    Underlying Primary Causes of Chronic Renal Failure: A Three-Year Study in Al-Thawra General Hospital, Sana'a, Yemen

    Objective: To estimate the underlying primary diseases of chronic renal failure (CRF) among patients attending Al-Thawra General Hospital, Sana'a, and Yemen.Methods: This was a cross-sectional, hospital-based study of 566 patients diagnosed with chronic kidney diseases (CKD). It studied the cases attending Al-Thawra General Hospital over a threeā€year period from January 2013 to December 2015.Results: Out of 566 patients, 339 (59.9%) were males. The mean age of the patients at diagnosis of CKD was 39.51 years, most of whom (36.6%) were of the age group of 21ā€“35 years. Hypertension was the most frequent cause (43.2%) of CRF, followed by different infectious diseases such as malaria, schistosomiasis and bacterial infections (19%) and obstructive nephropathy (17.9%). On the other hand, CRF of unknown etiology represented 15.2% of cases. Other causes of CRF diabetic nephropathy (9.7%), effect of antibiotics and analgesics (7.0%), ischemic heart disease (4.8%, polycystic kidney disease (3.5%) and congenital anomalies (3.4%). The least common causes were autoimmune diseases, bleeding, traumatic accidents and chronic diarrhea, being responsible for 2.2%, 1.8%, 0.9% and 0.4% of CRF cases, respectively.Conclusions: The majority of CKD cases in our study were males aged between 21 and 35 years old. In addition, most cases of CKD are due to hypertension, followed by infections and obstructive nephropathy. Application of future prevention and control measures are highly recommended to reduce the burden of CRF in Yemen for early detection and proper management of its underlying primary conditions/diseases

Researchers aspire to create a unique and different design studio environment that motivates studentsā€™ creativity. This study presents the biophilia theory as a technique that generates a creative design studio environment. The purpose is to determine the applicability of the biophilic design theory patterns to the design studio environment, which in turn has an impact on motivating the students' creativity. The research method will be conducted through a comprehensive analysis of several world- famous architectural schools, by understanding the application of the principles of nature and connecting them with biophilic patterns. As a result, the research has yielded applicable criteria for biophilic patterns in existing design studios

    Challenges Facing Distance Education for Children in Virtual Kindergartens

    For kindergarten children, distance education was neither an alternative nor a potential option to in-person schooling before the global COVID-19 pandemic-imposed isolation and mandatory social distancing measures. These measures guaranteed the safety of school staff and children and made the adoption of distance education an urgent necessity for children to continue their education safely. The current study was conducted to reveal the most common challenges that faced childrenā€™s distance education in virtual kindergartens. The study was conducted from the teachersā€™ point of view to determine the impact of each of the job title variables and years of experience. The study sample consisted of 32 kindergarten teachers in Jubail Governorate. The research team applied the scale of challenges facing childrenā€™s education in virtual kindergarten. The results of the study concluded that the most important challenges facing childrenā€™s education in virtual kindergartens were related to communication skills (79.9%), technical skills (79.5%), educational aids (76.4%), and motivation (69.6%). The results of the study also revealed that there were statistically significant differences in the challenges related to motivation and readiness and in the total challenges facing childrenā€™s education in the virtual kindergarten from the teachersā€™ point of view depending on the job titleā€”that is, the assistant teacher where the calculated ā€œUā€ value reached 28.5 and 37.5. In addition, there was an absence of statistically significant differences due to the variable years of experience, whether in the total number of challenges or in the dimensions of the scale, except for the dimension of communication skills, where the calculated ā€œKa2ā€ value reached 8.755, which is a statistically significant value at the level of significance (0.05)

    Allelic Discrimination of Vitamin D Receptor Polymorphisms and Risk of Type 2 Diabetes Mellitus: A Case-Controlled Study

