    Oral semaglutide adequate glycaemia control with safe cardiovascular ‎profile

    BackgroundType 2 diabetes is a chronic and progressive disease that ‎associated with series complication such as major adverse ‎cardiovascular events. Adequate glycaemic control proven ‎to reduce this risk. Orally administered semaglutide ‎promising medication in managing patient with type 2 ‎diabetes.‎AimsTo assess the cardiovascular safety and efficacy of semaglutide, a recently approved glucagon-like peptide 1 receptor agonist (GLP-1 RA) for type 2 diabetes.Methods Pub Med, ‎Google Scholar, and EBSCO ‎ databases were ‎systematically search for relevant articles. The terms‎ diabetes‎, Glucagon-like peptide, semaglutide‎ were used. Out of hundred twenty-two records, only ‎four fulfilled ‎the inclusion criteria.Results Four placebo-controlled studies with oral semaglutide ‎were included. Single study concern about the cardiovascular safety of oral semaglutide ‎and showed that, ‎compared with placebo, semaglutide ‎ was not associated ‎with increased in the cardiovascular events. On the other ‎hand, the remaining trials shown that, semaglutide ‎ can ‎effectively control the blood glucose as evident by ‎reduction in HA1c.ConclusionOral semaglutide can effectively and safely lower blood glucose without increase in the major adverse ‎cardiovascular events‎‎ (MACE).

    Comparison of Different Antiviral Regimens in the Treatment of Patients with Severe COVID-19: A Retrospective Cohort

    The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) causes respiratory disorders, with disease severity ranging from asymptomatic to critical manifestations. The current retrospective study compared the efficacies of different antiviral regimens used in patients suffering from severe COVID-19 disease from 19 January 2020 to December 2021 in a single center in Saudi Arabia. In total, 188 patients were enrolled in the current study, including 158 patients treated with different antiviral regimens, and 30 who did not receive any antiviral treatment. Different antiviral regimens, including favipiravir, remdesivir, oseltamivir, favipiravir/remdesivir, and favipiravir/oseltamivir were adopted. The effects of using different antivirals and antibiotics on the survival rate were evaluated, as well as the presence of comorbidities. Among all severely affected patients, 39/188 (20.7%) survived. Both age and comorbidities, including diabetes and hypertension, were significantly correlated with high case fatality following SARS-CoV-2 infection. Remdesivir alone and the combination of favipiravir and remdesivir increased the survival rate. Surprisingly, both imipenem and linezolid helped in the deterioration of disease outcome in the patients. A negative correlation was detected between increased mortality and the use of favipiravir and the use of either imipenem or linezolid. Among the compared antiviral regimens used in the treatment of severe COVID-19, remdesivir was found to be an effective antiviral that reduces COVID-19 case fatality. Antibiotic treatment using imipenem and/or linezolid should be carefully re-evaluated

    The Level of Burden among Caregivers of Patients with Alzheimer’s Disease in Saudi Arabia

    Background: Caregiver burden is a serious global issue associated with the growing number of older adult patients with Alzheimer’s disease (AD). AD patients become more dependent on their caregivers and require assistance with basic daily life activities. This study aims to measure the caregiver burden of informal caregivers of AD patients and to determine their characteristics. In addition, it intends to understand caregiver coping techniques and assess their medication knowledge. Methods: This was a cross-sectional study including 148 informal caregivers mainly recruited by the Saudi Alzheimer’s Disease Association (SADA). A four-part study questionnaire was used for data collection in the Arabic Language and included the following: socio-demographic characteristics of AD patients and their caregivers, the 12-item version of the Zarit Burden Interview (ZBI), and adapted questions on coping techniques and medication knowledge. Results: A total of 148 caregivers (62% were female) participated in this study, and 79.06% were between 30 and 60 years old. The ZBI average score was 27, indicating a moderate to high burden. Caregivers reported their need for services to improve their quality of life. The medication knowledge was insufficient in most aspects except that more than half were aware of medications’ side effects. Conclusion: Our study revealed that the average burden among informal caregivers of AD patients was moderate–high

    The impact of non-pharmaceutical interventions on the spread of COVID-19 in Saudi Arabia: Simulation approach

