    Experimental investigation of an Al2O3/distilled water nanofluid used in the heat pipes of heat exchangers

    This study investigates the thermal performance of a heat pipe heat recovery system in air-to-air heat recovery systems using a nanofluid of Al2O3 (aluminum oxide) particles and distilled water. The experimental setup used 15 wickless vacuumed copper pipes with a length of 1000 mm, a 10.5 mm inner diameter and a 12 mm outer diameter. The evaporator section consists of 450 mm of heat pipes, the condenser section is 400 mm, and the adiabatic section is 150 mm. In experimental studies, 33% of the evaporator volume of the heat pipes was filled with working fluids. Experiments were carried out at temperatures between 25 degrees C and 90 degrees C by using five different cooling air flows (40 g/s, 42 g/s, 45 g/s, 61 g/s and 84 g/s), and two different heating powers (3 kW and 6 kW) for the evaporation section, to determine the heat removed from the condensation section. Experiments were performed for distilled water and Al2O3 nanofluid, respectively, and the results were compared with each other. As a result of the experiments, it was observed that using a nanofluid as the working fluid increased the efficiency of the heat pipe. The highest efficiency (eta = 59%) was obtained in the experiments carried out using an Al2O3 nanofluid at a heating power of 3 kW and an air flow of 112 g/s.This work was supported by Scientific Research Projects Coordination Unit of Karabuk University. It is widely known that a plenty of energy efficiency applications are performed in heat transfer devices. In this regard, it is aimed to improve the thermal performance of recuperator, an air–air heat exchanger, including heat pipe bundle by utilizing a nanofluid, which is the mixture of nanoparticles of a metal oxide and deionized water, by this experimental study. The tests were conducted by filling the heat pipe at the rate of 1/3 of heat pipe volume both deionized water and alumina nanofluid, respectively. Coolant air was used to determine how much heat could be extracted from the condenser section. The findings obtained from the tests show that thermal performance of a heat pipe bundle-including recuperator was remarkably enhanced when nanoparticles containing working fluid was used as the working fluid in place of deionized water.     Mutation and association analysis of the PVR and PVRL2 genes in patients with non-syndromic cleft lip and palate

    Orofacial clefts (OFC; MIM 119530) are among the most common major birth defects. Here, we carried out mutation screening of the PVR and PVRL2 genes, which are both located at an OFC linkage region at 19q13 (OFC3) and are closely related to PVRL1, which has been associated with both syndromic and non-syndromic cleft lip and palate (nsCLP). We screened a total of 73 nsCLP patients and 105 non-cleft controls from the USA for variants in PVR and PVRL2, including all exons and encompassing all isoforms. We identified four variants in PVR and five in PVRL2. One non-synonymous PVR variant, A67T, was more frequent among nsCLP patients than among normal controls, but this difference did not achieve statistical significance

    The content analysis of drug advertisements in two local medical journals

    As a communication and information channel, journals and drug advertisements in them have a significant role. However the regulation on drug advertisements may vary in countries due to the target group. Healthcare professionals, especially physicians and pharmacists are decision makers on drug-use in Turkey like in many other countries. In this respect, drug advertisements in medical journals aim to give information on drugs. Thus healthcare professionals can be more helpful for their patients if the advertisement contains scientific and unbiased information. Because of these, the content of advertisements is important for public health. In this study, the content of advertisements in two local medical journals; “JAMA Türkiye” and “Eczacı” were investigated by using content analysis. As a result, frequently used messages in advertisements were on the effectiveness of drugs. However, the lack of information about side effects, adverse effects and contraindications in drug advertisements were remarkable. Because of the missing points in drug advertisements’ content, it can be stated that messages in the advertisements may not be objective. In the light of these findings both healthcare professionals and publishers of the journals should be more careful and objective while assessing drug advertisements in order to serve better for their patients

    Amaç: Pankreatikoduodenektomi ampulla Vateri, pankreas başı, distal koledok tümörleri ve bazı kronik pankreatit olgularında yaygın kabul gören cerrahi prosedürdür. Rekonstrüksiyon sonrası pankreatik fistül halen ciddi bir problemdir. Rekonstrüksiyon yöntemleri hususunda üzerinde fikir birliği sağlanmış bir yöntem henüz yoktur.Gereç ve Yöntemler: Pankreas tümörü nedeniyle pankreatikoduodenektomi uygulanan hastalarda yapılan rekonstrüksiyon yöntemleri ve sonuçları retrospektif olarak araştırılmıştır. Tüm hastalardaki anastomoz Roux-en-Y şeklinde yapılmış olup birbirinden farkları ise şöyledir; Tip 1: Y bacağı ile sadece pankreatik anastomoz, Tip 2: Y bacağı ile pankreas ve hepatik kanal anastomozu birlikte yapılmıştır.Bulgular: Çalışmaya 31 hasta dahil edilmiştir. Hastaların 21'i erkek, 10'u kadındı. Çalışmamızda pankreatik fistül, kanama, abse, yara yeri enfeksiyonu ve akciğer enfeksiyonu postoperatif dönemde gözlenen komplikasyonlardı. Her ne kadar grup 2'de komplikasyonların sayısı grup 1'e kıyasla daha fazla gözlense de istatistiksel olarak anlamlı fark tespit edilmedi. Mortalite her iki grupta da birer hastada gelişti.Sonuç: Kaçağın sebeplerinden birinin aynı ans üzerine yapılan pankreas ve safra kanalı anastomozlarının birlikte debiyi yükseltmesi ve anastomoz basıncını arttırarak fistül oluşumuna neden olması olduğunu düşünüyoruz. Çalışmamızın dezavantajı ise hasta sayısının az olmasıdır. Pankreatik sıvı ile safranın ayrı anastomozlarla rekonstüksiyonu kronik pankreatik fistülleri azaltabilir.Objective: Pancreaticoduodenectomy is a surgical procedure which is commonly accepted in cases of ampulla of Vater, head of pancreas, distal common bile duct neoplasms and severe chronic pancreatitis. Pancreatic fistula is still a serious problem after reconstruction. Yet, there is no consensus on a single reconstruction method. Material and Methods: The reconstruction methods on patients who had pancreaticoduodenectomy due to pancreatic tumor, and results of these reconstruction methods were retrospectively analyzed. Anastomosis was performed on all patients in the form of Roux-en-Y, but they varied as follows; Type 1: Only pancreatic anastomosis to the Y limb, Type 2: Pancreas and hepatic canal anastomosis together to the Y limb.Results: 31 patients participated in the study. 21 of them were male, and 10 were female. In our study, postoperative complications included pancreatic fistula, hemorrhage, abscess, wound site infection, and pulmonary infection. Although more complications were observed in group 2 than in group 1, there was no statistically significant difference. There was one mortality in each group.Conclusion: In our opinion, one of the reasons of leakage is that anastomosis of both the biliary and pancreatic ducts to the same loop increases anastomotic pressure due to the raised output thus leading to fistula formation. A limitation of our study was the low number of patients. Reconstruction of the pancreas and bile secretions through separate anastomosis may reduce the rate of pancreatic fistulas

