National Prevalence and Exposure Risk for Cockroach Allergen in U.S. Households
We characterized the prevalence of cockroach allergen exposure in a nationally representative sample of U.S. homes and assessed risk factors for elevated concentrations. DESIGN: We used data from the National Survey of Lead and Allergens in Housing, a population-based cross-sectional survey. PARTICIPANTS: Participants were residents of 831 U.S. homes in the survey. EVALUATIONS/MEASUREMENTS: We analyzed allergen, questionnaire, and observational data of 831 U.S. homes. RESULTS: Cockroach allergen (Bla g 1) concentrations exceed 2.0 U/g, a level associated with allergic sensitization, in 11% of U.S. living room floors and 13% of kitchen floors. Concentrations exceed 8.0 U/g, a level associated with asthma morbidity, in 3% of living room floors and 10% of kitchen floors. Elevated concentrations were observed in high-rise apartments, urban settings, pre-1940 constructions, and households with incomes < $20,000. Odds of having concentrations > 8.0 U/g were greatest when roach problems were reported or observed and increased with the number of cockroaches observed and with indications of recent cockroach activity. CONCLUSIONS: Household cockroach allergen exposure is characterized in a nationally representative context. The allergen is prevalent in many settings, at levels that may contribute to allergic sensitization and asthma morbidity. RELEVANCE TO CLINICAL OR PROFESSIONAL PRACTICE: Likelihood of exposure can be assessed by consideration of demographic and household determinants
Enabling Process Variants and Versions in Distributed Object-Aware Process Management Systems
Business process variants are common in many enterprises and properly managing them is indispensable. Some process management suites already offer features to tackle the challenges of creating and updating multiple variants of a process. As opposed to the widespread activity-centric process modeling paradigm, however, there is little to no support for process variants in other process support paradigms, such as the recently proposed artifact-centric or object-aware process support paradigm. This paper presents concepts for supporting process variants in the object-aware process management paradigm. We offer insights into the distributed object-aware process management framework PHILharmonicFlows as well as the concepts it provides for implementing variants and versioning support based on log propagation and log replay. Finally, we examine the challenges that arise from the support of process variants and show how we solved these, thereby enabling future research into related fundamental aspects to further raise the maturity level of data-centric process support paradigms
Gas inflows in the polar ring of NGC 4111: the birth of an AGN
We have used Hubble Space Telescope (HST) images, SAURON Integral Field
Spectroscopy (IFS) and adaptative optics assisted Gemini NIFS near-infrared
K-band IFS to map the stellar and gas distribution, excitation and kinematics
of the inner few kpc of the nearby edge-on S0 galaxy NGC 4111. The HST images
map its 450 pc diameter dusty polar ring, with an estimated gas mass
M. The NIFS datacube maps the inner 110 pc radius at
7 pc spatial resolution revealing a 220 pc diameter polar
ring in hot ( K) molecular H 1-0 S(1) gas embedded in the polar
ring. The stellar velocity field shows disk-dominated kinematics along the
galaxy plane both in the SAURON large-scale and in the NIFS nuclear-scale data.
The large-scale [O III] \AA velocity field shows a superposition
of two disk kinematics: one similar to that of the stars and another along the
polar ring, showing non-circular motions that seem to connect with the velocity
field of the nuclear H ring, whose kinematics indicate accelerated inflow
to the nucleus. The estimated mass inflow rate is enough not only to feed an
Active Galactic Nucleus (AGN) but also to trigger circumnuclear star formation
in the near future. We propose a scenario in which gas from the polar ring,
which probably originated from the capture of a dwarf galaxy, is moving inwards
and triggering an AGN, as supported by the local X-ray emission, which seems to
be the source of the H 1-0 S(1) excitation. The fact that we see neither
near-UV nor Br emission suggests that the nascent AGN is still deeply
buried under the optically thick dust of the polar ring.Comment: 18 pages, 21 figure
Gravitational Lensing
Gravitational lensing has developed into one of the most powerful tools for
the analysis of the dark universe. This review summarises the theory of
gravitational lensing, its main current applications and representative results
achieved so far. It has two parts. In the first, starting from the equation of
geodesic deviation, the equations of thin and extended gravitational lensing
are derived. In the second, gravitational lensing by stars and planets,
galaxies, galaxy clusters and large-scale structures is discussed and
summarised.Comment: Invited review article to appear in Classical and Quantum Gravity, 85

