    恶性黑色素瘤合并嗜肺军团菌感染性肺炎病例报告

    Legionella pneumonia is mainly in community-acquired and nosocomial pneumonias caused by the L. pneumophila. The paper reported a case of Legionella pneumonia caused by Legionella pneumophila Sg1 in a man with malignant melanoma. The method for diagnosing Legionella pneumonia by standard culture method,serotyping,PCR-enzymatic digestion analysis and gene sequencing was elaborate. To confirm the diagnosis result of this rapid diagnostic method, sequencing of the bacteria in patient’s sputum partial gene was also carried out. The diagnosis result of this rapid diagnostic method was consistent with the culture method which indicated that it was effective in diagnosing L. pneumophila infection.军团菌肺炎主要是由嗜肺军团菌感染引起的一种社区获得性或医院内感染性肺炎。本文报告了1例临床上极为罕见的恶性黑色素瘤合并嗜肺军团菌血清1型感染引起的军团菌肺炎,并对其实验室诊断作了系统描述,包括病人痰液标本的细菌分离培养、血清学分型、PCR-酶切分型和基因测序鉴定等分子生物学诊断技术,结果表明PCR-酶切分型对于诊断军团菌病是一种快速、准确可靠的试验方法

    Disorder-induced linear magnetoresistance in Al2_2O3_3/SrTiO3_3 heterostructures

    The unsaturated linear magnetoresistance (LMR) has attracted widely attention because of potential applications and fundamental interest. By controlling the growth temperature, we realized the metal-to-insulator transition in Al2_2O3_3/SrTiO3_3 heterostructures. The LMR is observed in metallic samples with the electron mobility varying over three orders of magnitude. The observed LMR cannot be explained by the guiding center diffusion model even in samples with very high mobility. The slope of the observed LMR is proportional to the Hall mobility, and the crossover field, indicating a transition from quadratic (at low fields) to linear (at high fields) field dependence, is proportional to the inverse Hall mobility. This signifies that the classical model is valid to explain the observed LMR. More importantly, we develop an analytical expression according to the effective-medium theory that is equivalent to the classical model.     In the title complex, [Dy(C7H3NO4)(C7H4NO4)]n, one of the ligands is fully deprotonated while the second has lost only one H atom. Each DyIII ion is coordinated by six O atoms and two N atoms from two pyridine-2,6-dicarboxyl­ate and two 6-carboxy­picolinate ligands, displaying a bicapped trigonal-prismatic geometry. The average Dy—O bond distance is 2.40 Å, some 0.1Å longer than the corresponding Ho—O distance in the isotypic holmium complex. Adjacent DyIII ions are linked by the pyridine-2,6-dicarboxyl­ate ligands, forming a layer in (100). These layers are further connected by π–π stacking inter­actions between neighboring pyridyl rings [centroid–centroid distance = 3.827 (3) Å] and C—H⋯O hydrogen-bonding inter­actions, assembling a three-dimensional supra­molecular network. Within each layer, there are other π–π stacking inter­actions between neighboring pyridyl rings [centroid–centroid distance = 3.501 (2) Å] and O—H⋯O and C—H⋯O hydrogen-bonding inter­actions, which further stabilize the structure

    Factors related to children’s caries: a structural equation modeling approach

    BACKGROUND: Dental caries among preschool children is highly prevalent in many less-developed countries. METHODS: A model which explored the factors related to children’s dental caries was tested in this study using structural equation modeling. Caregivers of children aged 5 years were surveyed on their socioeconomic status, and their oral health knowledge, attitudes and practices. In addition, information on their children’s oral health practices, dental insurance and dental service utilization were collected. Examination of caries was conducted on all children who returned fully completed questionnaires. RESULTS: The results showed that socioeconomic factors influenced children’s oral health practices through the impact of caregivers’ oral health knowledge and practices; that caregivers’ oral health knowledge affected children’s oral health practices through the influence of caregivers’ oral health attitudes and practices; and finally, that children’s oral health practices were linked directly to their caries. CONCLUSION: The findings have important applications for promoting policies aimed at advancing children’s oral health

    Pharmacokinetics of diltiazem hydrochloride delay-onset sustained-release pellet capsules in healthy volunteers

