    Lactobacillus rhamnosus GG supplementation on eradication rate and dyspepsia in Helicobacter pylori infection treated with three-in-one bismuth quadruple therapy

    IntroductionHelicobacter pylori (Hp)-related dyspepsia has been related to gastroduodenal dysbiosis. The role of probiotic supplementation in the clinical management of Hp infection has been the object of several studies in terms of improvement of efficacy and tolerability of eradication treatments but data on their effects on the outcomes of post-eradication dyspepsia are lacking. The aim of the present study was to evaluate the influence of Lactobacillus rhamnosus GG (LGG) supplementation on bismuth quadruple therapy (BQT) in the clinical management of Hp-related infection both in terms of efficacy and tolerability and persistence of post-treatment dyspepsia. MethodsA total of 164 (121 women) Hp-positive adult patients were enrolled in this pilot study and assigned to two different treatment regimens: group A received BQT for 10 days (three capsules qid, IPP bid) and group B received BQT for 10 days in combination with 6 x 10(9)CFU LGG (ATCC53103) taken for 24 days (7 days before, 10 days during, and 7 days after therapy). Eradication was assessed after 45 days using the C-13-urea breath test (C-13-UBT). Dyspepsia, distinguished into postprandial distress syndrome (PDS) and epigastric pain syndrome (EPS), was assessed at the time of enrollment and 6 months after eradication. ResultsApproximately 98 patients were enrolled in group A and 66 patients in group B. At the enrollment, dyspepsia was present in 76.5% of group A and 86.5% of group B. No significant differences were observed in eradication rate between the 2 groups, both in intention-to-treat (ITT) analysis (82.3 vs. 75.0%) and per-protocol (PP) analysis (95 vs. 96%), and in the presence of side effects during the treatment (70.6 vs. 65.4%). At 6 months after eradication of Hp infection, the persistence of dyspepsia was statistically higher in patients of group A than in group B (38.8 vs. 16.1%; p = 0.032). The positive influence of LGG supplementation in improving post-eradication dyspepsia resulted in statistically more effectiveness in PDS dyspepsia, whose remission was 41.7% in group A and 84% in group B patients (p = 0.011). ConclusionIn conclusion, LGG supplementation during Hp eradication therapy, even if not affecting eradication rates and therapy-related side effects, significantly impacts the remission of dyspepsia

The aim of this work was to undertake a comparison for varietal identity of minor, lesser cultivars in two field collections of neighboring regions (Lombardy and Piedmont) located in northern Italy, verifying synonyms in different areas. Forty-seven accessions were described by morphological (OIV descriptors) and genetic (microsatellites) characteristics. Thirty-eight varieties were identified belonging to three categories: varieties of great importance in Italy and locally misnamed, varieties important in the region of Piedmont and misnamed in Lombardy, and currently minor grapes but of historical significance in both Piedmont and Lombardy regions

    Identification and characterization of Vitis vinifera subsp sylvestris populations in north-western Italy

    The Italian peninsula, for its favorable environmental and geo-morphological conditions, can be considered an ultimate area for survival and development of Vitis vinifera subsp. sylvestris (Gmelin) Hegi, even though severely affected by human impact. Large surveys started in early 1990 throughout the country. At the time few regions, like Piedmont located in the north-west of the country, were considered lacking of wild vinifera. More recent prospection started several years ago, leading to the discovering of five vinifera sylvestris populations plus other sites with few individuals. The sites of discovery were described for their ecological features and the identified plants were referenced and characterized by morphology (18 descriptors from the OIV list) and genetics (14 n-SSR loci). The esteemed consistency of each population ranged from 20 to 150 individuals. Morphological and biological traits (dioecious plants, females producing very small roundish black berries), as well as genetic profiles, indicated the observed plants are true vinifera sylvestris. As to the ecological requirements, plants were confirmed to be highly dependent on water availability into the soil. The neighbor-joining (NJ) dendrogram resulting from SSR allelic pattern of the individuals belonging to the five populations and to one location with isolated plants, indicated population's genetic similarity broadly reflects site's geographic distance. Considering the numerous reports in the past, spreading and consistency of wild grape germplasm from the region of Piedmont severely decreased over a period of 100-150 years. The relative short distance from wild population's sites and vineyards must also be regarded as a worrying condition because of contamination risks. All means to avoid the loss of this native Vitis germplasm must be undertaken by protection policy and proper land management

    Effect of rLH Supplementation during Controlled Ovarian Stimulation for IVF: Evidence from a Retrospective Analysis of 1470 Poor/Suboptimal/Normal Responders Receiving Either rFSH plus rLH or rFSH Alone

    We retrospectively studied a real-life population of 1470 women undergoing IVF, with poor/suboptimal/normal ovarian responsiveness to controlled ovarian stimulation (COS), comparing the cumulative live birth rate (cLBR) when COS was performed using rFSH alone or rFSH + rLH in a 2:1 ratio. Overall, we observed significantly higher cLBR in the rFSH alone group than in the rFSH + rLH group (29.3% vs. 22.2%, p < 0.01). However, considering only suboptimal/poor responders (n = 309), we observed comparable cLBR (15.6% vs. 15.2%, p = 0.95) despite the fact that patients receiving rFSH + rLH had significantly higher ages and worse ovarian reserve markers. The equivalent effectiveness of rFSH + rLH and rFSH alone was further confirmed after stratification according to the number of oocytes retrieved: despite basal characteristics were still in favor of rFSH alone group, the cLBR always resulted comparable. Even subdividing patients according to the POSEIDON classification, irrespective of differences in the baseline clinical characteristics in favor of FSH alone group, the cLBR resulted comparable in all subgroups. Despite the retrospective, real-life analysis, our data suggest that rLH supplementation in COS may represent a reasonable option for patients with predictable or unexpected poor/suboptimal ovarian responsiveness to FSH, those matching the Bologna criteria for poor responsiveness, and those included in the POSEIDON classification

