    Neue Biomarker zur Diagnosestellung der spontan bakteriellen Peritonitis

    Die SBP stellt eine schwere Komplikation bei Patienten mit einer Leberzirrhose dar. Die zugrundeliegenden Pathomechanismen sind bis heute nicht abschließend geklärt. Aufgrund der nach wie vor hohen 1 Jahres-Mortalität der SBP sind neue Biomarker zur früheren Diagnosestellung von großer klinischer Bedeutung. In unserer Untersuchung wurden 190 Aszitesflüssigkeitsproben von insgesamt 128 Patienten mittels ELISA und BCA-Assay analysiert. Die Diagnose einer SBP (PMNs ≥ 250/µl Aszitesflüssigkeit) konnte in 29 Proben gestellt werden. Ein klinisch sehr relevantes Ergebnis unserer Studie ist unser Befund, dass eine entzündliche Aktivität in der Aszitesflüssigkeit bereits bei Zellzahlen von weniger als 250 neutrophilen Granulozyten pro µl Aszitesflüssigkeit nachgewiesen werden kann. Es kann keine Korrelation von IL 6 oder IP-10 mit den Diagnosekriterien oder dem Krankheitsverlauf bei einer SBP dargestellt werden. Trotz einer statistischen Korrelation von IL 8 und IL-10 mit den Diagnosekriterien einer SBP eignen sich die beiden Proteine nicht als Biomarker im Krankheitsbild einer SBP. Die C3a-Konzentration zeigt sich im Falle einer SBP verringert, verglichen mit Proben von Patienten ohne Leberzirrhose. Lactoferrin zeigt keine statistische Korrelation mit den Diagnosekriterien einer SBP, jedoch lassen sich aufgrund der Analyse von Verlaufsproben Abschätzungen zum Krankheitsverlauf einer SBP anstellen. Calprotectin korreliert sowohl mit den Diagnosekriterien als auch mit dem Krankheitsverlauf einer SBP und stellt einen potenziellen Biomarker im Krankheitsbild der SBP dar. Im Rahmen dieser Arbeit gelingt die Erstbeschreibung von S100a12 als neuer Biomarker zur Diagnostik und Beurteilung des klinischen Verlaufes im Krankheitsbild der SBP. Die Entwicklung eines Bedside-Tests, ggf. auch als App, aufgrund der in dieser Arbeit gewonnenen Erkenntnisse ist in Planung

    Evaluating the impact of interprofessional training wards on patient satisfaction and clinical outcomes: a mixed-methods analysis

    Introduction: Interprofessional teamwork is pivotal in modern healthcare, prompting the establishment of interprofessional training wards since 1996. While these wards serve as hubs for optimizing healthcare professional collaboration and communication, research into patient outcomes remains notably sparse and geographically limited, predominantly examining patient satisfaction and sparingly exploring other metrics like mortality or self-discharge rates. This study seeks to bridge this gap, comparing patient outcomes in interprofessional training wards and conventional wards under the hypothesis that the former offers no disadvantage to patient outcomes. Materials and methods: We explored patient outcomes within an interprofessional student ward called A-STAR at a University Hospital from October 2019 to December 2022. Engaging with patients discharged between May 2021 and April 2022, we utilized digital and paper-based anonymous questionnaires, catering to patient preference, to gather pertinent data. Results: Analysis of outcomes for 1,482 A-STAR (interprofessional student ward) and 5,752 conventional ward patients revealed noteworthy findings. A-STAR patients tended to be younger (59 vs. 61 years, p < 0.01) and more frequently male (73.5% vs. 70.4%, p = 0.025). Vital clinical outcomes, such as discharges against medical advice, complication-driven readmissions, and ICU transfers, were statistically similar between groups, as were mortality rates (1.2% vs. 1.3%, p = 0.468). A-STAR demonstrated high patient satisfaction, underscored by positive reflections on team competence, ward atmosphere, and responsiveness to concerns, emphasizing the value placed on interprofessional collaboration. Patient narratives commended team kindness, lucid explanations, and proactive involvement. Discussion: This data collectively underscores the safety and reliability of patient care within training wards, affirming that patients can trust the care provided in these settings. Patients on the interprofessional ward demonstrated high satisfaction levels: 96.7% appreciated the atmosphere and conduct of ward rounds. In comparison, 98.3% were satisfied with the discussion and information about their treatment during their hospital stay

