    Tertiary-Tree 12-GHz 32-bit Adder in 65nm Technology

    This paper presents a new 32-bit adder structure with 12 GHz low-power operation in 65nm technology. The Fast Conditional Sparse-Tree Logic (FCSL) is based on modifying the initial Sparse-Tree architecture [1] to enhance its speed using tertiary trees and applying a carry-select scheme in some of the more significant bits. This design has been compared with the Sparse-Tree adder and the Low-Voltage Swing adder in terms of speed and power.     Buton Asphalt is a natural asphalt which has advantages because it contains minerals and asphalt, even though its asphalt content does not completely meet the specification as a binder of asphalt concrete. Various studies of the Buton Asphalt USAge have been carried out. This paper presented the results of a study on asphalt concrete mixtures using materials of Buton Granular Aggregate (BGA), treating the BGA as aggregates, with the BGA content of 5% and 7%. Mixtures and specimens were analyzed to investigate the effects of BGA on the performance of asphalt concrete. The results showed that the indirect tensile strength for the mixture with BGA of 5% is higher than that obtained in the those with BGA content of 7%

    Comparison of the effects of eicosapentaenoic acid with docosahexaenoic acid on the level of serum lipoproteins in helicobacter pylori: A randomized clinical trial

    Background: Helicobacter pylori infection is the most common chronic bacterial infection around the world and an important cause of gastrointestinal disorders, which might be involved in the pathogenesis of some extragastrointestinal disturbances as well as changes in serum lipid profile. Hypolipemic properties of omega-3 fatty acids have been studied in several studies. Objectives: The present study aimed to compare the effects of eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) supplementation on the level of serum lipoproteins in H. pylori. Patients and Methods: In a randomized, double-blinded, placebo-controlled clinical trial in Iran, 105 Helicobacter pylori were randomly allocated to receive 2 g of daily EPA (35 patients), DHA (35 patients), or medium-chain triglyceride (MCT) oil as placebo (33 patients) along with conventional tetra-drug H. pylori eradication regimen for 12 weeks. Results: From 105 included patients, 97 (31 in EPA, 33 in DHA, and 33 in control groups) completed the study and were included in final analysis. The levels of total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C) and the ratios of TG/HDL-C, TC/HDL-C, and LDL-C/HDL-C were not significantly different among the three groups, while the level of triglyceride (TG) was statistically different. DHA (-16.6 ± 30.34) and control (+ 15.32 ± 56.47) groups were statistically different with regard to changes in TG levels (P = 0.000). Conclusions: There was no difference between the effects of 2 g of EPA or DHA supplementation for 12 weeks on the levels of total cholesterol, LDL-C, HDL-C, TC/HDL-C, TG/HDL-C, and LDL-C/HDL-C; however, it had a desirable effect on the level of TG in a way that the effect of DHA was clearer. © 2015, Iranian Red Crescent Medical Journal

    Quality of cargo management services is primary consumer expectation indicator measured by the dimension model and the minimum service time. Likert scale of five levels is use, analyzed with Important Performance Analysis and Performance Analysis Approach to portray the user perception and expectation. Five dimensions of quality aspect are selected such as reliability, responsiveness, assurance, empathy, and physical evidence. Two air cargo companies operating at Jakarta International Airport Soekarno Hatta are selected as respondents with sample sizes of 46 and 33 are observed. A grid with four quadrants is developed. The result shows that the two operator need improvement to reach minimum service time of 30 minutes. The existing system is in a range between 46 to 51.51 minutes to finish all system services steps. To reach the minimum service time, the operator management, equipment, and tools must be improved

    Efficacy of Probiotic Escherichia coli Nissle 1917 in Patients with Irritable Bowel Syndrome: a Double Blind Placebo-controlled Randomized Trial

    AIM: to evaluate potential improvement effect for probiotic E. coliNissle 1917 in the management of refractory IBS in an Iranian population. METHODS: a double blind placebo controlled approach has been used in the current clinical trial. 139 confirmed IBS patients were included into the study, and were given probiotic E.coli Nissle 1917 for 6 weeks. 11 items Birmingham IBS Symptom Questionnairehas been used for evaluation of changes in the symptoms every 2 weeks. CONCLUSION: probiotic therapy with E.coli Nissle 1917 was not able to induce significant improvement in the symptoms of patients with non-categorized IBS. Nevertheless, when IBS patients were recategorized to subgroups according to their main symptoms, evaluation of the efficacy of the probiotic on some individual items in the symptom list reached the significance level. Prospective clinical trials are recommended to confirm our findings. RESULTS: sixty eight subjects (49) were males. Mean±SD age of the participants was 38±13.3 years. 49(35.3) of the patients were diarrhea-predominant. The total scores showed no significant difference between the intervention vs. control group(-6.7±6.8 vs. -6.7±6.5, respectively; p=0.95); neither did any of the questionnaire items any significant alterations in the two groups. After stratification of patients based on their IBS type, diarrhea-predominant patients showed a positive response to the probiotic improving their sleep (p=0.05 and 0.03 at weeks 2 and 6, respectively). Patients with constipation-predominant IBS showed no response to the probiotic; while patients with diarrhea-constipation mixed IBS showed unfavorable response to the probiotic in the need for strain to pass a motion compared to the placebo (p=0.03 and 0.02 at weeks 4 and 6, respectively)

