New paullones as antileishmanial and antitrypanosomal agents
Bei einer Untersuchung verschiedener Paullonderivate an Leishmania donovani wurden Paullon-Chalkon-Hybride als neue antileishmanielle Leitstruktur entdeckt. Mit dem Ziel, die antiprotozoale Aktivität, das Toxizitätsprofil und die physikochemischen Eigenschaften zu verbessern, wurden im Rahmen der vorgelegten Arbeit Paullon-Zimtsäureamid-Hybride als Abwandlung der Paullon-Chalkon-Hybride dargestellt.
Die Paullon-Zimtsäureamid-Hybride zeigen bei in vitro-Untersuchungen an isolierten Erregern (Trypanosoma brucei rhodesiense Trypomastigoten und axenische Amastigoten von Leishmania donovani) antiparasitäre Aktivität mit Hemmkonzentrationen im mikromolaren Bereich. Allerdings erwiesen sich die Verbindungen bei Tests an THP-1-Zellen in ähnlichen Konzentrationen als toxisch für humane Zellen.
Des Weiteren wurden ein Dihydrochalkon-Paullon-Hybrid und ein Paullon-Chalkon-Hybrid mit einer invertierten a,ß-ungesättigten Carbonylstruktur synthetisiert, um den Einfluss der Chalkonstruktur auf die Struktur-Wirkungs-Beziehungen zu untersuchen.
Zur Identifizierung des leishmaniellen Targets der Paullon-Chalkon-Hybride wurde ein Paullon-Chalkon-Hybrid mit einer freien Carboxylgruppe synthetisiert. Kooperationspartner konnten diese Verbindung über die freie Carboxylgruppe an einem festen Trägermaterial (Diaminodipropylamin¬derivatisierte Agarose) immobilisieren. Eine anschließende affinitätschromato¬graphische Trennung von Proteinen eines Leishmanien-Homogenats führte zur Isolierung eines bisher nicht identifizierten Proteins, das als Zielstruktur der hier bearbeiteten Wirkstoffklasse in Frage kommt.A study of several paullone derivatives on Leishmania donovani revealed chalcone paullone hybrid molecules as new antileishmanial lead structures. In the course of this thesis cinnamamide paullone hybrids were synthesized as a modification of chalcone paullone hybrid molecules with the aim to improve antiprotozoal activity, toxicity profile and drug-like and physicochemical properties.
In in vitro assays with isolated pathogens (Trypanosoma brucei rhodesiense trypomastigotes and Leishmania donovani axenic amastigotes) cinnamamide paullone hybrids show an antiparasitic activity with inhibitory concentrations in the micromolar range. However, testing with THP-1 cells demonstrated that the compounds are toxic to human cells in similar concentrations.
Additionally, a dihydrochalcone paullone hybrid and a chalcone paullone hybrid with an inverted a,ß-unsaturated carbonyl structure were synthesized in order to examine the influence of the chalcone scaffold regarding the structure-activity relationships.
With the intention to identify the leishmanial target of the chalcone paullone hybrids, a chalcone paullone hybrid with a free carboxyl group was prepared. Scientists of a collaborating research group immobilized this compound through its free carboxyl group on a resin (diaminodipropylamine derivatized cross-linked agarose). A subsequent protein separation of a Leishmania homogenate using affinity chromatography led to the isolation of a yet unidentified protein which can be considered as a target of the compound class presented here

Congenital adrenal hyperplasia resulting from 17a-hydroxylase/
17.20-lyase deficiency (17OHD) is a rare autosomal recessive disorder. Deficiency of enzyme is caused
by mutation of the P450c17 gene termed CYP17 gene. 17a-Hydroxylase/17.20-lyase deficiency is characterized by reduced
or absent secretion of androgens and estrogens in the gonads as well as cortisol and C19 adrenal steroid precursors, with accompanying ACTH-mediated overproduction
of adrenal mineralocorticoids: 11-deoxicorticosteron(DOC) and corticosteron. It is characterised by hypertension, hypokalaemia and sexual abnormalities such as infantilism in females or male pseudohermaphroditism.
We describe a 34-yr-old female (karyotype 46 XX) with clinical and hormonal features typical for 17a-hydroxylase/17.20-lyase deficiency. She presented hypokalaemic
hypertension, primary amenorrhoea and lack of development of secondary sex features. The hormonal profile of
the patient demonstrated significant reduction of cortisol, estrogens and androgens, as well as elevation of progesterone, ACTH and gonadotropins. We also described the pathogenesis of biochemical and hormonal disturbancies
and different clinical forms of this rare disorder.Wrodzony przerost nadnerczy w następstwie niedoboru enzymu 17a-hydroksylazy/17,20-liazy jest
rzadką chorobą uwarunkowaną genetycznie, dziedziczącą się autosomalnie recesywnie. Jest następstwem mutacji inaktywującej genu kodującego enzym układu cytochromu P450c17 o aktywności zarówno 17a-hydroksylazy, jak i 17,20-liazy i noszącego nazwę CYP17. W wyniku mutacji dochodzi do zmniejszenia lub braku aktywności enzymu i upośledzenia
biosyntezy prekursorów kortyzolu i steroidów płciowych w nadnerczach i gonadach oraz do
nadprodukcji mineralokortykoidów: dezoksykortykosteronu
(DOC) i kortykosteronu. Klasyczny obraz
kliniczny obejmuje nadciśnienie z hipokaliemią oraz niedorozwój płciowy u płci żeńskiej lub obojnactwo
rzekome u płci męskiej.