    Effectiveness of Limberg and Karydakis flap in recurrent pilonidal sinus disease

    OBJECTIVE: Sacrococcygeal pilonidal sinus is common in young men and may recur over time after surgery. We investigated whether a factor exists that can aid in the determination of the preferred technique between the early Limberg flap and Karydakis flap techniques for treating recurrent pilonidal sinus. MATERIALS AND METHODS: This prospective and randomized study enrolled 71 patients with recurrent pilonidal sinus in whom the Limberg flap or Karydakis flap techniques were applied for reconstruction after excision. Patients were divided into two groups as follows: 37 patients were treated with the Limberg flap technique and 34 patients were treated with the Karydakis flap technique. Fluid collection, wound infection, flap edema, hematoma, partial wound separation, return to daily activities, pain score, complete healing time, painless seating and patient satisfaction were compared between the groups.     A Rare Cause of Intestinal Obstruction: Paraduodenal Hernia

    Internal abdominal hernias may rarely be the cause of intestinal obstruction with an incidence of less than 1% and paraduodenal hernias constitute approximately 50% of them. Those hernias emerge as a result of abnormalities in gut rotation at the embryonic stage. The clinical spectrum of a symptomatic internal hernia may range from abdominal pain to frank intestinal obstruction. Delay in the diagnosis and treatment should be avoided, since the content of paraduodenal hernia may quickly progress to strangulation and necrosis because of vascular compromise.     Diagnosis and treatment in cystic neoplasms of the pancreas: Analysis of 12 cases and review of the literature

    Objectives: Cystic Neoplasms of the Pancreas (CNP)accounts for only 1-5% of all pancreatic neoplasms but inrecent years approximately 30% of all pancreatic resectionsare performed for CNP. In this study we aimed toargue diagnosis, treatment and outcomes of the patientswhom operated in our clinic for CNP.Materials and methods: The demographic characteristics,preoperative findings, surgical procedures, histopathologicaldiagnosis, postoperative complications andlong term follow-up outcomes of the CNP patients operatedin our clinic between 2009 -Jan and 2012-Feb wereevaluated.Results: One patient was male,11 patients were femaleand mean age was 51,5 years (19-73 years).Localizationsof the lesions were in the head of pancreas for 4 patients,in the body for 2 patients and in tail for 6 patients.Mean size of the cysts was 5.1 cm (3-10 cm).Pylorus-reservedpancreaticoduodenectomy for 4 patients, subtotal/distal pancreatectomy + splenectomy for 7 patients andspleen reserved distal pancreatectomy was performedfor 1 patient. Histopathological diagnosis was reportedas serous cystadenoma in 6, mucinous cystadenoma in3, mucinous cystadenocarcinoma in 2 and pseudocystin 1 patient respectively. Postoperative complication ratewas 33%. Mean follow-up time was 18.4 months (2-38months).Conclusions: Management should be based upon oncarefully weighting the malignant potential of a pancreaticcystic lesions and the risk of surgery.Key words: Pancreas, cystic neoplasm, resectio

    Rectus Sheath Hematoma

    A hematoma is a collection of blood in an extravascular space and is named according to its location. Rectus sheath hematoma (RSH) was first described by Hippocrates and Galen about 25 centuries ago due to abdominal trauma, which is a rare cause of acute abdomen. It is uncommon, which may lead to delayed diagnosis in patients with acute abdomen. This condition arises due to trauma or hypertension in patients with bleeding disorders, using anticoagulants, doing heavy physical exercise, pregnant women, connective tissue diseases, and hematological diseases. The diagnosis can be made by detailed anamnesis, physical examination, ultrasonography, and contrast-enhanced abdominal tomography. For a accurate diagnosis, first of all, the medical history of these patients should be carefully questioned. CT and ultrasonography (USG) are used in the diagnosis of this condition. In many patients, conservative treatment by eliminating the predisposing factor is sufficient. In conclusion, with the increase in use of anticoagulation, the incidence of RSH is expected to increase. Every physician in the surgical field should keep rectus sheath hematoma at the top of the differential diagnosis list in patients presenting with acute abdominal pain and palpable abdominal mass