    Helicobacter pylori infection, the most common chronic bacterial infection in the world, and an important cause of gastrointestinal disorders, may be involved in the pathogenesis of some extra-gastrointestinal disturbances, as well as an increase in blood levels of certain inflammatory markers. Anti-bacterial activity against Helicobacter pylori and anti-inflammatory properties of omega-3 fatty acids have been studied in several research studies. The purpose of the present study was the comparison of the effects of Eicosapentaenoic Acid and Docosahexaenoic Acid supplementation on Helicobacter pylori eradication, serum levels of some inflammatory markers and total antioxidant capacity. In a randomized, double-blind, placebo-controlled clinical trial, 97 Helicobacter pylori positive patients (64 patients in the two intervention groups and 33 in the control group), received 2 grams daily of Eicosapentaenoic Acid, Docosahexaenoic Acid or Medium Chain Triglyceride oil as placebo, along with conventional tetra-drug Helicobacter pylori eradication regimen, for 12 weeks. Helicobacter pylori eradication test and measurement of concentration of interleukine-6, interleukine-8, high-sensitivity C-reactive protein and total antioxidant capacity were performed after the intervention. There was no significant difference in eradication rate of the infection, levels of interleukine-6 and total antioxidant capacity among the three groups, while the levels of interleukine-8 and high-sensitivity C-reactive protein were statistically different. Eicosapentaenoic Acid or Docosahexaenoic Acid supplementation had no significant differential impact on the eradication of Helicobacter pylori infection, and serum levels of interleukine-6 and total antioxidant capacity. However, it had a desirable effect on the levels of interleukine-8 and high-sensitivity C-reactive protein in Helicobacter pylori positive patients. © 2015 by School of Pharmacy Shaheed Beheshti University of Medical Sciences and Health Services

    Female gender and Helicobacter pylori infection, the most important predisposition factors in a cohort of gastric cancer: A longitudinal study

    Background: Gastric cancer (GC) is one of the most common Cancers in the world and Helicobacter pylori (HP) infection is considered a causative factor. The aim of this study was to determine the characteristics and the associated factors of (GC) in a small cohort. Methods: Overall, 54 patients with diagnosed gastric cancer were followed-up at the Department of Gastroenterology&Hepatology at Baqiyatallah University of Medical Sciences. 37 (68.5) of them were positive for H pylori infection in histopathological evaluations. Univariate and multivariate regression analyses were used to determine the associations of demographic features and HP infection status with GC characteristics and prognosis. Results: Univariate analysis showed female gender (odds ratio (OR): 6.53; 95 confidence interval (95CI): 1.59-26.8; P=0.008), and illiteracy (compared to intermediate education; OR: 5.9, 95CI: 1.37-25.43; p=0.05) were associated significantly with higher rate of HP infection. After a mean±SD follow-up duration of 254±329 months, only female gender was significantly associated with HP infection in GC (OR:4.56; 95 CI: 1.0-21.76; P=0.05). H pylori positive patients had significantly higher grade of GC (OR: 3.97; 95 CI: 1.0-16.16; P=0.05), and a trend toward greater GC stage (OR: 4.46, 95 CI: 9.39-21.23; p=0.06). There was no association between survival rate and H pylori infection. Conclusion: In the current study, we found a significant association of female gender with GN and an association of higher grade of GC with female gender. These findings may indicate a sex disparity in susceptibility to HP infection regarding GC future studies of larger populations are recommended

    Frequency of Celiac Disease in Patients With Increased Intestinal Gas (Flatulence)

