    Design and Analysis of Plastic Fuel Tank for Three Wheeler CNG Passenger Vehicle

    The current trend is to provide weight/strength effective and reliable products which satisfy the stringent requirements. The fuel tank is used in automobile industries to carry fuel and supply fuel to the engine via fuel filter, fuel pump. The aim of this project is to design plastic fuel tank for three wheeler vehicles. This study aimed at to make a new design of plastic fuel tank for three wheeler vehicles in order to reduce production cost of fuel tank and make most secure and safe fuel tank. The project involves geometry and finite element modeling of plastic fuel tank design. Study the existing fuel tank and then begins the attempt to make plastic fuel tank. Geometrical modeling was done using Pro-E; finite element modeling using Hyper mesh software and analysis was done using ANSYS software. Experimental test was conducted to examine the effectiveness of the plastic fuel tank. Result of static load analysis of the existing fuel tank and plastic fuel tank design is compared to prove the design is safe. Similarly experimental result of the existing fuel tank bracket and the plastic fuel tank bracket are compared to prove the design is safe.     High Mobility Group Box Protein 1 (HMGB1): The Prototypical Endogenous Danger Molecule

    High mobility group box protein 1 (HMGB1) is an evolutionary ancient nuclear protein that exerts divergent biological tasks inside and outside of cells. The functions of HMGB1 depend on location, binding partners and redox states of the molecule. In the nucleus, HMGB1 organizes DNA and nucleosomes and regulates gene transcription. Upon cell activation or injury, nuclear HMGB1 can translocate to the cytoplasm, where it is involved in inflammasome activation and pyroptosis, as well as regulation of the autophagy/apoptosis balance. When actively secreted or passively released into the extracellular milieu, HMGB1 has cytokine, chemokine, neuroimmune and metabolic activities. Thus, HMGB1 plays multiple roles in the pathogenesis of inflammatory diseases and mediates immune responses that range from inflammation and bacterial killing to tissue repair. HMGB1 has been associated with divergent clinical conditions such as sepsis, rheumatoid arthritis and atherosclerosis. HMGB1 initiates and perpetuates immune responses during infectious and sterile inflammation, as the archetypical alarmin and damage-associated molecular pattern (DAMP) molecule. We here describe advances in the understanding of HMGB1 biology with focus on recent findings of its mission as a DAMP in danger sensing and as a therapeutic target in inflammatory diseases

    Caesarean myomectomy in a patient with multiple maternal and fetal comorbidities: a rare interesting case report

    Caesarean myomectomy has been traditionally discouraged due to the fear of complications like intractable haemorrhage, peripartum hysterectomy, and increased postoperative morbidity. Recently, a number of authors in their studies have shown that myomectomy during caesarean section does not increase the risk of haemorrhage or postoperative morbidity, if adequate haemostatic methods are used and done by skilled surgical hands. Caesarean myomectomy was performed in a case of elderly primigravida with 36-week gestation with chronic hypertension with polyhydramnios with anterior wall pedunculated fibroid of 8.6×7.2×8 cm and multiple small fibroids in the anterior wall, fundus and subserosal plane. Caesarean myomectomy is a safe and effective procedure in a tertiary care centre at hands of an experienced surgeon

    Didelphys uterus: an interesting case report of pregnancy in a rare Mullerian anomaly

    Mullerian duct anomalies (MDAs) are congenital defects of the female genital system that arise from abnormal embryological development of the Mullerian ducts. A didelphys uterus, also known as a double uterus, is one of the least common amongst the MDAs. This report discussed a case of pregnancy with uterus didelphys. This patient was a 27-year-old primigravida with 34.2-week gestation with spontaneous conception who presented with decreased fetal movements. On examination patient had a non-communicating, thick vaginal septum extending from the introitus to the cervix was seen and two cervices one on each side of the septum were located. The patient underwent emergency lower segment caesarean section in view of foetal distress and doppler changes. Intra-operatively, evidence of didelphys uteri was seen. Intraoperative and post-operative period was uneventful. A fetus of 1790 gram was delivered, with APGAR 9/10. There were no renal anomalies on subsequent ultrasonography

    Cholinergic Control of Inflammation, Metabolic Dysfunction, and Cognitive Impairment in Obesity-Associated Disorders: Mechanisms and Novel Therapeutic Opportunities

    Obesity and obesity-associated disorders have become world-wide epidemics, substantially increasing the risk of debilitating morbidity and mortality. A characteristic feature of these disorders, which include the metabolic syndrome (MetS) and type 2 diabetes, is chronic low-grade inflammation stemming from metabolic and immune dysregulation. Inflammation in the CNS (neuroinflammation) and cognitive impairment have also been associated with obesity-driven disorders. The nervous system has a documented role in the regulation of metabolic homeostasis and immune function, and recent studies have indicated the important role of vagus nerve and brain cholinergic signaling in this context. In this review, we outline relevant aspects of this regulation with a specific focus on obesity-associated conditions. We outline accumulating preclinical evidence for the therapeutic efficacy of cholinergic stimulation in alleviating obesity-associated inflammation, neuroinflammation, and metabolic derangements. Recently demonstrated beneficial effects of galantamine, a centrally acting cholinergic drug and cognitive enhancer, in patients with MetS are also summarized. These studies provide a rationale for further therapeutic developments using pharmacological and bioelectronic cholinergic modulation for clinical benefit in obesity-associated disorders

    A pharmacovigilance study in patients of chronic non-infective respiratory diseases attending outpatient department of pulmonary medicine in a tertiary care teaching hospital

    Background: Adverse drug reactions (ADR) are the known dangers of any medicinal therapy. They are not only responsible for increasing the mortality and morbidity but also for multiplying the health care expenditure. It is important to monitor the adverse effects of the drugs in the patients on treatment for chronic non-infective respiratory diseases attending OPD of pulmonary medicine in a tertiary care teaching hospitalMethods: The study was single-centric, non-randomized and observational hospital-based study which was carried out for a period of 1 and a half years in JJ Hospital. The patients who were included in the study suffered from either of the 4 diseases-Chronic obstructive pulmonary disease (COPD), asthma, bronchiectasis or interstitial lung diseases (ILD). Data were analyzed by using Microsoft excel sheet. Based on the outcome of modified Hartwig and Siegel severity assessment scale, ADRs were grouped into various severity categories.Results: One hundred and thirty-two number of ADRs were seen in 69 out of 352 patients (19.6 %) of the study population. The occurrence of ADR was found slightly higher in males i.e., 53.62% as compared to females i.e., 46.38%. The patients who were on treatment for ILD showed highest percentage of ADRs i.e., 57.89% which is followed by bronchiectasis (17.39%), COPD (16.17%) and lastly asthma (10.26%). The ADRs belonging to GIT system were highest in number i.e., 80. The most frequently occurring ADR in the study was palpitation which occurred in 14 cases i.e., 20.29%. Out of 132 ADRs observed, 96 i.e., 72.73% belonged to the mild category and 36 ADRs i.e., 27.27% belonged to the moderate category. Not a single severe ADR was found in the study.Conclusions: It was found that 19.6% of the patient population suffered from ADRs, which is a considerable number. It is essential that health care professionals should support ADR monitoring process for the safety of the medicinal product. Proper implementation of ADR monitoring will help to reduce the harmful effects by early detection of drug safety problems in patients, assessing the risk-benefit in an individual and the population, improving the selection, rational use of drugs through the provision of timely warning to healthcare professionals
