    Mandibular reconstruction with bridging customized plate after ablative surgery for ONJ: A multi‐centric case series

    Purpose: Computer‐aided methods for mandibular reconstruction have improved both functional and morphological results in patients who underwent segmental mandibular resection. The purpose of this study is to evaluate the overlaying of virtual planning in terms of measures of the Computer Assisted Design/Computer Assisted Manufacturing CAD/CAM plate for mandibular reconstruction in patients who are ineligible for the insertion of reconstructing the titanium plate supported by fibular free flap, due to their poor health status, or in the presence of specific contraindications to autologous bone flap harvest. Materials and methods: The retrospective study performed analyzed the results of nine patients. The patients were treated at the Maxillofacial Surgery Unit of Policlinico S. Orsola of Bologna, Italy, and Policlinico San Marco, Catania, Italy, from April 2016 to June 2021. Superimposition between planning and post operative Computed Tomography CT scan was performed to assess the accuracy. Results: All reconstructive procedures were carried out successfully. No microsurgery‐related complications occurred. In two cases, we had plate misplacement, and in one case, plate exposure that led to plate removal. The average accuracy of the series assessed after CT superimposition, as previously described, was 0.95 mm. Conclusions: Considering that microvascular bone transfer is a high‐risk procedure in BRONJ patients, we can conclude that the positioning of a customized bridging mandibular prosthesis (CBMP), whether or not it is associated with a microvascular soft tissue transfer, is a safe technique in terms of surgical outcome and feasibility

    Simulation of nitrous oxide emissions at field scale using the SPACSYS model

    AbstractNitrous oxide emitted to the atmosphere via the soil processes of nitrification and denitrification plays an important role in the greenhouse gas balance of the atmosphere and is involved in the destruction of stratospheric ozone. These processes are controlled by biological, physical and chemical factors such as growth and activity of microbes, nitrogen availability, soil temperature and water availability. A comprehensive understanding of these processes embodied in an appropriate model can help develop agricultural mitigation strategies to reduce greenhouse gas emissions, and help with estimating emissions at landscape and regional scales. A detailed module to describe the denitrification and nitrification processes and nitrogenous gas emissions was incorporated into the SPACSYS model to replace an earlier module that used a simplified first-order equation to estimate denitrification and was unable to distinguish the emissions of individual nitrogenous gases. A dataset derived from a Scottish grassland experiment in silage production was used to validate soil moisture in the top 10cm soil, cut biomass, nitrogen offtake and N2O emissions. The comparison between the simulated and observed data suggested that the new module can provide a good representation of these processes and improve prediction of N2O emissions. The model provides an opportunity to estimate gaseous N emissions under a wide range of management scenarios in agriculture, and synthesises our understanding of the interaction and regulation of the processes

    Economical viabiality of the melon crop (Cucumis melo L.) in the Ilha Solteira region

    Com o objetivo de se verificar a viabilidade econômica da cultura do melão, na região de Ilha Solteira, ano agrícola 1991/92, procedeu-se a uma análise econômica para estudar o comportamento de dois cultivares de melão Eldorado 300 e Valenciano Amarelo CAC, variando a adubação de cobertura, ou seja, utilizando-se 3 doses de N e de K2O: 2,5; 5,0 e 10,0 g/planta. O experimento foi irrigado através de um sistema de gotejamento, sendo que o investimento inicial para aquisição do material de irrigação para uma área de 1 ha, foi da ordem de US11.500,00.Apesardoaltovalordoinvestimentoinicial,aculturasemostroualtamenterentaˊvel.Comosresultadosobtidos,verificousequetodosostratamentosapresentaramrentabilidadepositiva,sendoqueamaiorreceitalıˊquidaencontradafoideUS 11.500,00. Apesar do alto valor do investimento inicial, a cultura se mostrou altamente rentável. Com os resultados obtidos, verificou-se que todos os tratamentos apresentaram rentabilidade positiva, sendo que a maior receita líquida encontrada foi de US 4.048,00 do resultado econômico, gerado pela diferença entre a receita bruta e o custo total, recebe influência direta dos níveis de produtividade e do preço alcançado pelo produto.To verify the economical viability of the melon crop in Ilha Solteira, this experiment was carried out during 1991/1992 performing an economical analysis of the behavior of two melon varieties: Eldorado 300 and Valenciano Amarelo CAC, varying the fertilizer dressing: 2.5; 5.0 and 10.0 g/plant of N and K2O. The experimental area was irrigated with a drop irrigation system, of an initial investment of US11,500.00perhectare.Inspiteofthehighinitialinvestment,thecropshowedtobehighlyprofitable.Theobtainedresultsshowthatthealltreatmentspresentpositiveproductivity,withahighnetincomeofUS 11,500.00 per hectare. In spite of the high initial investment, the crop showed to be highly profitable. The obtained results show that the all treatments present positive productivity, with a high net income of US 4,048.00 per hectare. The economic result caused by the difference between the gross income and the total cost, is directly influenced by the productivity levels and the prices of the product obtained in the market

