Evaluation of Metamizole and Carprofen as postoperative analgesics in canine total hip replacement

Evaluierung von Metamizol und Carprofen als postoperative Analgetika nach Hüftgelenksersatz bei Hunden. Das Ziel dieser Studie war es, die analgetische Wirkung der von den jeweiligen Arzneimittelfirmen für den Hund empfohlenen Dosierungen von Metamizol im Vergleich zu Carprofen nach Hüftgelenkersatz zu bewerten. Es ist bekannt, dass Metamizol ein potentes Analgetikum beim Menschen ist. Bis heute gibt es keine Studien zur postoperativen Wirksamkeit von Metamizol beim Hund. Subjektive (Melbourne Schmerzskala (mMPS) und visuelle Analogskala (VAS)) und objektive (Ganganalyse, in welcher die vertikale Spitzenkraft (PVF) und der vertikale Impuls gemessen wurden) Bewertungsverfahren wurden in dieser Studie für die Evaluierung der Schmerzen herangezogen. 39 klinisch gesunde Hunde mit einem Körpergewicht zwischen 5,5 und 60,5 kg (keine Rassespezifität) wurden in diese Studie eingeschlossen. Die Hunde wurden nach Randomisierung in zwei Gruppen verteilt: Tiere der Gruppe M (n = 19) erhielten 50 mg•kg-1 IV Metamizol TID. Tiere der Gruppe C (n = 20) erhielten 4 mg•kg-1 Carprofen IV SID. Die Patienten wurden 3, 6, 9, 12, 20, 24, 28, 32, 36, 44, 48 and 56 Stunden nach Operationsende subjektiv beurteilt. Wurden bei der Evaluierung mittels mMPS bzw. VAS Punktwerte von 12 bzw. 50 Punkten überschritten, so wurde dies als Anzeichen von Schmerzen betrachtet, welche mit einer intravenösen Gabe von Buprenorphin, 10 μg•kg-1, behandelt wurden (rescue analgesia). Eine Ganganalyse wurde einmal präoperativ (preOP) und dann am ersten (OP1) und zweiten (OP2) postoperativen Tag durchgeführt. Drei Patienten in der Gruppe C benötigten in den ersten 3 bis 6 postoperativen Stunden rescue analgesia. Keines der Tiere in Gruppe M benötigte die Gabe zusätzlicher Schmerzmittel. Sowohl bei der mMPS als auch bei der VAS zeigten Tiere der Gruppe M im Vergleich zu Gruppe C über den gesamten Zeitraum niedrigere Schmerz-Werte. Je nach verwendeter Schmerzskala waren diese Unterschiede nach 6 h (mMPS) bzw. nach 24 h (VAS) als signifikant zu betrachten (p < 0.05). Die postoperativen Ganganalysen zeigten bei Hunden der Gruppe M eine bessere Belastung der operierten Gliedmaße (p < 0.05). Diese Ergebnisse zeigen, dass die alleinige Verwendung von Metamizol als Analgetikum eine potente und zufriedenstellende Analgesie nach orthopädischen Eingriffen bei Hunden gewährleistet. Des Weiteren wurde gezeigt, dass die alleinige Verwendung von Carprofen in der vom Hersteller empfohlenen Dosierung nach Hüftgelenksersatz bei Hunden nicht immer eine zufriedenstellende Analgesie hervorruft

The management of acute venous thromboembolism in clinical practice. Results from the European PREFER in VTE Registry

Venous thromboembolism (VTE) is a significant cause of morbidity and mortality in Europe. Data from real-world registries are necessary, as clinical trials do not represent the full spectrum of VTE patients seen in clinical practice. We aimed to document the epidemiology, management and outcomes of VTE using data from a large, observational database. PREFER in VTE was an international, non-interventional disease registry conducted between January 2013 and July 2015 in primary and secondary care across seven European countries. Consecutive patients with acute VTE were documented and followed up over 12 months. PREFER in VTE included 3,455 patients with a mean age of 60.8 ± 17.0 years. Overall, 53.0 % were male. The majority of patients were assessed in the hospital setting as inpatients or outpatients (78.5 %). The diagnosis was deep-vein thrombosis (DVT) in 59.5 % and pulmonary embolism (PE) in 40.5 %. The most common comorbidities were the various types of cardiovascular disease (excluding hypertension; 45.5 %), hypertension (42.3 %) and dyslipidaemia (21.1 %). Following the index VTE, a large proportion of patients received initial therapy with heparin (73.2 %), almost half received a vitamin K antagonist (48.7 %) and nearly a quarter received a DOAC (24.5 %). Almost a quarter of all presentations were for recurrent VTE, with &gt;80 % of previous episodes having occurred more than 12 months prior to baseline. In conclusion, PREFER in VTE has provided contemporary insights into VTE patients and their real-world management, including their baseline characteristics, risk factors, disease history, symptoms and signs, initial therapy and outcomes

Memory cells and digital circuits mainly occupy silicon area in smart-cards, so strong density technologies are used, but associated supply voltage Vdd becomes lower than 1 volt. Besides, smard-cards are dedicated to portable application, so their consumption is limited, whereas required functionnalities increase, then elementary function consumption must be reduced. Thus, designer sizes analog cells working with nanowatt specifications (low voltage supply - no more 1 volt- and low consumption- few tens at few hundreds nano-amper). This study deals with a nanowatt analog circuit design methodology and its application to smart-card area. Self-biased and fixed bias architectures were designed with the developped methodology. Circuits sized for nanowatt specifications were simulated with Spectre and BSIM3v3 parameters of ATMEL 0.15µm CMOS technology. Transistors used in circuits are high voltage devices (HV thick oxide) Simulation results and methodology prediction are similar. Intangible and tangible heritage

‘Intangible and tangible heritage – a topology of culture in contexts of faith’ presents a conceptual framework which could enable heritage professionals to approach cul-tural heritage in a more holistic understanding. My work emphasizes opportunities for a re-combination – in conceptual and practical terms – of two recently divided heritage typologies: the so-called ‘intangible’ and ‘tangible’ heritage. In arguing that the above division cannot be maintained when observing the dynamic construction and re-affirmation processes of heritage and identity, and further, that this division is a risk to the preservation of the heritage of humankind, I will emphasize that it is important to halt and redirect the progressing divergence of the two fields. This is particularly necessary in the context of UNESCO, which is the driving force behind this conceptual separation. rnTo achieve a conceptual recombination I propose to approach heritage by means of topologies instead of typologies. In topological analysis the researcher’s focus shifts from heritage expressions towards ideas or concepts of heritage, which are defined as logos localised in place, topos, and are proposed to be analysed by means of semiotic phenomenology. Finally, I describe the findings of a topological analysis conducted for a particular heritage concept: the Umayyad Mosque in Damascus