    (1) Background: Type 2 diabetes mellitus (T2DM) is one of the rapidly growing healthcare problems, and several vitamin D receptor (VDR) polymorphisms seem to modulate the risk of T2DM. Our research was designed to investigate the allelic discrimination of VDR polymorphisms and T2DM occurrence risk. (2) Methods: This case-control research included 156 patients with T2DM and 145 healthy control subjects. Most of the study population were males 56.6% vs. 62.8% in the case and control groups, respectively. Genotyping for VDR single nucleotide polymorphisms (SNPs), rs228570 (Fok1), rs7975232 (Apa1), and rs1544410 (Bsm1) was compared between both groups. (3) Results: There was a negative link between vitamin D levels and insulin sensitivity. A significant difference was noted in the allelic discrimination of VDR polymorphism rs228570 and rs1544410 between the study groups (p \u3c 0.001). No difference was observed in the allelic discrimination of VDR polymorphism rs7975232 between the groups (p = 0.063). Moreover, T2DM patients had significantly higher levels of fasting blood sugar (FBS), glycated hemoglobin HbA1c, 2-h post-prandial blood sugar (PP), serum glutamic oxaloacetic transaminase (SGOT), serum glutamic-pyruvic transaminase (SGPT), total cholesterol, and triglycerides (p \u3c 0.001), while High-Density Lipoprotein (HDL) Cholesterol (HDL-C) was significantly decreased (p = 0.006). (4) Conclusions: VDR polymorphisms had a positive association with T2DM risk among the Egyptian population. Further large-scale research using deep sequencing of samples is strongly urged to investigate different vitamin D gene variants and interactions, as well as the influence of vitamin D on T2DM

    Utilisation of an operative difficulty grading scale for laparoscopic cholecystectomy

    Background A reliable system for grading operative difficulty of laparoscopic cholecystectomy would standardise description of findings and reporting of outcomes. The aim of this study was to validate a difficulty grading system (Nassar scale), testing its applicability and consistency in two large prospective datasets. Methods Patient and disease-related variables and 30-day outcomes were identified in two prospective cholecystectomy databases: the multi-centre prospective cohort of 8820 patients from the recent CholeS Study and the single-surgeon series containing 4089 patients. Operative data and patient outcomes were correlated with Nassar operative difficultly scale, using Kendallā€™s tau for dichotomous variables, or Jonckheereā€“Terpstra tests for continuous variables. A ROC curve analysis was performed, to quantify the predictive accuracy of the scale for each outcome, with continuous outcomes dichotomised, prior to analysis. Results A higher operative difficulty grade was consistently associated with worse outcomes for the patients in both the reference and CholeS cohorts. The median length of stay increased from 0 to 4 days, and the 30-day complication rate from 7.6 to 24.4% as the difficulty grade increased from 1 to 4/5 (both pā€‰<ā€‰0.001). In the CholeS cohort, a higher difficulty grade was found to be most strongly associated with conversion to open and 30-day mortality (AUROCā€‰=ā€‰0.903, 0.822, respectively). On multivariable analysis, the Nassar operative difficultly scale was found to be a significant independent predictor of operative duration, conversion to open surgery, 30-day complications and 30-day reintervention (all pā€‰<ā€‰0.001). Conclusion We have shown that an operative difficulty scale can standardise the description of operative findings by multiple grades of surgeons to facilitate audit, training assessment and research. It provides a tool for reporting operative findings, disease severity and technical difficulty and can be utilised in future research to reliably compare outcomes according to case mix and intra-operative difficulty

    Wogonin and related natural flavones are inhibitors of CDK9 that induce apoptosis in cancer cells by transcriptional suppression of Mcl-1

