    Speech-language cooperation protocol for the fiberoptic laryngoscopy evaluation of larynx mobility

    TEMA: protocolo de avaliação da voz. OBJETIVO: proposição de um protocolo de cooperação fonoaudiológica para avaliação nasofibrolaringoscópica da mobilidade laríngea em doenças da tireóide (PAN), visando a composição de um instrumento objetivo, preciso e consensual para avaliação. MÉTODOS: a primeira versão do protocolo foi elaborada a partir de fundamentação bibliográfica; o PAN foi julgado em duas instâncias pelo método de triangulação por seis juízes em três etapas; foi constituída uma versão piloto do protocolo e aplicada em 11 pacientes; houve novo julgamento de médicos e fonoaudiólogos; a partir da concordância dos juízes, após a aplicação do piloto, foi construída a versão final do PAN. RESULTADOS: o PAN final foi composto por duas partes. A primeira parte considerada o procedimento padrão composta por 4 itens imprescindíveis e que devem necessariamente ser avaliados são: inspiração normal; inspiração forçada; vogal /é/ isolada e sustentada e vogal /i/ aguda isolada e sustentada. A segunda parte considerada de complementação fonoaudiológica é composta pelos itens que são entendidos pelos fonoaudiólogos como fatores informativos ou preditivos para a eficácia da terapia. Esses itens são: vogal /é/ sustentada e fraca; vogal /é/ sustentada e aguda; vogal /é/ sustentada e grave; vogal /é/ curta com ataque vocal brusco. CONCLUSÕES: o PAN, em sua versão final, contribui para a sistematização dos procedimentos de avaliação fundamentados em evidências e concordâncias profissionais. O PAN resulta na descrição de itens a serem solicitados durante a avaliação médica e fonoaudiológica no exame de nasofibrolaringoscopia da alteração da mobilidade laríngea em doenças da tireóide.BACKGROUND: voice protocol. AIM: to propose a protocol for the fiberoptic laryngoscopy evaluation of larynx mobility in thyroid illnesses (PAN), with the intention of having an objective, precise and consensual instrument for this assessment. METHOD: the first version of the protocol was elaborated based on data found in the literature; the protocol was judged twice, using the triangulation method; a pilot version was presented and applied in 11 patients; it was then judged again by doctors and speech-language pathologists; based on the analysis of the judges and after the application of the pilot version, the final version of the PAN was proposed. RESULTS: the final protocol was composed by two parts. The first part, considered a standard procedure, is composed by 4 essential items that necessarily should be evaluated: normal inspiration; forced inspiration; vowel /é/ isolated and sustained; and sharp vowel /i/, isolated and sustained. The second part, considered a speech-language complementation, is composed by items that should be understood as being important for speech-language pathologists as they are informative or predictive of the effectiveness of therapy: vowel /é/ sustained and weak; vowel /é/ sustained and sharp; vowel /é/ sustained and deep; vowel /é/ short with abrupt vocal onset. CONCLUSIONS: the PAN, in its final version, contributes for the systematization of the assessment procedures based on evidence and on the agreement of professionals.     Toxoplasmosis is frequently acquired through the oral route by the ingestion of cysts or oocysts of Toxoplasma gondii. Once ingested, the parasites penetrate the intestinal epithelial cells and rapidly disseminate to all organs in the host. During T. gondii infection, the intestinal microbiota plays an important role in stimulating a protective immune response against the parasite. In this sense the use of probiotics is worthy of note since they are live microorganisms that have beneficial effects on the host through stimulation of the immune response that can be important in the control of T. gondii proliferation and dissemination in the host. In the present study, the action of the probiotic Bifidobacterium animalis subsp. lactis was investigated in C57BL/6 mice infected with oocysts of ME49 strain of T. gondii. The probiotic had an immunomodulatory action, inducing CD19 lymphocyte proliferation and consequently increasing anti-T. gondii antibody level.Bifidobacterium animalis subsp. lactisprovided protection in supplemented mice, compared to the control group. In addition, supplemented animals had milder inflammatory process in the small intestine, indicating that the probiotic protects the intestinal mucosa during infection with T. gondii. It was concluded that the probioticB. animalis subsp. lactis induces humoral immune response capable of providing protection against T. gondii infection

