    TITAN code development for application to a PWR steam line break accident : final report 1983-1984

    Modification of the TITAN computer code which enables it to be applied to a PWR steam line break accident has been accomplished. The code now has the capability of simulating an asymmetric inlet coolant temperature transient by employing different temperature transient forcing functions for different core inlet regions. Up to ten regions of the core can be considered and each region can have at most 50 channels. A total inlet coolant mass flow rate boundary condition option has been added to the code. Flow/coolant temperature transient and control rod transient can be simulated simultaneously by the code as necessary for a steam line break accident simulation. Also, the transient restart capability has been fixed which allows users to change core conditions during a transient calculation for various purposes. All these modifications have been tested by a ten-channel test calculation.Three steam line break accident simulations (YA-1, YA-2, and YA-3) with different pressure forcing functions have been performed. Each simulation included both closed and open-channel calculations. The steady-state results show that a 1-D thermalhydraulic analysis gives accurate results.Case YA-1 employed a pressure forcing function taken from a Yankee Atomic report. No boiling during the whole calculation was observed. Also, no significant difference between closed and open-channel calculations was found.Case YA-2 employed a reduced pressure forcing function with constant pressure after 45 seconds (because of the limitation of W-3 correlation data base). Boiling was observed around 42 seconds after the beginning of the transient. The MCHFR dropped to a value below 6 after boiling. The MCHFR went back to a high value ("30) at 50 seconds for the open-channel calculation while the MCHFR for the closed-channel case still remained below 6. The open-channel model provided a better condition of flow mixing among channels.Case YW-3 had the same pressure forcing function as that of case YA-2 except the pressure kept decreasing after 45 seconds. The MCHFR was about equal for open-and closed-channels. It is concluded that the closed-channel calculations may produce conservative core power values, but the effect on MCHFR is not always conservative

    The effectiveness of private tutoring: students’ perceptions in comparison with mainstream schooling in Hong Kong.

    This paper examines Hong Kong students’ perceptions on the effectiveness of private supplementary tutoring relative to mainstream schooling. Drawing on survey and interview data, it shows that large proportions of secondary school students receive private tutoring. Students generally perceive private tutoring and private tutors to be more effective in the provision of examination support compared with mainstream schooling and teachers. However, perceptions vary according to students’ selfreported academic levels and motives for taking private tutoring. The operations of the parallel sector of private tutoring have significant implications for the nature of schooling and therefore need to be considered by teachers and school administrators. The Hong Kong data contribute to the international analysis of private tutoring and add a significant component to the wider conceptual literature.postprin

    Acute Cerebral Infarction Masked by a Brain Tumor

    We report on an 81-year-old man who presented with left limbs weakness and was brought to the emergency room where a brain computed tomography revealed a tumor at the right parasellar region. The patient was admitted to the neurosurgery department, and the symptoms were thought to be due to the tumor mass effect. The final diagnosis turned out to be acute ischemic infarction with an incidentally found brain tumor following angiography and magnetic resonance imaging

    Active and Passive Radiative Transfer Simulations for GPM-Related Field Campaigns

    Using a three-dimensional radiative transfer model combined with cloud-resolving model output, we simulate active and passive sensor observations of clouds and precipitaiton. This combination of tools allows us to diagnose the contributions of various hydrometeor types. Radar multiple scattering is most closely associated with the presence of graupel. At Wband, massive amounts multiple scattering in deep convection can decorrelate the reflectivity profile from the vertical structure, but for less intense events, multiple scattering could be a useful indicator of riming. For passive sensors, polarization differences at 166 GHz indicate the presence of horizontally aligned frozen particles with pronounced aspect ratios, while high concentrations of more isotropic aggregates and graupel dampen the polarization difference while also contributing to the lowest brightness temperature depressions. The insights into remote sensing measurements will facilitate the development of improved algorithms and advanced sensors

    Validating a novel score based on interaction between ACLF grade and MELD score to predict waitlist mortality

    Background and Aim: Among candidates listed for liver transplant (LT), MELD score may not capture acute on chronic liver failure (ACLF) severity. Data on interaction between ACLF and MELD score in predicting waitlist (WL) mortality are scanty. / Methods: UNOS database (01/2002 to 06/2018) on LT listings for adults with cirrhosis and ACLF (without HCC) was analyzed. ACLF grades 1, 2, 3a, and 3b- were defined using modified EASL-CLIF criteria. / Results: Of 18,416 candidates with ACLF at listing (mean age 54 years, 69% males, 63% Caucasians), 90-d WL mortality (patient death or being too sick for LT) was 21.6% (18%, 20%, 25%, and 39% for ACLF grades 1, 2, 3a, and 3b respectively). Fine and Gray regression model identified interaction between MELD and ACLF grade, with higher impact of ACLF at lower MELD score. Other variables included candidate’s age, gender, liver disease etiology, listing MELD, ACLF grade, obesity, and performance status. A score developed using parameter estimates from the interaction model on the derivation cohort (N=9181) stratified the validation cohort (N=9235) to four quartiles Q1 (score 15.50). WL mortality increased with each quartile from 13%, 18%, 23%, and 36% respectively. Observed versus expected deciles on WL mortality in validation cohort showed good calibration (goodness of fit P=0.98) and correlation (R=0.99). / Conclusion: Among selected candidates who are in ACLF at listing, MELD score and ACLF interact in predicting cumulative risk of 90-d WL mortality, with higher impact of ACLF grade at lower listing MELD score. Validating these findings in large prospective studies will support to factor in both MELD and ACLF in prioritizing transplant candidates and allocation of liver grafts

