases.1, 2, 3, 4 The process of remodeling, which begins as an adaptive response to long-term hemodynamic alterations such as elevated shear stress or increased intravascular pressure, may eventually become maladaptive, leading to impaired vascular function. The vascular endothelium, owing to its location lining the lumen of blood vessels, plays a pivotal role in regulation of all aspects of vascular function and homeostasis.5 Thus, not surprisingly, endothelial dysfunction has been recognized as the harbinger of all major cardiovascular diseases such as hypertension, atherosclerosis, and diabetes.6, 7, 8 The endothelium elaborates a variety of substances that influence vascular tone and protect the vessel wall against inflammatory cell adhesion, thrombus formation, and vascular cell proliferation.8, 9, 10 Among the primary biologic mediators emanating from the endothelium is nitric oxide (NO) and the arachidonic acid metabolite prostacyclin [prostaglandin I2 (PGI2)], which exert powerful vasodilatory, antiadhesive, and antiproliferative effects in the vessel wall

    Improvement of the convergence property of adaptive feedforward controllers and their application to the active control of ship interior noise

    Various structural measures against vibration and noise were taken in a training ship, the Oshima Maru. However, an unpleasant sound persisted in the mess hall, where crews take their breaks. To reduce the noise, active controllers were investigated to satisfy the causality constraint in their update. Some of the controllers were preconditioned using the inverse of the plant because their convergence rates are limited by the dynamics and coupling within the plant response. The overall response from the output of the control filter to the output of the error sensor is thus equal to the all-pass part of the plant response. Because this response has a flat magnitude, the convergence speed of the adaptive algorithm is not affected by resonances in the plant response, as it is for the normal filtered reference LMS algorithm. The algorithms were compared under the same conditions to investigate differences in their properties and also corrected to satisfy the causality of their update processes. A comparison of convergence properties shows that satisfying the causality constraint in their update results in an improvement, and consequently the effect was confirmed by the filtered reference - filtered error LMS algorithm. Simulations are presented for a control system that was introduced using plant responses measured from a loudspeaker to a microphone in the mess hall inside the Oshima Maru. After investigating the convergence speed in various gradient descent adaptation algorithms, the results were integrated with the actual plant response and applied to the active control of ship interior noise. It was also found that although the preconditioned LMS algorithm converges dramatically faster than the ordinary gradient descent adaptation algorithms with an accurate plant model, its convergence rate is still sensitive to the auto-correlation and cross-correlation properties of the reference signals