    Embora a farmacocinética (PK) do cloridrato de diltiazem nas formas de comprimidos de liberação imediata e cápsulas de liberação modificada em ensaios clínicos já tenha sido relatada, a pesquisa da PK do cloridrato de diltiazem na forma de cápsulas com peletes de liberação retardada e sustentada ainda é muito importante. Neste trabalho, propusemos avaliar a farmacocinética do cloridrato de diltiazem administrado através desta nova forma farmacêutica em voluntários chineses sadios, assim como a influência da ingestão de alimentos neste perfil farmacocinético. Foi realizado um ensaio clínico aberto, randomizado e paralelo em 36 voluntários, que receberam dose oral única de 90 mg, 180 mg ou 270 mg e dose múltiplas (90 mg/d × 6 d) pela mesma via de administração. Para avaliar o efeito da ingestão de alimentos sobre a PK do diltiazem foi realizada a administração de dose única (360 mg) em 24 voluntários chineses sadios. A concentração plasmática do diltiazem foi determinada por Cromatografia Liquida de Alta Eficiência em fase reversa (CLAE-FR) e os principais parâmetros farmacocinéticos foram analisados através do emprego do software PKSolver (Ver 2.0). O ensaio de farmacocinética clínica foi conduzido na clínica Pharmacological Center (No.JDX1999064) do Hospital de Xiangya, Central South University, China. Os parâmetros PK obtidos indicaram que a nova formulação de cápsulas de liberação retardada e sustentada de cloridrato de diltiazem possue marcantes características de liberação retardada e controlada do fármaco.The pharmacokinetics (PK) of ordinary tablets and sustained release capsules of diltiazem hydrochloride in human clinical trials had been studied. The PK of diltiazem hydrochloride delay-onset sustained-release pellet capsules, a new dosage form, has not been reported, although it is very important to clinical use. In this paper, we investigated the PK of diltiazem hydrochloride delay-onset sustained-release pellet capsules and the food influence in Chinese healthy volunteers. The PK parameters indicated that the diltiazem hydrochloride delay-onset sustained-release pellet capsules appeared marked characteristics of delayed and controlled release. An opened-label, randomized and parallel clinical trial was conducted in 36 Chinese healthy volunteers with single oral dose (90 mg, 180 mg or 270 mg) and a multiple oral dose (90 mg d-1×6 d) administration. The effect of food on the PK of one single oral dose (360 mg) was investigated in 24 healthy Chinese volunteers. Plasma diltiazem concentration was determined by reversed-phase high-performance liquid chromatography (RP-HPLC) and the main pharmacokinetic parameters were analyzed by PKSolver (Ver 2.0). All clinical studies were conducted in the Clinical Pharmacological Center (No. JDX1999064) of Xiangya Hospital Affiliated Central South University, China. The PK parameters suggested that the new formulation had marked characteristics of delayed and controlled release of diltiazem, and food intake did not alter significantly diltiazem pharmacokinetic parameters

    Variants of Interleukin-22 Gene Confer Predisposition to Autoimmune Thyroid Disease

    As there are no previous studies on the interleukin-22 (IL-22) variants in autoimmune thyroid disease (AITD), the present study aimed to explore the association between polymorphisms of IL-22 and the predisposition to AITD. The study had 975 AITD patients, including 639 Graves’ disease (GD) and 336 Hashimoto’s thyroiditis (HT) individuals and 851 healthy cohorts. Ligase detection reaction (LDR) and direct sequencing method were used for genotyping the IL-22 gene polymorphisms at rs2046068, rs2227478, rs2227485, rs11611206, and rs1179251. In comparison to female controls, genotype CC of rs1179251 was increased in the female AITD patients. Alleles C at rs2046068, C at rs2227478, and C at rs1179251 linked to the susceptibility of HT males. Genotype CC in rs1179251 was higher in male HT. Variants at rs2046068, rs2227478, and rs1179251 were associated with the AITD teenagers. Besides, genotype GG in rs11611206 was correlated with thyroid-associated ophthalmopathy (TAO). Moreover, allele G at rs11611206 was associated with decreased risk for TAO by 28.9%. Similarly, genotype CC of rs1179251 and genotype GG of rs11611206 were associated with Graves’ ophthalmopathy (GO). Allele G in rs11611206 increased people with HT towards the predisposition of hypothyroidism. In conclusion, genetic variants of IL-22 are associated with the occurrence of AITD
