    Development and validation of a rapid lateral flow E1/E2-antigen test and ELISA in patients infected with emerging Asian strain of Chikungunya virus in the Americas

    Since its 2013 emergence in the Americas, Chikungunya virus (CHIKV) has posed a serious threat to public health. Early and accurate diagnosis of the disease, though currently lacking in clinics, is integral to enable timely care and epidemiological response. We developed a dual detection system: a CHIKV antigen E1/E2-based enzyme-linked immunosorbent assay (ELISA) and a lateral flow test using high-affinity anti-CHIKV antibodies. The ELISA was validated with 100 PCR-tested acute Chikungunya fever samples from Honduras. The assay had an overall sensitivity and specificity of 51% and 96.67%, respectively, with accuracy reaching 95.45% sensitivity and 92.03% specificity at a cycle threshold (Ct) cutoff of 22. As the Ct value decreased from 35 to 22, the ELISA sensitivity increased. We then developed and validated two lateral flow tests using independent antibody pairs. The sensitivity and specificity reached 100% for both lateral flow tests using 39 samples from Colombia and Honduras at Ct cutoffs of 20 and 27, respectively. For both lateral flow tests, sensitivity decreased as the Ct increased after 27. Because CHIKV E1/E2 are exposed in the virion surfaces in serum during the acute infection phase, these sensitive and specific assays demonstrate opportunities for early detection of this emerging human pathogen

    Emergency obstetric hysterectomy: review at a tertiary care hospital

    Background: The objective of the study was to study indications and maternal outcome of emergency obstetric hysterectomy.Methods: A retrospective study of the cases of emergency obstetric hysterectomy performed over a period of 7.5years from 2008 to May 2016 in a tertiary care hospital was done. Maternal characteristics, indications, and maternal morbidity and mortality were analyzed.Results: During the study period there were 45 emergency obstetric hysterectomies and 28,207 deliveries, giving an incidence of 0.16% or 1.6/1000. Majority of the cases were unbooked (55.6%). It was more common in multipara (71.1%). Ruptured uterus (37.7%) and morbidly adherent placenta (26.6%) were the common indications. the maternal mortality was 2.22%.Conclusions: Emergency obstetric hysterectomy is a lifesaving procedure. The maternal outcome greatly depends on timely decision and good clinical judgment because unnecessary delay can cost life and undue haste can cause morbidity.

Like, osteoarthritis, shortness of breath, congestive heart failure, all can lead to difficulty with sleep initiation and maintenance. Trouble with sleep is strongly correlated with complaints about health and depression. Studies examining the prevalence of sleep disturbances in patients with chronic medical diseases have reported that 31% of arthritis and 66% of chronic pain patients report difficulty falling asleep, while 81% of arthritis, 85% of chronic pain, and 33% of diabetes patients report difficulty staying asleep. In a recent National Sleep Foundation survey of adults aged 65 years and over, those with more medical conditions, including cardiac and pulmonary disease and depression, reported significantly more sleep complaints. Objective: This study has been conducted to evaluate the sleeping and waking time of elderly and Diseases pattern with sleep status. Methodology: Interview based Questionnaire was prepared to know the sleep timings, waking up timings of elderly. 96 subjects of both sex was selected from OPD of AIIA, and nearby place and collected data was analyzed by using odd’s. Conclusion: According to the study the health status is directly affected by the timings of sleeping and waking up i.e. people who wake up early tends to be healthier. So, to add the quality in the end years of life, sleep will be main domain to work upon

    In popular orthogonal frequency division multiplexing (OFDM) communication system processing is one of the key procedures Fast Fourier transform (FFT) and inversely for that Fast Fourier Transform (IFFT) is one of them. In this VLSI implementation Structured pipeline architectures, low power consumption, high speed and reduced chip area are the important concerns. In this paper, presentation of the worthy implementation of FFT/IFFT processor for OFDM applications is described. We obtain the single-path delay feedback architecture, to get a ROM of smaller size and this proposed architecture applies a reconfigurable complex multiplier. To minimize the error of truncation we apply a fixed width modified booth multiplier. As a result, the proposed radix-2k feed forward architectures even offer an attractive solution for current applications, and also open up a new research line on feed forward structures
