    Optical pulse propagation in a switched-on photonic lattice: Rabi effect with the roles of light and matter interchanged

    A light pulse propagating in a suddenly switched on photonic lattice, when the central frequency lies in the photonic band gap, is an analog of the Rabi model where the two-level system is the two resonant (i.e. Bragg-coupled) Fourier modes of the pulse, while the photonic lattice serves as a monochromatic external field. A simple theory of these Rabi oscillations is given and confirmed by the numerical solution of the corresponding Maxwell equations. This is a direct, i.e. temporal, analog of the Rabi effect, additionally to the spatial analog in optical beam propagation described in Opt. Lett. 32, 1920 (2007). An additional high-frequency modulation of the Rabi oscillations reflects the lattice-induced energy transfer between the electric and magnetic fields of the pulse.Comment: 3 pages, 5 figure

We consider the association between enhanced government oversight and financial reporting aggressiveness in the healthcare industry. Specifically, we examine the association between criminal cases (pending cases and successful cases) against healthcare providers and the quality of subjective accounts that require significant judgment and have been shown to be linked to healthcare earnings management – revenue accruals and the allowance for doubtful accounts.     Photonic Bloch oscillations and Zener tunneling in two-dimensional optical lattices

    We report on the first experimental observation of photonic Bloch oscillations and Zener tunneling in two-dimensional periodic systems. We study the propagation of an optical beam in a square photonic lattice superimposed on a refractive index ramp, and demonstrate the tunneling of light from the first to the higher-order transmission bands of the lattice bandgap spectrum, associated with the spectral dynamics inside the first Brillouin zone and corresponding oscillations of the primary beam.Comment: 4 pages, 4 figure

    Nonlinear localized modes at phase-slip defects in waveguide arrays

    We study light localization at a phase-slip defect created by two semi-infinite mismatched identical arrays of coupled optical waveguides. We demonstrate that the nonlinear defect modes possess the specific properties of both nonlinear surface modes and discrete solitons. There remains debate as to the optimal treatment for a given patient and fracture. Despite the popularity of volar locking plate fixation, there are few large cohort or long term follow up studies to justify this modality. Our aim was to report the functional outcome of a large number of patients at a significant follow up time after fixation of their distal radius with a volar locking plate. Methods 180 patients with 183 fractures and a mean age of 62.4 years were followed up retrospectively at a mean of 30 months (Standard deviation = 10.4). Functional assessment was performed using the Disabilities of the Arm, Shoulder and Hand (DASH) and modified MAYO wrist scores. Statistical analysis was performed to identify possible variables affecting outcome and radiographs were assessed to determine time to fracture union. Results The median DASH score was 2.3 and median MAYO score was 90 for the whole group. Overall, 133 patients (74%) had a good or excellent DASH and MAYO score. Statistical analysis showed that no specific variable including gender, age, fracture type, post-operative immobilisation or surgeon grade significantly affected outcome. Complications occurred in 27 patients (15%) and in 11 patients were major (6%). Conclusion This single centre large population series demonstrates good to excellent results in the majority of patients after volar locking plate fixation of the distal radius, with complication rates comparable to other non-operative and operative treatment modalities. On this basis we recommend this mode of fixation for distal radius fractures requiting operative intervention.Peer Reviewe

    Short courses of daily prednisolone during upper respiratory tract infections reduce relapse frequency in childhood nephrotic syndrome

    Background Relapses of childhood nephrotic syndrome (NS) are frequently precipitated by viral upper respiratory tract infections (URTIs). A review of the literature reveals that in patients with steroid-dependent NS on alternate day corticosteroids, a short course of daily corticosteroid therapy during the course of an URTI may reduce relapse frequency. Objective To assess the effect of a short course of low-dose corticosteroid therapy during the course of an URTI on relapse frequency in patients with steroid-sensitive NS who have not been taking any treatment for a minimum period of 3 months. Methods A double-blind placebo-controlled crossover trial was conducted on 48 patients with idiopathic NS who had not been receiving corticosteroid therapy for a minimum of 3 months. Patients were randomized into two groups. Group A received 5 days of daily prednisolone at 0.5 mg/kg at the onset of an URTI while group B received 5 days of placebo. Both groups were followed up for 1 year and the URTI-induced relapse frequency was noted. A crossover was performed during the next year, with group A receiving placebo and group B receiving prednisolone. Results Thirty-three patients completed the study. In the treatment group, 115 episodes of URTI led to 11 relapses while in the control group 101 episodes of URTI led to 25 relapses. There was no significant difference between the mean number of URTIs between the treatment and control groups. The treatment group had significantly less relapses compared to the control group (p = 0.014). Within the treatment group, 65.6% did not relapse, while the remainder had a single relapse. In contrast, only 40.6% of the control group remained in remission while 40.6% suffered a single relapse and 18.8% had two or more relapses. Conclusions Prescribing a short course of daily corticosteroids during an URTI significantly reduces the frequency of URTI-induced relapse in patients with steroid-responsive NS who are off corticosteroid therapy

    Observation of localized modes at phase slips in two-dimensional photonic lattices

    We study experimentally light localization at phase-slip waveguides and at the intersection of phase-slips in a two-dimensional (2D) square photonic lattice.
