Investigating the Numerical Stability of Using an Impedance Boundary Condition to Model Broadband Noise Scattering With Acoustic Liners
Reducing aircraft noise is a major objective in the field of computational aeroacoustics. When designing next generation quiet aircraft, it is important to be able to accurately and efficiently predict the acoustic scattering by an aircraft body from a given noise source. Acoustic liners are an effective tool for achieving aircraft noise reduction and are characterized by a frequency-dependent impedance value. Converted into the time-domain using Fourier transforms, an impedance boundary condition can be used to simulate the acoustic wave scattering by geometric bodies treated with acoustic liners. A Broadband Impedance Model will be discussed in which the liner impedance is specified along a wide range of frequencies. The liner impedance boundary condition will be derived and coupled with a time-domain boundary integral equation to model acoustic scattering by a flat plate consisting of both un-lined and lined surfaces. The scattering solution will be obtained iteratively using both spatial and temporal basis functions and the stability will be demonstrated through eigenvalue analysis. Stability will be assessed for its dependence on time step, spatial discretization, as well as temporal basis function order. Both second- and third-order backward difference Lagrange temporal basis functions are considered
Is the Focal Muscle Vibration an Effective Motor Conditioning Intervention? A Systematic Review
Mechanical vibration, applied to single or few muscles, can be a selective stimulus for muscle spindles, able to modify neuromuscular management, inducing short and long-term effects, are now mainly employed in clinic studies. Several studies reported as treatments with focal vibratory (FVT) can influence neuromuscular parameters also in healthy people. However, the application modalities and the consequent effects are remarkably fragmented. This paper aims to review these studies and to characterize the FVT effectiveness on long-term conditional capacities in relation to FVT characteristics. A systematic search of studies published from 1985 to 2020 in English on healthcare databases was performed. Articles had to meet the following criteria: (1) treatment based on a locally applied vibration on muscle belly or tendon; (2) healthy adults involved; (3) outcomes time analysis enduring for more than 24 h. Twelve studies were found, all of them presented an excellent quality score of 6575%. All selected papers reported positive changes, comparable with traditional long-lasting training effects. Muscle force and power were the most investigated parameters. The after-effects persisted for up to several months. Among the different FV administration modalities, the most effective seems to show a stimulus frequency of 48100 Hz, repeated more times within three-five days on a voluntary contracted muscle
Yersinia enterocolitica in Italy. A case of septicemia and abdominal aortic aneurysm infection
We report a case of Yersinia enterocolitica septicemia in a 63-year-old patient admitted to the Vascular Surgery Department of Umberto I Hospital (Rome, Italy) for an abdominal aortic aneurysm. The microorganism, recovered from both peripheral blood cultures and aneurysmatic aortic wall specimens, was identified as Y. enterocolitica using matrix-assisted laser desorption ionization-time of flight analysis (MALDI-TOF MS) and 16S rDNA gene sequencing. The isolate responsible for septicemia belonged to the O:9 serotype (biogroup 2). A genetic screening of the isolate made it possible to detect the presence of both the yst and ail genes, encoding a heat-stable enterotoxin and a protein involved in invasion/adherence and serum resistance, respectively. Our case contributes in enriching epidemiological data concerning Y. enterocolitica infections, which might represent severe complications in patients suffering from cardiovascular diseases. Moreover, this study, together with the others, should be regarded as valuable and useful tools for monitoring the rate of infections worldwide
The muscles of the athletes to learn surface anatomy - The Influence of classical statues on anatomy teaching
Gross anatomy classes are still regarded as an integral part of human biomedical education worldwide. The first documentary evidence of the practice of anatomical dissection for teaching purposes dates back to the 13th century AD, although this practice seems to have originated in Ancient Greece, if not in earlier times. Dissection of the human body is practiced in most anatomy institutions worldwide despite increasing pressure to reduce material and staff costs, regardless the ongoing debate concerning the suitability of body donors for medical education. Moreover, anatomical teaching skills are also evolving and need to be tailored for the different areas of anatomical expertise students have to acquire: therefore, anatomic dissection goes probably beyond the scope of anatomy teaching in some classes such as sports sciences. However, there is no doubt that a practical approach to the study and teaching of anatomy is surely preferable to basic ex cathedra anatomy lectures. JC virus-DNA detection is associated with CD8 fffector accumulation in peripheral blood of patients with multiple sclerosis under natalizumab treatment, independently from JC virus serostatus
Although natalizumab (anti-α4 integrin) represents an effective therapy for relapsing remitting multiple sclerosis (RRMS), it is associated with an increased risk of developing progressive multifocal leukoencephalopathy (PML), caused by the polyomavirus JC (JCV). The aim of this study was to explore natalizumab-induced phenotypic changes in peripheral blood T-lymphocytes and their relationship with JCV reactivation. Forty-four patients affected by RRMS were enrolled. Blood and urine samples were classified according to natalizumab infusion number: 0 (N0), 1-12 (N12), 13-24 (N24), 25-36 (N36), and over 36 (N > 36) infusions. JCV-DNA was detected in plasma and urine. T-lymphocyte phenotype was evaluated with flow cytometry. JCV serostatus was assessed. Ten healthy donors (HD), whose ages and sexes matched with the RRMS patients of the N0 group, were enrolled. CD8 effector (CD8 E) percentages were increased in natalizumab treated patients with detectable JCV-DNA in plasma or urine compared to JCV-DNA negative patients (JCV-) (p < 0.01 and p < 0.001, resp.). Patients with CD8 E percentages above 10.4% tended to show detectable JCV-DNA in plasma and/or urine (ROC curve p = 0.001). The CD8 E was increased when JCV-DNA was detectable in plasma or urine, independently from JCV serology, for N12 and N24 groups (p < 0.01). As long as PML can affect RRMS patients under natalizumab treatment with a negative JCV serology, the assessment of CD8 E could help in the evaluation of JCV reactivation
