    A CNO Dichotomy among O2 Giant Spectra in the Magellanic Clouds

    From a survey of the 3400 Å region in the earliest O-type spectra, we have found that two of the four O2 giants observed in the Large Magellanic Cloud have O IV lines there that are stronger than the N IV lines, while the other two have the opposite. A Small Magellanic Cloud counterpart also has N IV stronger than O IV. Inspection of the blue spectra of these stars shows that the former pair have weaker N lines in all ionization states (III, IV, and V) present as well as lines of C IV λ4658, while the latter three have stronger N lines and greater He/H. Space ultraviolet observations of two of the N-strong stars show N V wind profiles substantially stronger than those of C IV, while in the N-weak stars the C IV features are equal to or stronger than the N V. The N-strong stars are now reclassified as ON2 III(f*), newly defining that category. These characteristics strongly suggest a larger fraction of processed material in the atmospheres of the ON2 stars, which we confirm by modeling the optical spectra. In the context of current models, it is in turn implied that the ON2 stars are in a more advanced evolutionary state than the others, and/or that they had higher initial rotational velocities. The recent formulation of the effects of rotation on massive stellar evolution introduces an additional fundamental parameter, which the CNO abundances are in principle able to constrain. We present some illustrative comparisons with current Geneva evolutionary models for rotating massive stars. It is possible that these very hot, nitrogen-rich objects are products of homogeneous evolution. Our results will provide motivation for further physical modeling of the atmospheres and evolutionary histories of the most massive hot stars.Fil: Walborn, Nolan Revere.     Intermediate range chemical ordering of cations in simple molten alkali halides

    The presence of first sharp diffraction peaks in the partial structure factors is investigated in computer simulations of molten mixtures of alkali halides. An intermediate range ordering appears for the Li+ ions only, which is associated with clustering of this species and is not reflected in the arrangement of other ions. This ordering is surprising in view of the simplicity of the interionic interactions in alkali halides. The clustering reflects an incomplete mixing of the various species on a local length scale, which can be demonstrated by studying the complementary sub-space of cations in the corresponding pure alkali halides by means of a void analysis.Comment: 5 pages, 5 figure

    A three-decade outburst of the LMC luminous blue variable R127 draws to a close

    The paradigmatic luminous blue variable R127 in the Large Magellanic Cloud has been found in the intermediate, peculiar early-B state, and substantially fainter in visual light, signaling the final decline from its major outburst that began between 1978 and 1980. This transformation was detected in 2008 January, but archival data show that it began between early 2005 and early 2007. In fact, significant changes from the maximum, peculiar A-type spectrum, which was maintained from 1986 through 1998, had already begun the following year, coinciding with a steep drop in visual light. We show detailed correspondences between the spectrum and light, in which the decline mimics the rise. Moreover, these trends are not monotonic but are characterized by multiple spikes and dips, which may provide constraints on the unknown outburst mechanism.     Does the process map influence the outcome of quality improvement work? A comparison of a sequential flow diagram and a hierarchical task analysis diagram

    Background: Many quality and safety improvement methods in healthcare rely on a complete and accurate map of the process. Process mapping in healthcare is often achieved using a sequential flow diagram, but there is little guidance available in the literature about the most effective type of process map to use. Moreover there is evidence that the organisation of information in an external representation affects reasoning and decision making. This exploratory study examined whether the type of process map - sequential or hierarchical - affects healthcare practitioners' judgments.Methods: A sequential and a hierarchical process map of a community-based anti coagulation clinic were produced based on data obtained from interviews, talk-throughs, attendance at a training session and examination of protocols and policies. Clinic practitioners were asked to specify the parts of the process that they judged to contain quality and safety concerns. The process maps were then shown to them in counter-balanced order and they were asked to circle on the diagrams the parts of the process where they had the greatest quality and safety concerns. A structured interview was then conducted, in which they were asked about various aspects of the diagrams.Results: Quality and safety concerns cited by practitioners differed depending on whether they were or were not looking at a process map, and whether they were looking at a sequential diagram or a hierarchical diagram. More concerns were identified using the hierarchical diagram compared with the sequential diagram and more concerns were identified in relation to clinical work than administrative work. Participants' preference for the sequential or hierarchical diagram depended on the context in which they would be using it. The difficulties of determining the boundaries for the analysis and the granularity required were highlighted.Conclusions: The results indicated that the layout of a process map does influence perceptions of quality and safety problems in a process. In quality improvement work it is important to carefully consider the type of process map to be used and to consider using more than one map to ensure that different aspects of the process are captured

    A new spectral classification system for the earliest O stars: Definition of type O2

    High-quality, blue-violet spectroscopic data are collected for 24 stars that have been classified as type O3 and that display the hallmark N Iv and N v lines, A new member of the class is presented; it is the second known in the Cyg OB2 association, and only the second in the northern hemisphere. New digital data are also presented for several of the other stars. Although the data are inhomogeneous, the uniform plots by subcategory reveal some interesting new relationships. Several issues concerning the classification of the hottest O-type spectra are discussed, and new digital data are presented for the five original O3 dwarfs in the Carina Nebula, in which the N Iv, N v features are very weak or absent. New spectral types O2 and O3.5 are introduced here as steps toward resolving these issues. The relationship between the derived absolute visual magnitudes and the spectroscopic luminosity classes of the O2-O3 stars shows more scatter than at later O types, at least partly because some overluminous dwarfs are unresolved multiple systems, and some close binary systems of relatively low luminosity and mass emulate O3 supergiant spectra. However, it also appears that the behavior of He II λ4686, the primary luminosity criterion at later O types, responds to other phenomena in addition to luminosity at spectral types O2-O3. There is evidence that these spectral types may correspond to an immediate pre-WN phase, with a correspondingly large range of luminosities and masses. A complete census of spectra classified into the original O3 subcategories considered here (not including intermediate O3/WN types or O3 dwarfs without N Iv, N v features) totals 45 stars; 34 of them belong to the Large Magellanic Cloud and 20 of the latter to 30 Doradus.Facultad de Ciencias Astronómicas y Geofísica

    Chagas Disease in the New York City Metropolitan Area.

    Background:Chagas disease, caused by the parasite Trypanosoma cruzi, once considered a disease confined to Mexico, Central America, and South America, is now an emerging global public health problem. An estimated 300 000 immigrants in the United States are chronically infected with T. cruzi. However, awareness of Chagas disease among the medical community in the United States is poor. Methods:We review our experience managing 60 patients with Chagas disease in hospitals throughout the New York City metropolitan area and describe screening, clinical manifestations, EKG findings, imaging, and treatment. Results:The most common country of origin of our patients was El Salvador (n = 24, 40%), and the most common detection method was by routine blood donor screening (n = 21, 35%). Nearly half of the patients were asymptomatic (n = 29, 48%). Twenty-seven patients were treated with either benznidazole or nifurtimox, of whom 7 did not complete therapy due to side effects or were lost to follow-up. Ten patients had advanced heart failure requiring device implantation or organ transplantation. Conclusions:Based on our experience, we recommend that targeted screening be used to identify at-risk, asymptomatic patients before progression to clinical disease. Evaluation should include an electrocardiogram, echocardiogram, and chest x-ray, as well as gastrointestinal imaging if relevant symptoms are present. Patients should be treated if appropriate, but providers should be aware of adverse effects that may prevent patients from completing treatment
