    Genetic Algorithm-assisted Maximum-Likelihood Multi-user Detection for Multi-Rate MC-CDMA Systems

    In this letter , the use of Genetic Algorithms (GAs) has been investigated in order to implement a computationally tractable Multi-User Detection (MUD) for synchronous multi-rate Multi-Carrier-Code-Division-Multiple-Access (MR MC-CDMA) systems. It is known that MC-CDMA performances can be seriously limited by multi-user interference. This is particularly true in the case of multi-rate systems where some users could exploit a reduced diversity against multipath channel distortions. The use of Genetic Algorithms (GAs) is therefore proposed as a sub-optimal solution to the MUD problem in a multi-rate Variable-Spreading-Length (VSL) MC-CDMA downlink transmission. A set of selected numerical experiments have been reported and discussed in order to assess the capability of the proposed GA-based approach. The reported results, obtained with an affordable computational burden, have shown a near-optimum behavior of the proposed GA-based MUD

    China–Turkey Relations from the Perspective of Neoclassical Realism

    There is plenty of studies focusing on China’s global outreach through its Belt and Road Initiative (BRI). In tandem with this, the extensive literature on China depicts it as the next hegemon to succeed in the USA. Along this line, flourishing ties with various Asian nations, including the Middle Eastern countries, as a result of China’s recent foreign policy activism has been addressed extensively. While most research has been stressing the rising assertiveness of China in world politics, only a limited number of studies have touched upon the responses from middle or small powers against China’s ascent. Drawing from neoclassical realism, this article contends two levels of analysis for delineating the interaction between Turkey, a middle power, and China, a rising great power. First, the exchange between Turkey and the USA is vital in determining the cordial relations between Turkey and China. Alteration in the American policy vis-à-vis Turkey in the wake of the Arab Spring is relevant to Turkey’s growing relations with China. Second, is the rising anti-Westernism of foreign policy elites as part of the alteration in the strategic culture of Turkish politics, which makes Turkey’s rapprochement with China possible. Nevertheless, it should be noted that these two levels are intertwined and feed each other. Consequently, employing a neoclassical realist approach, the article argues that the middle powers’ stance against a rising hegemon is conditional upon the bilateral relations with the current hegemon and peculiarities of domestic politics

    Single-Step IGHV Next-Generation Sequencing Detects Clonality and Somatic Hypermutation in Lymphoid Malignancies: A Phase III Diagnostic Accuracy Study

    Background: Multiplex PCR based on consensus primers followed by capillary electrophoresis and Sanger sequencing are considered as the gold standard method for the evaluation of clonality and somatic hypermutation in lymphoid malignancies. As an alternative, the next-generation sequencing (NGS) of immune receptor genes has recently been proposed as a solution, due to being highly effective and sensitive. Here, we designed a phase III diagnostic accuracy study intended to compare the current gold standard methods versus the first commercially available NGS approaches for testing immunoglobulin heavy chain gene rearrangements. Methods: We assessed IGH rearrangements in 68 samples by means of both the NGS approach (LymphoTrack® IGH assay, and LymphoTrack® IGH somatic hypermutation assay, run on Illumina MiSeq) and capillary electrophoresis/Sanger sequencing to assess clonality and somatic hypermutations (SHM). Results: In comparison to the routine capillary-based analysis, the NGS clonality assay had an overall diagnostic accuracy of 96% (63/66 cases). Other studied criteria included sensitivity (95%), specificity (100%), positive predictive value (100%) and negative predictive value (75%). In discrepant cases, the NGS results were confirmed by a different set of primers that provided coverage of the IGH leader sequence. Furthermore, there was excellent agreement of the SHM determination with both the LymphoTrack® FR1 and leader assays when compared to the Sanger sequencing analysis (84%), with NGS able to assess the SHM rate even in cases where the conventional approach failed. Conclusion: Overall, conventional Sanger sequencing and next-generation-sequencing-based clonality and somatic hypermutation analyses gave comparable results. For future use in a routine diagnostic workflow, NGS-based approaches should be evaluated prospectively and an analysis of cost-effectiveness should be performed

    Autologous hematopoietic stem cell transplantation in multiple sclerosis: a phase II trial

    Objective: To assess in multiple sclerosis (MS) the effect of intense immunosuppression followed by autologous hematopoietic stem cells transplantation (AHSCT) vs mitoxantrone (MTX) on disease activity measured by MRI. Methods: We conducted a multicenter, phase II, randomized trial including patients with secondary progressive or relapsing-remitting MS, with a documented increase in the last year on the Expanded Disability Status Scale, in spite of conventional therapy, and presence of one or more gadolinium-enhancing (Gd+) areas. Patients were randomized to receive intense immunosuppression (mobilization with cyclophosphamide and filgrastim, conditioning with carmustine, cytosine-arabinoside, etoposide, melphalan, and anti-thymocyte globulin) followed by AHSCT or MTX 20 mg every month for 6 months. The primary endpoint was the cumulative number of new T2 lesions in the 4 years following randomization. Secondary endpoints were the cumulative number of Gd+ lesions, relapse rate, and disability progression. Safety and tolerability were also assessed. Twenty-one patients were randomized and 17 had postbaseline evaluable MRI scans. Results: AHSCT reduced by 79% the number of new T2 lesions as compared to MTX (rate ratio 0.21, p 0.00016). It also reduced Gd+ lesions as well as the annualized relapse rate. No difference was found in the progression of disability. Conclusion: Intense immunosuppression followed by AHSCT is significantly superior to MTX in reducing MRI activity in severe cases of MS. These results strongly support further phase III studies with primary clinical endpoints. The study was registered as EUDRACT No. 2007-000064-24
