    Use of care and the oral health status of people aged 60 years and older in France: results from the National Health and Disability Survey

The fact of living at home or in an institution can influence the use of care and alter the patients’ perception of their oral health status.Methods: The data analyzed were taken from a survey on disability and health carried out in 2008–2009, which is representative of the population living in France. It consists of two sections, one centered on disability and health among home-dwellers and the other on patients living in an institution. In each of these two populations, we carried out descriptive analysis of three themes: use of care, forgoing of care, and oral health.Results: Although visits to physicians and specialists were frequent, visits to dentists were lower in both populations. While a minority of patients forwent care, it was dental care that was mainly forgone by both home-dwellers and institutionalized patients. The cost factor remained the principal reason, but other factors such as fear or accessibility problems were cited. Use of a dental appliance was considerably more frequent among institutionalized patients than among home-dwellers, with just over half the institutionalized population wearing a dental appliance. Perceived state of oral health remained difficult to interpret.Conclusion: To improve access to oral care for the elderly, the patients, their entourage and health providers need increased awareness and information on the importance of good oral health. Better information must be associated with regular clinical examination.     Characteristics and prediction of the alveolar bone loss : essay of modelling

    The alveolar bone loss is a phenomenon which intervenes throughout the life and which can be aggravated by the action of individual and behavioural factors. From this observation we shall try to characterize it and to propose formulas of prediction of the alveolar bone loss according to the age of the patient. We shall expose an analysis of factors bound to the alveolar bone loss and propose a modeling of the alveolar bone loss according to the age in an essentially predictive purpose. The methodology is based on the medical exploitation of CT-dentascanners and medical questionnaire as well as administrative questionnaire used in odontology. Measures of the distance ECJ and the summit of crest are made on the radiology and the individual factors and behavioural factors are scored. The descriptive analysis of the data allowed us to characterize the phenomenon of alveolar bone loss in a therapeutic purpose. The statistical treatment of these data will establish various models according to gender. The described method is simple and its applications seem numerous in the several domains : prevention, improvement of prosthetic and periodontal therapeutics.La résorption alvéolaire est un phénomène qui intervient tout au long de la vie et qui peut être aggravé par l'action de facteurs individuels et comportementaux. En partant de cette constatation nous tenterons de la caractériser et de proposer des formules de prédiction de l'alvéolyse en fonction de l'âge du patient. Nous exposerons une analyse des facteurs liés à l'alvéolyse et proposerons une modélisation de la résorption alvéolaire en fonction de l'âge dans un but essentiellement prédictif. La méthodologie est basée sur l'exploitation de tomodensitométries et de fiches médico-administratives d'usage en odontologie. Des mesures de la distance JEC/sommet de crête sont effectuées grâce à un pied à coulisse digital sur les radiologies et les facteurs individuels et comportementaux sont scorés. L'analyse descriptive des données nous a permis de caractériser le phénomène d'alvéolyse dans un but thérapeutique. Le traitement statistique de ces données a établi différents modèles en fonction du sexe. La méthode décrite est simple et ses applications dans les domaines de la prévention et de l'amélioration des thérapeutiques prothétiques et parodontologiques semblent nombreuses