    Design of a customised bridging mandibular prosthesis for complex reconstruction: a pilot study

    La ricostruzione mandibolare è attualmente effettuata mediante il trasferimento di lembi liberi rivascolarizzati di tessuto osseo, supportati da placche di osteosintesi. Sebbene questa procedura sia generalmente efficace e riproducibile, talvolta la scarsa prognosi oncologica o le condizioni cliniche scadenti del paziente costringono il chirurgo a considerare alternative ricostruttive. In tali casi, la principale possibilità è rappresentata dallutilizzo di placche ricostruttive a ponte, associate a lembi liberi di tessuti molli. Comunque la ricostruzione così concepita espone a un significativo rischio di sviluppare complicanze di vario genere. Le più frequenti complicanze sono rappresentate dalla rottura e dallesposizione della placca.     Symbolic Implementation of Extensions of the PyCosmo\texttt{PyCosmo} Boltzmann Solver

    PyCosmo\texttt{PyCosmo} is a Python-based framework for the fast computation of cosmological model predictions. One of its core features is the symbolic representation of the Einstein-Boltzmann system of equations. Efficient C/C++\texttt{C/C++} code is generated from the SymPy\texttt{SymPy} symbolic expressions making use of the sympy2c\texttt{sympy2c} package. This enables easy extensions of the equation system for the implementation of new cosmological models. We illustrate this with three extensions of the PyCosmo\texttt{PyCosmo} Boltzmann solver to include a dark energy component with a constant equation of state, massive neutrinos and a radiation streaming approximation. We describe the PyCosmo\texttt{PyCosmo} framework, highlighting new features, and the symbolic implementation of the new models. We compare the PyCosmo\texttt{PyCosmo} predictions for the Λ\LambdaCDM model extensions with CLASS\texttt{CLASS}, both in terms of accuracy and computational speed. We find a good agreement, to better than 0.1% when using high-precision settings and a comparable computational speed.     Pre-operative evaluation of dna methylation profile in oral squamous cell carcinoma can predict tumor aggressive potential

    Background: Prognosis of oral squamous cell carcinoma (OSCC) is difficult to exactly assess on pre-operative biopsies. Since OSCC DNA methylation profile has proved to be a useful pre-operative diagnostic tool, the aim of the present study was to evaluate the prognostic impact of DNA methylation profile to discriminate OSCC with high and low aggressive potential. Methods: 36 OSCC cases underwent neoplastic cells collection by gentle brushing of the lesion, before performing a pre-operative biopsy. The CpG islands methylation status of 13 gene (ZAP70, ITGA4, KIF1A, PARP15, EPHX3, NTM, LRRTM1, FLI1, MiR193, LINC00599, MiR296, TERT, GP1BB) was studied by bisulfite Next Generation Sequencing (NGS). A Cox proportional hazards model via likelihood-based component-wise boosting was used to evaluate the prognostic power of the CpG sites. Results: The boosting estimation identified five CpGs with prognostic significance: EPHX3-24, EPHX3-26, ITGA4-3, ITGA4-4, and MiR193-3. The combination of significant CpGs provided promising results for adverse events prediction (Brier score = 0.080, C-index = 0.802 and AUC = 0.850). ITGA4 had a strong prognostic power in patients with early OSCC. Conclusions: These data confirm that the study of methylation profile provides new insights into the molecular mechanisms of OSCC and can allow a better OSCC prognostic stratification even before surgery

    Shared epigenetic alterations between oral cancer and periodontitis: A preliminary study