    Objectives: This paper aims to measure the impact of the implemented nonpharmaceutical interventions (NPIs) in the Kingdom of Saudi Arabia (KSA) during the pandemic using simulation modeling. Methods: To measure the impact of NPI, a hybrid agent-based and system dynamics simulation model was built and validated. Data were collected prospectively on a weekly basis. The core epidemiological model is based on a complex Susceptible-Exposed-Infectious-Recovered and Dead model of epidemic dynamics. Reverse engineering was performed on a weekly basis throughout the study period as a mean for model validation which reported on four outcomes: total cases, active cases, ICU cases, and deaths cases. To measure the impact of each NPI, the observed values of active and total cases were captured and compared to the projected values of active and total cases from the simulation. To measure the impact of each NPI, the study period was divided into rounds of incubation periods (cycles of 14 days each). The behavioral change of the spread of the disease was interpreted as the impact of NPIs that occurred at the beginning of the cycle. The behavioral change was measured by the change in the initial reproduction rate (R0). Results: After 18 weeks of the reverse engineering process, the model achieved a 0.4 % difference in total cases for prediction at the end of the study period. The results estimated that NPIs led to 64 % change in The R0. Our breakdown analysis of the impact of each NPI indicates that banning going to schools had the greatest impact on the infection reproduction rate (24 %). Conclusion: We used hybrid simulation modeling to measure the impact of NPIs taken by the KSA government. The finding further supports the notion that early NPIs adoption can effectively limit the spread of COVID-19. It also supports using simulation for building mathematical modeling for epidemiological scenarios

    According to some estimates, haemorrhoids afflict up to one-quarter of all individuals. There are numerous methods available to manage them, ranging from topical and medicinal medicines to outpatient treatments and surgical techniques to repair or excise. Given the disease's polysymptomaticism, determining which therapy choice is optimal is tough. Hemorrhoid disease treatment is one of the most difficult fields in general surgery, with various approaches utilised to cure this illness. In this research, we contrasted Hemorrhoidectomy verses Rubber Band Ligation results of treatment methods for hemorrhoids. Review the effectiveness and safety of the two most often used conventional treatments for haemorrhoids, rubber band ligation and excisional haemorrhoidectomy, and compare between the clinical results for both procedures. The PubMed database and EBSCO Information Services were utilized to choose the articles. In this review, all pertinent articles related to both our topic and other articles were used. Other articles that have nothing to do with this subject were not included. The group members looked through a certain format in which the data had been extracted. Internal hemorrhoid is a common pathological anorectal appearance, although it is a difficult condition to treat. Internal haemorrhoids symptoms and indicators should be thoroughly explored, as should clinical grading. Individual thinking and clinical considerations should influence the various possibilities for managing internal haemorrhoids and specific therapeutic approaches. At first, lifestyle changes should be made, such as consuming a high-fiber diet, developing sane bathroom routines, and administering phlebotropic drugs. When alternative treatments don't work, surgical methods and outpatient procedures should be used. Therapy management such as Hemorrhoidectomy or Rubber Band Ligation is critical to preventing future consequences from internal haemorrhoids

    SARS-CoV-2 vaccination modelling for safe surgery to save lives: data from an international prospective cohort study

    Background: Preoperative SARS-CoV-2 vaccination could support safer elective surgery. Vaccine numbers are limited so this study aimed to inform their prioritization by modelling. Methods: The primary outcome was the number needed to vaccinate (NNV) to prevent one COVID-19-related death in 1 year. NNVs were based on postoperative SARS-CoV-2 rates and mortality in an international cohort study (surgical patients), and community SARS-CoV-2 incidence and case fatality data (general population). NNV estimates were stratified by age (18-49, 50-69, 70 or more years) and type of surgery. Best- and worst-case scenarios were used to describe uncertainty. Results: NNVs were more favourable in surgical patients than the general population. The most favourable NNVs were in patients aged 70 years or more needing cancer surgery (351; best case 196, worst case 816) or non-cancer surgery (733; best case 407, worst case 1664). Both exceeded the NNV in the general population (1840; best case 1196, worst case 3066). NNVs for surgical patients remained favourable at a range of SARS-CoV-2 incidence rates in sensitivity analysis modelling. Globally, prioritizing preoperative vaccination of patients needing elective surgery ahead of the general population could prevent an additional 58 687 (best case 115 007, worst case 20 177) COVID-19-related deaths in 1 year. Conclusion: As global roll out of SARS-CoV-2 vaccination proceeds, patients needing elective surgery should be prioritized ahead of the general population