    Influencing functional outcomes: a look at role performance and satisfaction with life following liver transplant

    Abstract 572 The success of orthotopic liver transplantation (OLT), originally measured as survival, now extends to quality of the life saved. Return to work (RTW) is also a desired outcome. Our AIM was to explore the relationship between 5 pre-OLT factors & 5 post-OLT quality of life (QOL) domains with life satisfaction and primary productive role to better understand how to improve both. METHODS: Patients (pts)1-3 yrs post-OLT filled QOL form during follow-up clinic visits between 7/04 to 6/05. The Liver transplantation Database-Quality of life (LTD-QOL) form yielded data on 5 domains: measure of disease (MOD), psychological distress/well-being (PDW), personal function (PF), social/role function (SRF) & general health perception (GHP). Results: 229 pts were first categorized as satisfied overall with life (79%), or dissatisfied, and then assigned to groups based on primary productive role (51%), no primary productive role, or retired. Pre-OLT variables were age, gender, marital status, education, & etiology of liver disease; HCV (33%), alcohol liver disease (ALD)(11%), HCV+ALD (10%), & others (46%). Marital status & age were not significantly related to the outcome variables. Etiology of liver disease, education, and time since OLT and 5 post-OLT QOL domains were significantly associated with both outcome variables; satisfaction and primary productive role (p<.0001).To understand the differences, the 5 physical & men-tal QOL domains were regressed on primary productive role and satisfaction. Pts (mean age 54 yrs (19-74 yrs), males, 70%) fell into the category of primary productive role rates (51%). Pts transplanted for ALD were significantly (p<.05) more likely to be satisfied with life, whereas individuals with HCV±ALD, had lowest satisfaction and were most likely to be unable/uninterested in work. Stepwise logistical regression analysis of satisfaction demonstrated that GHP and SRF correlated most highly. Although satisfaction was significant in bivariate analysis, regression analysis of the influence of domains of QOL, as well as employment, demonstrated that SRF & GHP correlated most highly with life satisfaction. CONCLUSIONS: SRF and GHP correlate with good QOL post OLT. HCV patients have low levels of satisfaction whereas the highest level of satisfaction is in the ALD group. Further studies should address methods to improve satisfaction in those with HCV

    Efficacy of Sofosbuvir, Velpatasvir, and GS-9857 in Patients With Hepatitis C Virus Genotype 2, 3, 4, or 6 Infections in an Open-Label, Phase 2 Trial

    © 2016 AGA Institute Background & Aims Studies are needed to determine the optimal regimen for patients with chronic hepatitis C virus (HCV) genotype 2, 3, 4, or 6 infections whose prior course of antiviral therapy has failed, and the feasibility of shortening treatment duration. We performed a phase 2 study to determine the efficacy and safety of the combination of the nucleotide polymerase inhibitor sofosbuvir, the NS5A inhibitor velpatasvir, and the NS3/4A protease inhibitor GS-9857 in these patients. Methods We performed a multicenter, open-label trial at 32 sites in the United States and 2 sites in New Zealand from March 3, 2015 to April 27, 2015. Our study included 128 treatment-naïve and treatment-experienced patients (1 with HCV genotype 1b; 33 with HCV genotype 2; 74 with HCV genotype 3; 17 with genotype HCV 4; and 3 with HCV genotype 6), with or without compensated cirrhosis. All patients received sofosbuvir-velpatasvir (400 mg/100 mg fixed-dose combination tablet) and GS-9857 (100 mg) once daily for 6–12 weeks. The primary end point was sustained virologic response 12 weeks after treatment (SVR12). Results After 6 weeks of treatment, SVR12s were achieved by 88% of treatment-naïve patients without cirrhosis (29 of 33; 95% confidence interval, 72%–97%). After 8 weeks of treatment, SVR12s were achieved by 93% of treatment-naïve patients with cirrhosis (28 of 30; 95% CI, 78%–99%). After 12 weeks of treatment, SVR12s were achieved by all treatment-experienced patients without cirrhosis (36 of 36; 95% CI, 90%–100%) and 97% of treatment-experienced patients with cirrhosis (28 of 29; 95% CI, 82%–100%). The most common adverse events were headache, diarrhea, fatigue, and nausea. Three patients (1%) discontinued treatment due to adverse events. Conclusions In a phase 2 open-label trial, we found sofosbuvir-velpatasvir plus GS-9857 (8 weeks in treatment-naïve patients or 12 weeks in treatment-experienced patients) to be safe and effective for patients with HCV genotype 2, 3, 4, or 6 infections, with or without compensated cirrhosis.     Sensor forward models are an important tool for interpreting remote sensing observations of geophysical phenomena. By implementing a three-dimensional framework, we can simulate and analyze observations from various sensors on disparate platforms. To demonstrate our model framework, we simulate observations from the Olympic Mountains Experiment (OLYMPEX).