    Introduction: We recently developed a non-invasive sampling procedure for oral squamous cell carcinoma (OSCC) detection based on DNA methylation analysis of a panel of 13 genes. Oral cancer, as well as acute and chronic inflammatory diseases, may influence the methylation level of several genes in the oral cavity. In the present study, we evaluated the presence of periodontal disease (PD) and the methylation status using our 13-gene panel. Methods: Oral brushing specimens were collected from three different patient groups: 23 gingival OSCC patients, 15 patients affected by PD, and 15 healthy volunteers lacking evidence of PD. DNA methylation analysis was performed and each sample was determined to be positive or negative based on a predefined cut-off value. Results: Positive results were found for 23/23 OSCC patients, 3/15 PD patients, and 0/15 samples from healthy volunteers. The GP1BB and MIR193 genes in the PD group exhibited mean methylation levels similar to OSCC patients. ZAP70 showed different methylation levels among three groups. Conclusion: Preliminary data identified shared epigenetic alterations between PD and OSCC patients in two inflammatory genes (GP1BB and MIR193). This study may help to identify potential links between the two diseases and serve as a starting point for the future research focused on pathogenesis

    An Evolutionary Cancer Epigenetic Approach Revealed DNA Hypermethylation of Ultra-Conserved Non-Coding Elements in Squamous Cell Carcinoma of Different Mammalian Species

    BACKGROUND: Ultra-conserved non-coding elements (UCNEs) are genomic sequences that exhibit > 95% sequence identity between humans, mammals, birds, reptiles, and fish. Recent findings reported their functional role in cancer. The aim of this study was to evaluate the DNA methylation modifications of UNCEs in squamous cell carcinoma (SCC) from different mammal species. METHODS: Fifty SCCs from 26 humans, 17 cats, 3 dogs, 1 horse, 1 bovine, 1 badger, and 1 porcupine were investigated. Fourteen feline stomatitis and normal samples from 36 healthy human donors, 7 cats, 5 dogs, 5 horses, 2 bovines and 1 badger were collected as normal controls. Bisulfite next generation sequencing evaluated the DNA methylation level from seven UCNEs (uc.160, uc.283, uc.416, uc.339, uc.270, uc.299, and uc.328). RESULTS: 57/59 CpGs were significantly different according to the Kruskal-Wallis test (p < 0.05) comparing normal samples with SCC. A common DNA hypermethylation pattern was observed in SCCs from all the species evaluated in this study, with an increasing trend of hypermethylation starting from normal mucosa, through stomatitis to SCC. CONCLUSIONS: Our findings indicate that UCNEs are hypermethylated in human SCC, and this behavior is also conserved among different species of mammals

    Dieta com baixo teor de FODMAPs para distúrbios de dor abdominal funcional em crianças: revisão crítica do conhecimento atual (Low FODMAPs diet for functional abdominal pain disorders in children: critical review of current knowledge)

    Objective: This narrative review aimed to provide practitioners a synthesis of the current knowledge on the role of a low Fermentable Oligosaccharides Disaccharides Monosaccharides and Polyols diet in reducing symptoms associated with functional abdominal pain disorders in children. This review is focused on the pathophysiology, efficacy and criticism of low Fermentable Oligosaccharides Disaccharides Monosaccharides and Polyols diet in children. / Sources: Cochrane Database, Pubmed and Embase were searched using specific terms for Fermentable Oligosaccharides Disaccharides Monosaccharides and Polyols diet interventions and functional abdominal pain disorders. / Summary of the findings: In children, only one Randomized Control Trial and one open‐label study reported positive results of low Fermentable Oligosaccharides Disaccharides Monosaccha‐rides and Polyols diet; one Randomized Control Trial showed exacerbation of symptoms with fructans in children with Irritable Bowel Syndrome; no effect was found for the lactose‐free diet whilst fructose‐restricted diets were effective in 5/6 studies. / Conclusions: In children there are few trials evaluating low Fermentable Oligosaccharides Disaccharides Monosaccharides and Polyols in functional abdominal pain disorders, with encour‐aging data on the therapeutic efficacy particularly of fructose‐restricted diet. Additional effort sare still needed to fill this research gap and clarify the most efficient way for tailoring dietary restrictions based on the patient's tolerance and/or identification of potential biomarkers of low Fermentable Oligosaccharides Disaccharides Monosaccharides and Polyols efficacy, to maintain nutritional adequacy and to simplify the adherence to diet by labeling FermentableOligosaccharides Disaccharides Monosaccharides and Polyols content in commercial products