Merkel cell polyomavirus (MCPyV) in the context of Immunosuppression. Genetic analysis of noncoding control region (NCCR) variability among a HIV-1-positive population
Background: Since limited data are available about the prevalence of Merkel cell polyomavirus (MCPyV) and the genetic variability of its noncoding control region (NCCR) in the context of immunosuppression, this study aimed to investigate the distribution of MCPyV in anatomical sites other than the skin and the behavior of NCCR among an HIV-1-positive population. Methods: Urine, plasma, and rectal swabs specimens from a cohort of 66 HIV-1-positive patients were collected and subjected to quantitative real-time polymerase chain reaction (qPCR) for MCPyV DNA detection. MCPyV-positive samples were amplified by nested PCR targeting the NCCR, and NCCRs alignment was carried out to evaluate the occurrence of mutations and to identify putative binding sites for cellular factors. Results: MCPyV DNA was detected in 10/66 urine, in 7/66 plasma, and in 23/66 rectal samples, with a median value of 5 × 102 copies/mL, 1.5 × 102 copies/mL, and 2.3 × 103 copies/mL, respectively. NCCR sequence analysis revealed a high degree of homology with the MCC350 reference strain in urine, whereas transitions, transversions, and single or double deletions were observed in plasma and rectal swabs. In these latter samples, representative GTT and GTTGA insertions were also observed. Search for putative binding sites of cellular transcription factors showed that in several strains, deletions, insertions, or single base substitutions altered the NCCR canonical configuration. Conclusions: Sequencing analysis revealed the presence of numerous mutations in the NCCR, including insertions and deletions. Whether these mutations may have an impact on the pathogenic features of the virus remains to be determined. qPCR measured on average a low viral load in the specimens analyzed, with the exception of those with the GTTGA insertion
Increased prevalence of Human Polyomavirus JC viruria in Chronic Inflammatory Rheumatic Diseases patients in treatment with anti-TNF α: a 18 month follow-up study.
Chronic inflammatory rheumatic diseases (CIRDs) are immune-mediated pathologies
involving joints. To date, TNFα-blocking agents administration is the most promising
therapy, although these treatments are associated with an increased Polyomavirus
JC (JCPyV) reactivation, the etiological agent of the Progressive Multifocal
Leukoencephalopathy (PML). The aim of this study was the recruitment and the
analysis of a CIRDs cohort in order to investigate a possible correlation between
JCPyV presence and the influence of anti-TNF-α agents on viral loads. Blood and urine
samples were collected from 34 CIRDs subjects prior the first anti-TNF-α infusion (T0)
and after 3 (T3), 6 (T6), 12 (T12), and 18 (T18) months. Results showed persistent JC
viruria significantly higher than JC viremia throughout the 18 month follow-up study
(p=0.002). In JCPyV positive samples, the non-coding control region (NCCR) was
analyzed. Results evidenced archetypal structures (type II-S) in all isolates with the
exception of a sequence isolated from a plasma sample, that corresponds to the type
II-R found in PML subjects. Finally, the viral protein 1 (VP1) genotyping was performed
and results showed the prevalence of the European genotypes 1A, 1B, and 4. Since
only few studies have been carried out to understand whether there is a PML risk in
CIRDs population infected by JCPyV, this study contributes to enrich literature insight on
JCPyV biology in this cluster. Further investigations are necessary in order to recognize
the real impact of biologics on JCPyV life cycle and to identify possible and specific viral
variants related to increased virulence in CIRDs patient
SARS-CoV-2 diagnostics in the virology laboratory of a University Hospital in Rome during the lockdown period
Italy was one of the most affected nations by coronavirus disease 2019 outside China. The infections, initially limited to Northern Italy, spread to all other Italian regions. This study aims to provide a snapshot of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) epidemiology based on a single-center laboratory experience in Rome. The study retrospectively included 6565 subjects tested for SARS-CoV-2 at the Laboratory of Virology of Sapienza University Hospital in Rome from 6 March to 4 May. A total of 9995 clinical specimens were analyzed, including nasopharyngeal swabs, bronchoalveolar lavage fluids, gargle lavages, stools, pleural fluids, and cerebrospinal fluids. Positivity to SARS-CoV-2 was detected in 8% (527/6565) of individuals, increased with age, and was higher in male patients (P <.001). The number of new confirmed cases reached a peak on 18 March and then decreased. The virus was detected in respiratory samples, in stool and in pleural fluids, while none of gargle lavage or cerebrospinal fluid samples gave a positive result. This analysis allowed to gather comprehensive information on SARS-CoV-2 epidemiology in our area, highlighting positivity variations over time and in different sex and age group and the need for a continuous surveillance of the infection, mostly because the pandemic evolution remains unknown
Beyond appearance: An unusual manifestation of isolated oral secondary syphilis
Syphilis is a sexually acquired chronic infection caused by Treponema pallidum and is characterized by a variety of clinical manifestations. The secondary stage of the disease results from the hematogenous and lymphatic dissemination of treponemes after a few weeks or months, and it is characterized by recurrent activity of the disease, with muco-cutaneous as well as systemic manifestations. Mucosal lesions range from small, superficial ulcers that resemble painless aphthae to large gray plaques, and they are generally associated with systemic manifestations of the disease. The exclusive asymptomatic oral localization not associated with general manifestations is uncommon but may actually be unrecognized and under-reported. We report a case of isolated oral manifestation as the unique presentation of secondary syphilis