W niniejszej pracy przedstawiono opis przypadku 34-letniej kobiety, z kariotypem 46 XX, z nadciśnieniem tętniczym, pierwotnym brakiem miesiączki oraz zaburzeniami rozwoju płciowego, u której obraz kliniczny i wyniki badań hormonalnych wskazują na niedobór 17a-hydroksylazy/17,20-liazy, które to podejrzenie podjęto dopiero
w 4. dekadzie życia. Profil hormonalny pacjentki wykazał znaczne zmniejszenie osoczowego stężenia kortyzolu, estrogenów i androgenów z towarzyszącym wzrostem progesteronu, ACTH i gonadotropin.
W artykule przedstawiono także patogenezę zaburzeń biochemicznych i hormonalnych tego rzadkiego zespołu chorobowego oraz jego różnorodne objawy kliniczne
Giant nodular goitre – case report
Wole guzowate nietoksyczne to choroba tarczycy, w której
dominują ogniskowe zaburzenia jej budowy, wynikające
najczęściej z rozrostu oraz ze zmian zwyrodnieniowych
lub zwłóknienia, bez zaburzeń czynności tarczycy. O wolu
mówimy wówczas, gdy jego objętość w obrazie ultrasonograficznym
wynosi > 20ml u kobiet i > 25ml u mężczyzn.
Schorzenie to postępuje powoli i latami może nie powodować
zaburzeń czynności tego narządu. Oznaczenie stężenia
hormonu tyreotropowego (TSH) jest pierwszorzutowym
i podstawowym badaniem w ocenie czynności tego gruczołu.
Natomiast objawy dużego wola mogą być związane z przesunięciem
i uciskiem tarczycy na tchawicę, przełyk czy naczynia
szyjne. Podstawowym badaniem weryfikującym zmiany
w obrębie gruczołu jest ultrasonografia jako nieinwazyjna
i powszechnie dostępna metoda obrazowa w połączeniu
z biopsją aspiracyjną cienkoigłową. Pacjenci z wolem guzowatym
nadczynnym lub objawami uciskowymi powinni być
leczeni radykalnie – operacyjnie lub przy pomocy radiojodu.
W artykule przedstawiony jest opis przypadku pacjentki,
która została przyjęta do szpitala z powodu wola olbrzymiego
z objawami uciskowymi. Choroba postępowała u chorej przez
wiele lat, ale dopiero ograniczona ruchomość szyi i duszność
nasilająca się w pozycji leżącej na wznak spowodowały, iż
pacjentka zgłosiła się do lekarza. W badaniu fizykalnym
uwagę zwracała asymetryczna szyja, ze znacznie powiększonym obwodem, o guzowatej powierzchni i z widocznymi
poszerzonymi naczyniami żylnymi. U opisywanej pacjentki
z uwagi na występujące objawy uciskowe, bardzo dużą masę
gruczołu oraz eutyreozę zdecydowano o radykalnym leczeniu
operacyjnym, które przyniosło znaczną poprawę jakości życia
pacjentki.Nodular goitre of the thyroid is associated with focal structure
abnormalities such as hyperplasia, degeneration or fibrosis
with no functional disorders of the gland. The goitre is
recognised when the volume of the thyroid measured in
ultrasonography is more than 20 ml in women and more
than 25 ml in men. This disease develops slowly and may
not cause any functional disorders for years. Thyrotropic
hormone concentration (TSH) in blood serum is a basic
test in the functional assessment of the gland. However,
symptoms of giant nodular goitre can be connected with
modelling trachea, oesophagus and blood vessels of the
neck. The basic investigation verifying changes in the gland
is ultrasonography, which is non-invasive and available
diagnostic procedure, in connection with fine-needle biopsy.
Patients with toxic goitre or pressure symptoms should be
treated with radical surgery or radioactive iodine. In the
article there was presented a case report of a female patient
who was admitted to the hospital due to giant nodular goitre
with pressure symptoms. The disease had been developing
for many years until restricted mobility of the neck and
dyspnea made her seek for medical advice. Assymetry of the
neck with significantly increased circumference and nodular
surface with enlarged veins were observed in the physical
examination. In the described patient there was radical
surgery performed due to pressure symptoms, large mass of the gland and euthyreosis. The treatment significantly
improved patient’s quality of life

Evaluating the impacts of screening and smoking cessation programmes on lung cancer in a high-burden region of the USA: a simulation modelling study
Objective: While the US Preventive Services Task Force has issued recommendations for lung cancer screening, its effectiveness at reducing lung cancer burden may vary at local levels due to regional variations in smoking behaviour. Our objective was to use an existing model to determine the impacts of lung cancer screening alone or in addition to increased smoking cessation in a US region with a relatively high smoking prevalence and lung cancer incidence. Setting: Computer-based simulation model. Participants: Simulated population of individuals 55 and older based on smoking prevalence and census data from Northeast Pennsylvania. Interventions Hypothetical lung cancer control from 2014 to 2050 through (1) screening with CT, (2) intensified smoking cessation or (3) a combination strategy. Primary and secondary outcome measures Primary outcomes were lung cancer mortality rates. Secondary outcomes included number of people eligible for screening and number of radiation-induced lung cancers. Results: Combining lung cancer screening with increased smoking cessation would yield an estimated 8.1% reduction in cumulative lung cancer mortality by 2050. Our model estimated that the number of screening-eligible individuals would progressively decrease over time, indicating declining benefit of a screening-only programme. Lung cancer screening achieved a greater mortality reduction in earlier years, but was later surpassed by smoking cessation. Conclusions: Combining smoking cessation programmes with lung cancer screening would provide the most benefit to a population, especially considering the growing proportion of patients ineligible for screening based on current recommendations