    The wogonin-containing herb Scutellaria baicalensis has successfully been used for curing various diseases in traditional Chinese medicine. Wogonin has been shown to induce apoptosis in different cancer cells and to suppress growth of human cancer xenografts in vivo. However, its direct targets remain unknown. In this study, we demonstrate for the first time that wogonin and structurally related natural flavones, for example, apigenin, chrysin and luteolin, are inhibitors of cyclin-dependent kinase 9 (CDK9) and block phosphorylation of the carboxy-terminal domain of RNA polymerase II at Ser2. This effect leads to reduced RNA synthesis and subsequently rapid downregulation of the short-lived anti-apoptotic protein myeloid cell leukemia 1 (Mcl-1) resulting in apoptosis induction in cancer cells. We show that genetic inhibition of Mcl-1 or CDK9 expression by siRNA is sufficient to mimic flavone-induced apoptosis. Pull-down and in silico docking studies demonstrate that wogonin directly binds to CDK9, presumably to the ATP-binding pocket. In contrast, wogonin does not inhibit CDK2, CDK4 and CDK6 at doses that inhibit CDK9 activity. Furthermore, we show that wogonin preferentially inhibits CDK9 in malignant compared with normal lymphocytes. Thus, our study reveals a new mechanism of anti-cancer action of natural flavones and supports CDK9 as a therapeutic target in oncology

    Metabolic profiling of HepG2 cells incubated with S(āˆ’) and R(+) enantiomers of anti-coagulating drug warfarin

    Warfarin is a commonly prescribed oral anticoagulant with narrow therapeutic index. It achieves anti-coagulating effects by interfering with the vitamin K cycle. Warfarin has two enantiomers, S(āˆ’) and R(+) and undergoes stereoselective metabolism, with the S(āˆ’) enantiomer being more effective. We reported the intracellular metabolic profile in HepG2 cells incubated with S(āˆ’) and R(+) warfarin by GCMS. Chemometric method PCA was applied to analyze the individual samples. A total of 80 metabolites which belong to different categories were identified. Two batches of experiments (with and without the presence of vitamin K) were designed. In samples incubated with S(āˆ’) and R(+) warfarin, glucuronic acid showed significantly decreased in cells incubated with R(+) warfarin but not in those incubated with S(āˆ’) warfarin. It may partially explain the lower bio-activity of R(+) warfarin. And arachidonic acid showed increased in cells incubated with S(āˆ’) warfarin but not in those incubated with R(+) warfarin. In addition, a number of small molecules involved in Ī³-glutamyl cycle displayed ratio variations. Intracellular glutathione detection further validated the results. Taken together, our findings provided molecular evidence on a comprehensive metabolic profile on warfarin-cell interaction which may shed new lights on future improvement of warfarin therapy

    The Interaction of Social, Physical and Nutritive Factors in Triggering Early Developmental Language Delay in a Sample of Egyptian Children

    BACKGROUND: Language acquisition and child development during the early years of life depend on multiple interacting factors. AIM: To explore potential factors that can impact language development in 2 groups of Egyptian children, one with normal language development and the second with delayed development. Also, to explore to what extent can the involvement of impaired motor development potentiate the risk of developmental language delay. METHODS: This cross-sectional case-control study involved Egyptian children belonging to the middle socioeconomic class between 18 and 36 months of age. Children were classified according to their performance on language domain of Bayley Scales of Infant and Toddler Development (Bayley-III) into two groups, infants with the average or above score (control group) and those having below-average scores (cases). Motor development was assessed on the same scale. Factors affecting language development were tested, including socio-demographic, obstetric, and maternal medical factors in addition to Infant Feeding Practices. RESULTS: The independent factors lowering the language scores were early introduction of complementary food, low family income, history of delivery problems, pregnancy-related diseases of the mother, and maternal education. Impaired motor development appears as a further highly significant risk factor to the previously mentioned factors CONCLUSION: In Egyptian children, delayed language development is severely affected by the interaction of medical, social and nutritional factors. Providing adequate maternal health care during pregnancy and childbirth, regular developmental monitoring at each child visit, and screening for such risk factors, can reduce size of the problem and promote childĆ¢ā‚¬ā„¢s social and psychological development
