    Real-time detection and data acquisition system for the left ventricular outline

    To automate the data acquisition procedure, a real-time contour detection and data acquisition system for the left ventricular outline was developed using video techniques. The X-ray image of the contrast-filled left ventricle is stored for subsequent processing on film (cineangiogram), video tape or disc. The cineangiogram is converted into video format using a television camera. The video signal from either the TV camera, video tape or disc is the input signal to the system. The contour detection is based on a dynamic thresholding technique. Since the left ventricular outline is a smooth continuous function, for each contour side a narrow expectation window is defined in which the next borderpoint will be detected. A computer interface was designed and built for the online acquisition of the coordinates using a PDP-12 computer. The advantage of this system over other available systems is its potential for online, real-time acquisition of the left ventricular size and shape during angiocardiography

    Dedicated bifurcation analysis: basic principles

    Over the last several years significant interest has arisen in bifurcation stenting, in particular stimulated by the European Bifurcation Club. Traditional straight vessel analysis by QCA does not satisfy the requirements for such complex morphologies anymore. To come up with practical solutions, we have developed two models, a Y-shape and a T-shape model, suitable for bifurcation QCA analysis depending on the specific anatomy of the coronary bifurcation. The principles of these models are described in this paper, as well as the results of validation studies carried out on clinical materials. It can be concluded that the accuracy, precision and applicability of these new bifurcation analyses are conform the general guidelines that have been set many years ago for conventional QCA-analyses

    New approaches for the assessment of vessel sizes in quantitative (cardio-)vascular X-ray analysis

    This paper presents new approaches for the assessment of the arterial and reference diameters in (cardio-)vascular X-ray images, designed to overcome the problems experienced in conventional quantitative coronary and vascular angiography approaches. In single or “straight” vessel segments, the arterial and reference diameter directions were made independent of each other in order to be able to measure the minimal lumen diameter (MLD) more accurately, especially in curved vessel segments. For ostial segments, an extension of this approach was used, to allow measurement of ostial lesions in sidebranches more proximal than using conventional methods. Furthermore, two new bifurcation approaches were developed. The validation study shows that the straight segment approach results in significant smaller MLDs (on average 0.032 mm) and the ostial approach achieves on average an increase in %DS of 3.8% and an increase in lesion length of 0.59 mm due to loosening the directional constraint. The validation of our new bifurcation approaches in phantom data as well as clinical data shows only small differences between pre- and post-intervention measurements of the reference diameters outside the bifurcation core (errors smaller than 0.06 mm) and the bifurcation core area (errors smaller than 1.4% for phantom data). In summary, these new approaches have led to further improvements in the quantitative analyses of (cardio-)vascular X-ray angiographies

    Effects of the mixed phosphodiesterase III/IV inhibitor, zardaverine, on airway function in patients with chronic airflow obstruction

    AbstractZardaverine is a selective inhibitor of phosphodiesterase (PDE) III and IV isozymes. It has been shown to exert potent bronchodilator effects in animals. In order to study the efficacy and safety in man, a phase II clinical trial in 10 patients with partially reversible chronic airflow obstruction was carried out. The trial was designed as a double-blind, randomized, five-period change-over study. Zardaverine (at single doses of 1·5 mg, 3·0 mg, or 6·0 mg), salbutamol (0·3 mg) and placebo were administered by metered dose inhaler on separate days. As evaluated by spirometry over a time period of 4 h, salbutamol induced a significant bronchodilatation. In contrast, zardaverine did not improve airway function in these patients. Unwanted effects of the study medication were not observed

    Three-dimensional reconstruction of myocardial contrast perfusion from biplane cineangiograms by means of linear programming techniques