    Excessive flatulence which impairs social performance in patients is one of the common reasons for referrals to gastroenterology clinics. Celiac Disease is a rare but important cause of increased intestinal gas (bloating) and if not diagnosed, patients face complications such as malabsorption, anemia, osteoporosis and even intestinal lymphoma. This study aimed to determine the frequency of Celiac Disease in patients with excessive flatulence.One hundred and fifty patients with a chief complaint of experiencing flatulence more than 15 times a day and lasting for three months were referred to the gastroenterology clinic of Rasoul-e-Akram Teaching Hospital. Serological tests for Celiac Disease, Anti TTG Ab (IgA-IgG) were requested and the patients with positive tests underwent upper GI endoscopy. Biopsies of the second part of the duodenum were then sent to the laboratory.From one hundred and thirty patients who completed the study, 92 (70.7) were female. Mean age of the patients was 32 ± 13 years. Anti TTG Ab was found in 5 patients (3.85). Only 2 patients (1.5) had a documented positive pathology for Celiac Disease.According to the results of this study and other studies, we conclude that Celiac Disease is an uncommon etiology for excessive flatulence but it is of importance to investigate it in excessive flatulence patients

    Do symbiotic and vitamin e supplementation have favorite effects in nonalcoholic fatty liver disease? A randomized, double-blind, placebo-controlled trial

    Nonalcoholic fatty liver disease (NAFLD) is the most common chronic liver disease in the world. Oral administration of symbiotic and Vitamin E has been proposed as an effective treatment in NAFLD patients. This study was carried out to assess the effects of symbiotic and/or Vitamin E supplementation on liver enzymes, leptin, lipid profile, and some parameters of insulin resistance (IR) in NAFLD patients. Materials and Methods: We randomly assigned sixty NAFLD adult patients to receive (1) symbiotic twice daily + Vitamin E-like placebo capsule; (2) 400 IU/d Vitamin E + symbiotic-like placebo; (3) symbiotic twice daily + 400 IU/d Vitamin E; and (4) symbiotic-like placebo + Vitamin E-like placebo for 8 weeks. Results: Symbiotic plus Vitamin E supplementation led to a significant decrease in concentrations of liver transaminase (P � 0.05). Mean difference of apolipoprotein A-1 was more significant in symbiotic group compared to control. However, mean difference of apolipoprotein B100/A-1 was only significant in symbiotic group compared to control. At the end of the study, significant differences in total cholesterol (TC) and low-density lipoprotein cholesterol (LDL-C) were seen between the symbiotic plus Vitamin E and control groups (P < 0.001). Furthermore, intake of symbiotic plus Vitamin E supplements led to a significant decrease in concentrations of triglycerides (TG) after the intervention. Significant differences in leptin, fasting blood sugar (FBS), and insulin levels were seen between the symbiotic plus Vitamin E and control groups at the end of the study (P < 0.001). In contrast, symbiotic and/or Vitamin E supplementation did not affect high-density lipoprotein cholesterol and homeostasis model assessment for IR levels. Conclusion: In our study, symbiotic plus Vitamin E supplementation was the most effective treatment in lowering liver enzymes, leptin, FBS, insulin, TG, TC, and LDL-C among NAFLD patients. © 2016 Journal of Research in Medical Sciences

    Short term effects of ginger on quality of life, disease activity index, inflammatory and oxidative stress factors in ulcerative colitis

    Background: Chronic inflammation and oxidative stress are the two essential factors determining ulcerative colitis (UC) onset and severity status. In present study, we aimed to investigate short-term effects of ginger (Zingiber officinale) as a well-known antioxidant and anti-inflammatory agent on the quality of life, disease activity index and some of inflammatory and oxidative stress factors in patients with active mild to moderate UC. Methods: This study was a double blind placebo controlled randomized clinical trial conducted in nutrition and diet therapy clinic of Shahid Beheshti University of Medical Science, Tehran, Iran, from October 2017 to June 2018. Forty-six patients with active mild to moderate UC daily consumed four capsules of 500 mg dried ginger powder or similar placebo through eating their meals for 6 weeks. Before and after intervention, we analyzed patient´s scores of disease activity index, by simple clinical colitis activity index questionnaire (SCCAIQ) as well as their quality of life using inflammatory bowel disease questionnaire-9 (IBDQ-9). We also measured serum concentrations of total antioxidant capacity (TAC), malondialdehyde (MDA), tumor necrosis factor (TNF-α), high sensitive (hs)-CRP and nuclear factor κB (NF-κB) in fasted blood samples of each participant. Additionally, anthropometric and dietary intake values of energy, mac-ro/micronutrients and minerals of all of participants were assessed at the same time. Results: While the mean of anthropometric measures and dietary intake values remained unchanged during the study, MDA level decreased in ginger group (P=0.04) compared with placebo group. Additionally, ginger supplementation successfully lowered serum levels of TNF-α and disease activity index after 6 weeks of intervention compared with baseline in ginger consumer group, however the increase of quality of life score was not statistically significant in mentioned group versus baseline values. No significant change in other study outcomes was observed at the end of 6 weeks within and between groups. Conclusion: Our data indicates that two grams per day supplementation with dried ginger powder can reduce oxidative stress level of patients with active mild to moderate UC. © 2019, Tehran University of Medical Sciences.
