    Geospatial transmission hotspots of recent HIV infection — Malawi, October 2019–March 2020

A novel HIV infection surveillance initiative was implemented in Malawi to collect data on recent HIV infections among new diagnoses to characterize the epidemic and guide the public health response. What is added by this report? Higher proportions of recent infections were identified among females, persons aged <30 years, and clients at maternal and child health and youth clinics. Spatial analysis identified three hotspots of health facilities with significantly higher rates of recent infection than expected across five districts. What are the implications for public health practice? Geospatial analysis of recent HIV infection surveillance data can identify potential transmission hotspots. This information could be used to tailor program activities to strengthen HIV testing, prevention, and treatment services and ultimately interrupt transmission

    Hospitalized patients’ pain experience before and after the introduction of a nurse-based pain management programme: a separate sample pre and post study

    Background Many patients suffer from unrelieved pain in hospital settings. Nurses have a pivotal role in pain management. Hence, a nurse-based pain management programme may influence how hospitalized patients experience pain. In this study we investigated hospitalized patients’ experience of pain before and after the introduction of a two-component nurse-based pain management programme. Methods A quasi-experimental design with a separate sample pretest-posttest approach was conducted on a convenience sample of 845 patients (Survey 1: N = 282; Survey 2: N = 283; Survey 3: N = 280) admitted to the four inpatient units (medical, surgical, maternity, and gynecology) of a university medical center. Data were collected at baseline, before the intervention six weeks after pain management education, and finally immediately after four months of rounding using an interviewer-administered questionnaire adopted from a Brief Pain Inventory and the American Pain Society Patient Outcome Questionnaire. Results All the samples had similar sociocultural backgrounds. The proportion of patients who reported average moderate and severe pain intensity in the last 24 h were 68.8% in Survey 1, 72.8% in Survey 2 and then dropped to 48.53% in Survey 3 whereas those who reported moderate and severe pain intensity at the time of interview were 53.9% in Survey 1, 57.1% in Survey 2 and then dropped to 37.1% in Survey 3. The mean pain interference with the physical and emotional function was generally reduced across the surveys after the introduction of the nurse-based pain management programme. These reductions were statistically significant with p < 0.05. Conclusions Though the survey findings must be taken with caution, they demonstrate that the nurse-based pain management programme positively influenced patient-reported pain intensity and functional interference at the university medical center. This shows the potential clinical importance of the programme for hospitalized patients

    Tef or teff Eragrostis tef (Zucc.) Trotter, a cereal crop which adapts to extreme climatic and soil conditions, is extensively cultivated in the Horn of Africa. It is also considered as nutritious and a life-style crop due to its richness in essential nutrients and health-related benefits. However, the productivity of the crop is extremely low due to little scientific improvement made globally. It is, therefore, in the category of orphan crops. Together with all cereal crops, tef belongs to the Grass or Poaceae family. The improvement of tef focuses on selection and hybridization techniques. However, recently, molecular and high-throughput techniques have also been implemented to a limited scale. Forty-two tef varieties were approved for release by the Ethiopian National Variety Release Committee in the past four decades. Due to the adoption of improved varieties and technologies, the national average yield of tef has more than doubled over the last 20Ă‚ years. This review describes the progress in tef breeding and variety development as well as dissemination of the improved varieties to the farming community