    The assessment of coronary flow reserve from the instantaneous distribution of the contrast agent within the coronary vessels and myocardial muscle at the control state and at maximal flow has been limited by the superimposition of myocardial regions of interest in the two-dimensional images. To overcome these limitations, we are in the process of developing a three-dimensional (3D) reconstruction technique to compute the contrast distribution in cross sections of the myocardial muscle from two orthogonal cineangiograms. To limit the number of feasible solutions in the 3D-reconstruction space, the 3D-geometry of the endo- and epicardial boundaries of the myocardium must be determined. For the geometric reconstruction of the epicardium, the centerlines of the left coronary arterial tree are manually or automatically traced in the biplane views. Next, the bifurcations are detected automatically and matched in these two views, allowing a 3D-representation of the coronary tree. Finally, the circumference of the left ventricular myocardium in a selected cross section can be computed from the intersection points of this cross section with the 3D coronary tree using B-splines. For the geometric reconstruction of the left ventricular cavity, we envision to apply the elliptical approximation technique using the LV boundaries defined in the two orthogonal views, or by applying more complex 3D-reconstruction techniques including densitometry. The actual 3D-reconstruction of the contrast distribution in the myocardium is based on a linear programming technique (Transportation model) using cost coefficient matrices. Such a cost coefficient matrix must contain a maximum amount of a priori information, provided by a computer generated model and updated with actual data from the angiographic views. We have only begun to solve this complex problem. However, some studies showed that the bronchial membrane is distensible with airway inflation. Therefore, re-examination of the elasticity of bronchi is needed.PurposeTo assess the influence of respiration on bronchial lumen area (defined as distensibility) in different segmental bronchi and to explore the correlations between distensibility and both lung function and emphysema severity.Material and methodsIn 44 subjects with COPD related to alpha-1-antitrypsin deficiency (AATD), bronchial lumen area was measured in CT images, acquired at different inspiration levels. Measurements were done at matched locations in one apical and two basal segmental airways (RB1, RB10 and LB10). Airway distensibility was calculated as lumen area difference divided by lung volume difference.ResultsBronchial lumen area in the lower lobes (RB10 and LB10) correlated positively with FEV1%predicted (p=0.027 for RB10; and p=0.037 for LB10, respectively). Lumen area is influenced by respiration (p=0.006, p=0.045, and, p=0.005 for RB1, RB10 and LB10, respectively). Airway distensibility was different between upper and lower bronchi (p<0.001), but it was not correlated with lung function.ConclusionLumen area of third generation bronchi is dependent on inspiration level and this distensibility is different between bronchi in the upper and lower lobes. Therefore, changes in lumen area over time should be studied whilst accounting for the lung volume changes, in order to estimate the progression of bronchial disease while excluding the effects of hyperinflation

    Food Security in Crop, Livestock and Mixed Farming Systems in Mali

    This study evaluates the food security status, its determinants and the coping strategies in crop, livestock and mixed crop-livestock systems in southern Mali. Interviews were conducted with 258 households that were categorized into the three farming systems based on the revenue from livestock and crop production. A linear mixed model was used to analyse the effects of household characteristics on food security using the food consumption score (FCS), household dietary diversity score (HDDS), a modified household food insecurity access scale (mHFIAS) and coping strategies as indicators. Food consumption score was significantly influenced by the farming system with highest FCS for the livestock system (88) followed by the mixed system (77) and the crop system (69). Moreover, FCS was positively influenced by the number of crops cultivated, total farm milk production, off-farm income and number of raised chickens (p \u3c 0.1). The main difference in food intake between systems was for milk with average daily consumption of 201 ml, 110 ml and 60 ml in the livestock, mixed and crop systems, respectively. HDDS was also significantly influenced by the farming system with highest HDDS values for the livestock system (8.9), followed by the mixed system (8.2) and the crop system (8.0). Further, HDDS was positively influenced by crop diversity and number of chickens, and negatively influenced by the number of family dependents. During the food shortage period of August, households from the crop system were more food insecure than households from the mixed and livestock systems with mHFIAS values of 0.9, 1.5 and 4.3, respectively. Livestock and mixed systems were more resilient to food insecurity situations than the crop system. This study confirms the direct effect of milk production on milk consumption and the importance of livestock for enhancing food security and livelihood resilience

    Optimising contributions of goat farming to household economic success and food security in three production systems in Ethiopia

    The study aims to analyse factors affecting contributions of goat farming to household economic success and food security in three goat production systems of Ethiopia. A study was conducted in three districts of Ethiopia representing arid agro-pastoral (AAP), semi-arid agro-pastoral (SAAP) and highland mixed crop-livestock (HMCL) systems involving 180 goat keeping households. Gross margin (GM) and net benefit (NB1 and NB2) were used as indicators of economic success of goat keeping. NB1 includes in-kind benefits of goats (consumption and manure), while NB2 additionally constitutes intangible benefits (insurance and finance). Household dietary diversity score (HDDS) was used as a proxy indicator of food security. GM was significantly affected by an off-take rate and flock size interaction (P0.001). The increment of GM due to increased off-take rate was more prominent for farmers with bigger flocks. Interaction between flock size and production system significantly (P0.001) affected both NB1 and NB2. The increment of NB1 and NB2 by keeping larger flocks was higher in AAP system, due to higher in-kind and intangible benefits of goats in this system. Effect of goat flock size as a predictor of household dietary diversity was not significant (P0.05). Nevertheless, a significant positive correlation (P0.05) was observed between GM from goats and HDDS in AAP system, indicating the indirect role of goat production for food security. The study indicated that extent of utilising tangible and intangible benefits of goats varied among production systems and these differences should be given adequate attention in designing genetic improvement programs