    Fatores associados à amamentação na primeira hora de vida: revisão sistemática

    OBJECTIVE To identify independent risk factors for non-breastfeeding within the first hour of life. METHODS A systematic review of Medline, LILACS, Scopus, and Web of Science electronic databases, till August 30, 2013, was performed without restrictions on language or date of publishing. Studies that used regression models and provided adjusted measures of association were included. Studies in which the regression model was not specified or those based on specific populations regarding age or the presence of morbidities were excluded. RESULTS The search resulted in 155 articles, from which 18 met the inclusion criteria. These were conducted in Asia (9), Africa (5), and South America (4), between 1999 and 2013. The prevalence of breastfeeding within the first hour of life ranged from 11.4%, in a province of Saudi Arabia, to 83.3% in Sri Lanka. Cesarean delivery was the most consistent risk factor for non-breastfeeding within the first hour of life. “Low family income”, “maternal age less than 25 years”, “low maternal education”, “no prenatal visit”, “home delivery”, “no prenatal guidance on breastfeeding” and “preterm birth” were reported as risk factors in at least two studies. CONCLUSIONS Besides the hospital routines, indicators for low socioeconomic status and poor access to health services were also identified as independent risk factors for non-breastfeeding within the first hour of life. Policies to promote breastfeeding, appropriate to each context, should aim to reduce inequalities in health.OBJETIVO Identificar fatores de risco independentes para a não amamentação na primeira hora de vida. MÉTODOS Revisão sistemática nas bases de dados Medline, Lilacs, Scopus e Web of Science, sem restrição de idioma ou período de publicação, até 30 de agosto de 2013. Foram incluídos estudos que utilizaram modelos de regressão e forneceram medidas de associação ajustadas. Foram excluídos artigos que não especificaram o modelo de regressão utilizado ou que abordaram populações específicas quanto à faixa etária ou presença de morbidade. RESULTADOS Foram identificados 155 artigos, dos quais 18 preencheram os critérios de inclusão. Foram realizados na Ásia (nove), África (cinco) e América do Sul (quatro) entre 1999 e 2010. A prevalência da amamentação na primeira hora de vida variou de 11,4%, em uma província da Arábia Saudita, a 83,3% no Sri Lanka. A cesariana foi o fator de risco mais consistente para a não amamentação na primeira hora de vida. “Baixa renda familiar”, “idade materna menor que 25 anos”, “baixa escolaridade materna”, “ausência de consultas pré-natais”, “parto domiciliar”, “falta de orientação sobre amamentação no pré-natal” e “prematuridade” foram fatores de risco identificados em pelo menos dois estudos. CONCLUSÕES Além de rotinas hospitalares, indicadores associados a pior nível socioeconômico e menor acesso a serviços de saúde foram também identificados como fatores de risco independentes para a não amamentação na primeira hora de vida. Políticas de promoção da amamentação, adequadas a cada contexto, devem ter como meta a redução das desigualdades em saúde

    Coinfection with Different Trypanosoma cruzi Strains Interferes with the Host Immune Response to Infection

    A century after the discovery of Trypanosoma cruzi in a child living in Lassance, Minas Gerais, Brazil in 1909, many uncertainties remain with respect to factors determining the pathogenesis of Chagas disease (CD). Herein, we simultaneously investigate the contribution of both host and parasite factors during acute phase of infection in BALB/c mice infected with the JG and/or CL Brener T. cruzi strains. JG single infected mice presented reduced parasitemia and heart parasitism, no mortality, levels of pro-inflammatory mediators (TNF-α, CCL2, IL-6 and IFN-γ) similar to those found among naïve animals and no clinical manifestations of disease. On the other hand, CL Brener single infected mice presented higher parasitemia and heart parasitism, as well as an increased systemic release of pro-inflammatory mediators and higher mortality probably due to a toxic shock-like systemic inflammatory response. Interestingly, coinfection with JG and CL Brener strains resulted in intermediate parasitemia, heart parasitism and mortality. This was accompanied by an increase in the systemic release of IL-10 with a parallel increase in the number of MAC-3+ and CD4+ T spleen cells expressing IL-10. Therefore, the endogenous production of IL-10 elicited by coinfection seems to be crucial to counterregulate the potentially lethal effects triggered by systemic release of pro-inflammatory mediators induced by CL Brener single infection. In conclusion, our results suggest that the composition of the infecting parasite population plays a role in the host response to T. cruzi in determining the severity of the disease in experimentally infected BALB/c mice. The combination of JG and CL Brener was able to trigger both protective inflammatory immunity and regulatory immune mechanisms that attenuate damage caused by inflammation and disease severity in BALB/c mice

    Objective: This study aims to analyze the efficacy, safety, and applicability of lidocaine as an adjuvant in epidural anesthesia for cesarean sections, considering different protocols and their impacts on clinical parameters.  Introduction: Lidocaine as an adjuvant in epidural anesthesia for cesarean sections plays a crucial role in obstetric pain management. This review encompasses its efficacy, safety, and impacts on various clinical aspects, aiming to enhance contemporary anesthetic practices. Methodology: The methodology involved a systematic integrative review of clinical studies investigating the use of lidocaine as an adjuvant in epidural anesthesia for cesarean sections. The comprehensive analysis considered different administration protocols, dosages, and concentrations, evaluating efficacy, safety, and impacts on diverse clinical parameters. Results: The results consistently revealed the efficacy of lidocaine in epidural analgesia for cesarean sections, with positive impacts on pain relief. A relative safety was also observed, emphasizing the importance of individualizing administration protocols. Conclusion: In summary, lidocaine as an adjuvant in epidural anesthesia for cesarean sections demonstrates remarkable efficacy in pain relief while maintaining a safety profile when administered prudently. Its applicability and versatility reinforce its relevant role in optimizing obstetric anesthetic practices.Objetivo: Este estudo visa analisar a eficácia, segurança e aplicabilidade da lidocaína como adjuvante na anestesia peridural para cesarianas, considerando diferentes protocolos e seus impactos em parâmetros clínicos. Introdução: A lidocaína como adjuvante na anestesia peridural para cesarianas desempenha um papel crucial na gestão da dor obstétrica. Esta revisão abrange sua eficácia, segurança e impactos em diferentes aspectos clínicos, visando aprimorar as práticas anestésicas contemporâneas. Metodologia: A metodologia envolveu a revisão integrativa sistemática de estudos clínicos que investigaram o uso da lidocaína como adjuvante em anestesia peridural para cesarianas. A análise abrangente considerou diferentes protocolos de administração, dosagens e concentrações, avaliando a eficácia, segurança e impactos em parâmetros clínicos diversos. Resultados: Os resultados revelaram consistentemente a eficácia da lidocaína na analgesia peridural para cesarianas, com impactos positivos no alívio da dor. Observou-se também uma relativa segurança, destacando a importância da individualização dos protocolos de administração. Conclusão: Em síntese, a lidocaína como adjuvante na anestesia peridural para cesarianas demonstra eficácia notável no alívio da dor, mantendo um perfil de segurança quando administrada com prudência.