Nitrogen dioxide and allergic sensitization in the 2005–2006 National Health and Nutrition Examination Survey
Allergic sensitization is a risk factor for asthma and allergic diseases. The relationship between ambient air pollution and allergic sensitization is unclear
Nonsteroidal Anti-inflammatory Drugs and Endometrial Carcinoma Mortality and Recurrence
Background: Recent data suggest that the use of nonsteroidal anti-inflammatory drugs (NSAIDs) may be associated with reductions in endometrial cancer risk, yet very few have examined whether their use is related to prognosis among endometrial cancer patients
Period and Cohort Changes in Americans’ Support for Marijuana Legalization: Convergence and Divergence across Social Groups
We cast fresh light on how and why Americans’ views on marijuana legalization shifted between 1973 and 2014. Results from age-period-cohort models show a strong negative effect of age and relatively high levels of support for legalization among baby boom cohorts. Despite the baby boom effect, the large increase in support for marijuana legalization is predominantly a broad, period-based change in the population. Additional analyses demonstrate that differences in support for legalization by education, region, and religion decline, that differences by political party increase, and that differences between whites and African Americans reverse direction. We conclude by discussing the implications of these findings and by identifying promising directions for future research on this topic
Rationale and design of the Multidisciplinary Approach to Novel Therapies in Cardiology Oncology Research Trial (MANTICORE 101 - Breast): a randomized, placebo-controlled trial to determine if conventional heart failure pharmacotherapy can prevent trastuzumab-mediated left ventricular remodeling among patients with HER2+ early breast cancer using cardiac MRI

Abstract

Background

MANTICORE 101 - Breast (Multidisciplinary Approach to Novel Therapies in Cardiology Oncology Research) is a randomized trial to determine if conventional heart failure pharmacotherapy (angiotensin converting enzyme inhibitor or beta-blocker) can prevent trastuzumab-mediated left ventricular remodeling, measured with cardiac MRI, among patients with HER2+ early breast cancer.

Methods/Design

One hundred and fifty-nine patients with histologically confirmed HER2+ breast cancer will be enrolled in a parallel 3-arm, randomized, placebo controlled, double-blind design. After baseline assessments, participants will be randomized in a 1:1:1 ratio to an angiotensin-converting enzyme inhibitor (perindopril), beta-blocker (bisoprolol), or placebo. Participants will receive drug or placebo for 1 year beginning 7 days before trastuzumab therapy. Dosages for all groups will be systematically up-titrated, as tolerated, at 1 week intervals for a total of 3 weeks. The primary objective of this randomized clinical trial is to determine if conventional heart failure pharmacotherapy can prevent trastuzumab-mediated left ventricular remodeling among patients with HER2+ early breast cancer, as measured by 12 month change in left ventricular end-diastolic volume using cardiac MRI. Secondary objectives include 1) determine the evolution of left ventricular remodeling on cardiac MRI in patients with HER2+ early breast cancer, 2) understand the mechanism of trastuzumab mediated cardiac toxicity by assessing for the presence of myocardial injury and apoptosis on serum biomarkers and cardiac MRI, and 3) correlate cardiac biomarkers of myocyte injury and extra-cellular matrix remodeling with left ventricular remodeling on cardiac MRI in patients with HER2+ early breast cancer.

Discussion

Cardiac toxicity as a result of cancer therapies is now recognized as a significant health problem of increasing prevalence. To our knowledge, MANTICORE will be the first randomized trial testing proven heart failure pharmacotherapy in the prevention of trastuzumab-mediated cardiotoxicity. We expect the findings of this trial to provide important evidence in the development of guidelines for preventive therapy.

Trial Registration

ClinicalTrials.gov: NCT01016886</p
