    Pharmacotherapy for treatment-resistant schizophrenia

    Schizophrenia is a disabling mental illness with a lifetime prevalence of 0.7% worldwide and significant, often devastating, consequences on social and occupational functioning. A range of antipsychotic medications are available; however, suboptimal therapeutic response in terms of psychotic symptoms is common and affects up to one-third of people with schizophrenia. Negative symptoms are generally less amenable to treatment. Because of the consequences of inadequate symptom control, effective treatment strategies are required for people with treatment-resistant schizophrenia. Clozapine has been shown to be more effective than other antipsychotics in treatment-resistant populations in several studies; however, the occurrence of adverse effects, some of which are potentially life-threatening, are important limitations. In addition to those who are intolerant to clozapine, only 30% to 50% experience clinically significant symptom improvement. This review describes the recent evidence for treatment strategies for people not responding to nonclozapine antipsychotic agents and people not responding or only partially responding to clozapine

    The gradient of meteorological and chemical variables across the tropopause

    The downward transport of air through the tropopause can bring substantial amounts of ozone and reactive nitrogen into the upper troposphere. In this cold region of the atmosphere, O3 is particularly effective as a greenhouse gas. As part of the North Dakota Thunderstorm Project in June 1989, the NCAR Sabreliner made five flights through the tropopause. We measured ozone, nitric oxide (NO), total reactive nitrogen (NO(y)), carbon monoxide (CO), and water vapor (H2)), and took grab samples for hydrocarbon (HC) analysis. Hydrocarbons, CO, and H2O, species with sources primarily at the earth's surface, showed a strong concentration decrease with increasing altitude, while O3 and NO(y), species with a source in the stratosphere, showed a strong concentration increase with increasing altitude. Stratospheric concentrations of NO(x), NO(y), and H2O were all high relative to winter observations made during NASA's AASE. We suggest that midlatitude thunderstorms may inject wet, NO-rich air into the lower stratosphere. Calculation based on measured ratios of NO(x) and NO(y) to O3 yield a total flux of reactive nitrogen from the Northern Hemisphere stratosphere into the troposphere of 1 to 2 Tg(N) yr(exp -1) with about 8 percent in the form of NO(x). This value is higher than reported estimates of total stratospheric nitrogen fixation

    Comparison of three molecular methods for the detection and speciation of Plasmodium vivax and Plasmodium falciparum

    <p>Abstract</p> <p>Background</p> <p>Accurate diagnosis of <it>Plasmodium </it>spp. is essential for the rational treatment of malaria. Despite its many disadvantages, microscopic examination of blood smears remains the current "gold standard" for malaria detection and speciation. PCR assays offer an alternative to microscopy which has been shown to have superior sensitivity and specificity. Unfortunately few comparative studies have been done on the various molecular based speciation methods.</p> <p>Methods</p> <p>The sensitivity, specificity and cost effectiveness of three molecular techniques were compared for the detection and speciation of <it>Plasmodium falciparum </it>and <it>Plasmodium vivax </it>from dried blood spots collected from 136 patients in western Thailand. The results from the three molecular speciation techniques (nested PCR, multiplex PCR, and real-time PCR) were used to develop a molecular consensus (two or more identical PCR results) as an alternative gold standard.</p> <p>Results</p> <p>According to the molecular consensus, 9.6% (13/136) of microscopic diagnoses yielded false negative results. Multiplex PCR failed to detect <it>P. vivax </it>in three mixed isolates, and the nested PCR gave a false positive <it>P. falciparum </it>result in one case. Although the real-time PCR melting curve analysis was the most expensive method, it was 100% sensitive and specific and least time consuming of the three molecular techniques investigated.</p> <p>Conclusion</p> <p>Although microscopy remains the most appropriate method for clinical diagnosis in a field setting, its use as a gold standard may result in apparent false positive results by superior techniques. Future studies should consider using more than one established molecular methods as a new gold standard to assess novel malaria diagnostic kits and PCR assays.</p

    Carbon monoxide measurements at Mace Head, Ireland

    The North Atlantic Ocean is bordered by continents which may each, under the influence of seasonal weather patterns, act as sources of natural and anthropogenic trace gas and particulate species. Photochemically active species such as carbon monoxide (CO) react to form ozone (O3), a species of critical importance in global climate change. CO is sparingly soluble in water, and the relatively long lifetime of CO in the troposphere makes this species an ideal tracer of air masses with origin over land. We have measured CO using a nondispersive infrared gas filter correlation analyzer at Mace Head on the west coast of Ireland nearly continuously since August 9, 1991. Measurements of CO were acquired at 20-sec resolution and recorded as 60-sec averages. Daily, monthly, and diurnal variation data characteristics of CO mixing ratios observed at this site are reported. Depending on source regions of air parcels passing over this site, 60-min concentrations of CO range from clean air values of approximately 90 ppbv to values in excess of 300 ppbv. Data characterizing the correlation between 60-min CO and O3 mixing ratio data observed at this site are reported also

    Determining how polymer-bubble interactions impact algal separation using the novel "Posi"-dissolved air flotation process

    The novel dissolved air flotation (DAF) process that uses hydrophobically-modified polymers (HMPs) to generate positively charged bubbles (PosiDAF) has been shown to separate negatively charged algal cells without the need for coagulation-flocculation. Previous research has been limited to HMPs of poly(N,N-dimethylaminoethyl methacrylate) (PDMAEMA) and, while they were effective at bench-scale, performance at pilot-scale was better using commercial poly(N,N-diallyl-N,N-dimethylammonium chloride) (PDADMAC). Hence, the aim of this research was to compare the effectiveness of PDADMAC modified with aliphatic and aromatic moieties in comparison to previously tested PDMAEMA HMPs in respect to algal cell separation and minimisation of effluent polymer concentration, as well as defining the underlying polymer-bubble interaction mechanisms. Polymer-bubble adhesion properties were measured using atomic force microscopy (AFM) while polymer concentration was monitored via zeta potential and, where possible, assays using fluorescence spectroscopy. Both PDADMAC functionalised with a fluorinated aromatic group (PDADMAC-BCF) and PDMAEMA modified with 1-bromodecane respectively, gave effective cell separation, while the treated effluent zeta potential values at maximum cell removal were lower than the other polymers trialled. The effluent polymer concentration when using PDADMAC-BCF was four times lower in comparison to another aromatically modified PDADMAC polymer. AFM studies indicated that, in contrast to the PDMAEMA-based polymers, the PDADMAC-based polymers did not adsorb closely to the bubble surface. The different polymer-bubble interactions indicate that separation mechanisms will also vary, potentially leading to differences in process effectiveness when explored at pilot scale
