    What is the best management for patients with evidence of asymptomatic ischemia on exercise stress testing?

    Patients with intermediate- or high-risk Duke Treadmill Score (DTS) on exercise stress testing (EST) should undergo myocardial perfusion imaging or exercise echocardiography, especially if they have abnormal values of Chronotropic Index or post-EST Heart Rate Recovery. For patients who have a low-risk DTS, the 4-year mortality is less than 1% to 2%; therefore, risk-factor reduction without further investigation is appropriate (strength of recommendation: B, based on cohort studies and consensus guideline)

Kaufman 179 pages December 2009 The 1983 publication of A Nation at Riskwas the start of the latest chapter in the long history and development of the public school superintendent. A Nation at Riskbrought new pressure and awareness on university programs that train school administrators coupled with suggestions on how to improve their effectiveness. This study describes many of these suggested reforms and investigates their effectiveness and rate of implementation. This qualitative study describes the experiences and perceptions of pre-service learning from the perspective of individuals who have completed pre-service training, worked as public school superintendents and currently teach in a pre-service program for future superintendents. The opinions of these individuals on pre-service training are unique as they have seen the impact these programs can have from three unique perspectives. Through an interview format, these individuals describe how their pre-service training helped them succeed as a superintendent as well as their frustrations with the process. They also elaborate on how they attempt to prepare future educational leaders with the skills they feel their students need. This study adds to the knowledge base of pre-service training for the superintendent endorsement by describing the impact these programs have had from the perspective of individuals who have gone through pre-service training, served as superintendents and the program to the student and the professor. Prior research has focused on one or two of these aspects but has never brought all three together. The findings from this study may lead to future research on how to make pre-service experiences more applicable to the real world or help students see the connections between what is being learned and how it will help them in future administrative positions. Other findings suggest a need for better transitions to the superintendent through organized and continued support provided by the university, the school district and other administrators in the field

    Secondary Sex Ratio among Women Exposed to Diethylstilbestrol in Utero

    BACKGROUND. Diethylstilbestrol (DES), a synthetic estrogen widely prescribed to pregnant women during the mid-1900s, is a potent endocrine disruptor. Previous studies have suggested an association between endocrine-disrupting compounds and secondary sex ratio. METHODS. Data were provided by women participating in the National Cancer Institute (NCI) DES Combined Cohort Study. We used generalized estimating equations to estimate odds ratios (ORs) and 95% confidence intervals (CIs) for the relation of in utero DES exposure to sex ratio (proportion of male births). Models were adjusted for maternal age, child's birth year, parity, and cohort, and accounted for clustering among women with multiple pregnancies. RESULTS. The OR for having a male birth comparing DES-exposed to unexposed women was 1.05 (95% CI, 0.95-1.17). For exposed women with complete data on cumulative DES dose and timing (33%), those first exposed to DES earlier in gestation and to higher doses had the highest odds of having a male birth. The ORs were 0.91 (95% C, 0.65-1.27) for first exposure at ≄ 13 weeks gestation to < 5 g DES; 0.95 (95% CI, 0.71-1.27) for first exposure at ≄ 13 weeks to ≄ 5 g; 1.16 (95% CI, 0.96-1.41) for first exposure at < 13 weeks to < 5 g; and 1.24 (95% CI, 1.04-1.48) for first exposure at < 13 weeks to ≄ 5 g compared with no exposure. Results did not vary appreciably by maternal age, parity, cohort, or infertility history. CONCLUSIONS. Overall, no association was observed between in utero DES exposure and secondary sex ratio, but a significant increase in the proportion of male births was found among women first exposed to DES earlier in gestation and to a higher cumulative dose.National Cancer Institute (N01-CP-21168, N01-CP-51017, N01-CP-01289

    A helium film coated quasi‐parabolic mirror to focus a beam of ultra‐cold spin polarized atomic hydrogen

    A 350 mK helium‐4‐coated mirror was used to increase the intensity of an ultra‐cold electron‐spin‐polarized atomic hydrogen beam. The mirror uses the observed specular reflection of atomic hydrogen from a superfluid‐helium‐covered surface. A quasi‐parabolic polished copper mirror was installed with its focus at the 5 mm diameter exit aperture of an atomic hydrogen stabilization cell in the gradient of an 8 T solenoid field. The four‐coned mirror shape, which was designed specifically for operation in the gradient, increased the beam intensity focused by a sextupole magnet into a compression tube detector by a factor of about 7.5.Peer Reviewedhttp://deepblue.lib.umich.edu/bitstream/2027.42/87512/2/40_1.pd