    Axon sorting in the optic tract requires HSPG synthesis by ext2 (dackel) and extl3 (boxer)

    AbstractRetinal ganglion cell (RGC) axons are topographically ordered in the optic tract according to their retinal origin. In zebrafish dackel (dak) and boxer (box) mutants, some dorsal RGC axons missort in the optic tract but innervate the tectum topographically. Molecular cloning reveals that dak and box encode ext2 and extl3, glycosyltransferases implicated in heparan sulfate (HS) biosynthesis. Both genes are required for HS synthesis, as shown by biochemical and immunohistochemical analysis, and are expressed maternally and then ubiquitously, likely playing permissive roles. Missorting in box can be rescued by overexpression of extl3. dak;box double mutants show synthetic pathfinding phenotypes that phenocopy robo2 mutants, suggesting that Robo2 function requires HS in vivo; however, tract sorting does not require Robo function, since it is normal in robo2 null mutants. This genetic evidence that heparan sulfate proteoglycan function is required for optic tract sorting provides clues to begin understanding the underlying molecular mechanisms

    Regulation of Zebrafish Skeletogenesis by ext2/dackel and papst1/pinscher

    Mutations in human Exostosin genes (EXTs) confer a disease called Hereditary Multiple Exostoses (HME) that affects 1 in 50,000 among the general population. Patients with HME have a short stature and develop osteochondromas during childhood. Here we show that two zebrafish mutants, dackel (dak) and pinscher (pic), have cartilage defects that strongly resemble those seen in HME patients. We have previously determined that dak encodes zebrafish Ext2. Positional cloning of pic reveals that it encodes a sulphate transporter required for sulphation of glycans (Papst1). We show that although both dak and pic are required during cartilage morphogenesis, they are dispensable for chondrocyte and perichondral cell differentiation. They are also required for hypertrophic chondrocyte differentiation and osteoblast differentiation. Transplantation analysis indicates that dak−/− cells are usually rescued by neighbouring wild-type chondrocytes. In contrast, pic−/− chondrocytes always act autonomously and can disrupt the morphology of neighbouring wild-type cells. These findings lead to the development of a new model to explain the aetiology of HME

    AbstractThe idea that noncrop habitat enhances pest control and represents a win–win opportunity to conserve biodiversity and bolster yields has emerged as an agroecological paradigm. However, while noncrop habitat in landscapes surrounding farms sometimes benefits pest predators, natural enemy responses remain heterogeneous across studies and effects on pests are inconclusive. The observed heterogeneity in species responses to noncrop habitat may be biological in origin or could result from variation in how habitat and biocontrol are measured. Here, we use a pest-control database encompassing 132 studies and 6,759 sites worldwide to model natural enemy and pest abundances, predation rates, and crop damage as a function of landscape composition. Our results showed that although landscape composition explained significant variation within studies, pest and enemy abundances, predation rates, crop damage, and yields each exhibited different responses across studies, sometimes increasing and sometimes decreasing in landscapes with more noncrop habitat but overall showing no consistent trend. Thus, models that used landscape-composition variables to predict pest-control dynamics demonstrated little potential to explain variation across studies, though prediction did improve when comparing studies with similar crop and landscape features. Overall, our work shows that surrounding noncrop habitat does not consistently improve pest management, meaning habitat conservation may bolster production in some systems and depress yields in others. Future efforts to develop tools that inform farmers when habitat conservation truly represents a win–win would benefit from increased understanding of how landscape effects are modulated by local farm management and the biology of pests and their enemies