    A note on gravitational wave lensing

    In a recent paper [Ruffa] it was proposed that the massive black hole at the Galactic center may act as a gravitational lens focusing gravitational wave energy to the Earth. Considering the gravitational wave signal emitted by galactic spinning pulsars, an enhancement in the gravitational wave intensity by a factor of a few thousand is found. For galactic and extra-galactic sources the intensity enhancement can be as high as 4,000 and 17,000, respectively. In this note we consider the probability of significant signal enhancement from galactic and extra-galactic pulsars by the proposed mechanism and find that it is actually negligible. The lensing effect due to a possible companion object (a star or the galactic center black hole) of the gravitational wave source is also investigated in the framework of the classical microlensing theory.Comment: 4 pages, 1 Postscript figure, to be published in Astronomy and Astrophysic

    WISDOM project -- XVI. SMBH mass in the early-type galaxies NGC0612, NGC1574, and NGC4261 from CO dynamical modelling

    We present a CO dynamical estimate of the mass of the super-massive black hole (SMBH) in three nearby early-type galaxies: NGC0612, NGC1574 and NGC4261. Our analysis is based on Atacama Large Millimeter/submillimeter Array (ALMA) Cycle 3-6 observations of the 12^{12}CO(2-1) emission line with spatial resolutions of 145814-58 pc (0.010.260.01''-0.26''). We detect disc-like CO distributions on scales from 200\lesssim200 pc (NGC1574 and NGC4261) to 10\approx10 kpc (NGC0612). In NGC0612 and NGC1574 the bulk of the gas is regularly rotating. The data also provide evidence for the presence of a massive dark object at the centre of NGC1574, allowing us to obtain the first measure of its mass, MBH=(1.0±0.2)×108M_{\rm BH}=(1.0\pm0.2)\times10^{8} M_{\odot} (1σ\sigma uncertainty). In NGC4261, the CO kinematics is clearly dominated by the SMBH gravitational influence, allowing us to determine an accurate black hole mass of (1.62±0.04)×109(1.62{\pm 0.04})\times10^{9} M_{\odot} (1σ1\sigma uncertainty). This is fully consistent with a previous CO dynamical estimate obtained using a different modelling technique. Signs of non-circular gas motions (likely outflow) are also identified in the inner regions of NGC4261. In NGC0612, we are only able to obtain a (conservative) upper limit of MBH3.2×109M_{\rm BH}\lesssim3.2\times10^{9} M_{\odot}. This has likely to be ascribed to the presence of a central CO hole (with a radius much larger than that of the SMBH sphere of influence), combined with the inability of obtaining a robust prediction for the CO velocity curve. The three SMBH mass estimates are overall in agreement with predictions from the MBHσM_{\rm BH}-\sigma_{\star} relation.Comment: Main text: 20 pages, 14 Figures; Appendix: 7 pages, 6 Figures.     The use of chest ultrasonography in suspected cases of COVID-19 in the emergency department

    Aim: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) virus-specific reverse transcriptase-polymerase chain reaction (RT-PCR) represents the diagnostic gold standard. We explored the value of chest ultrasonography to predict positivity to SARS-CoV-2 on RT-PCR in suspected COVID-19 cases. Patients & methods: Consecutive patients with suspect COVID-19 were included if they had fever and/or history of cough and/or dyspnea. Lung ultrasound score (LUSS) was computed according to published methods. Results: A total of 76 patients were included. A 3-variable model based on aspartate transaminase (AST) > upper limit of normal, LUSS >12 and body temperature >37.5°C yielded an overall accuracy of 91%. Conclusion: A simple LUSS-based model may represent a powerful tool for initial assessment in suspected cases of COVID-19. The gold standard for diagnosis of COVID-19 is RT-PCR. During a pandemic emergency, it may be useful to identify suspect symptomatic patients who may safely be observed without undergoing testing for COVID-19. In this work, a simple model based on the findings of lung ultrasound, AST levels and fever showed an overall accuracy of 91% to predict the results of RT-PCR

    The WISDOM of power spectra: how the galactic gravitational potential impacts a galaxy?s central gas reservoir in simulations and observations

    Observations indicate that the central gas discs are smoother in early-type galaxies than their late-type counterparts, while recent simulations predict that the dynamical suppression of star formation in spheroid-dominated galaxies is preceded by the suppression of fragmentation of their interstellar media. The mass surface density power spectrum is a powerful tool to constrain the degree of structure within a gas reservoir. Specifically here, we focus on the power spectrum slope and aim to constrain whether the shear induced by a dominant spheroidal potential can induce sufficient turbulence to suppress fragmentation, resulting in the smooth central gas discs observed. We compute surface density power spectra for the nuclear gas reservoirs of fourteen simulated isolated galaxies and twelve galaxies observed as part of the mm-Wave Interferometric Survey of Dark Object Masses (WISDOM) project. Both simulated and observed galaxies range from disc-dominated galaxies to spheroids, with central stellar mass surface densities, a measure of bulge dominance, varying by more than an order of magnitude. For the simulations, the power spectra steepen with increasing central stellar mass surface density, thereby clearly linking the suppression of fragmentation to the shear-driven turbulence induced by the spheroid. The WISDOM observations show a different (but potentially consistent) picture: while there is no correlation between the power spectrum slopes and the central stellar mass surface densities, the slopes scatter around a value of 2.6. This is similar to the behaviour of the slopes of the simulated galaxies with high central stellar mass surface densities, and could indicate that high shear eventually drives incompressible turbulence
