    Evaluation of Neonatal Ankyloglossia

    Problem Ankyloglossia occurs when the tongue cannot extend past the gums or lips due to frenulum restriction. In breast feeding, proper movement of the tongue around the nipple is hindered causing difficulty for the infant to obtain a proper latch. This may result in slow weight gain (SWG), failure to thrive (FTT), and dehydration due to poor milk production. Evaluating for ankyloglossia is difficult, however, the Hazelbaker Ankyloglossia Tool for Lingual Frenulum Function (HATLFF) may be of benefit. Method Observational descriptive design utilizing a retrospective medical record review. The HATLFF was utilized in full term neonates suspected of ankyloglossia in a mid-western suburban primary care practice from February 28-April 8, 2022. Birth weights and one-month weights were recorded. Results Total sample size was nine (N=9). Ankyloglossia was not impacted by gender or race (χ2(1) = 0.32, p = .571). The mean appearance score was 5.44 (SD=1.59), mean function score was 3.44 (SD=1.01), and mean total score was 8.89 (SD=2.09). The mean weight gain for infants who underwent a frenotomy was 4.5 kg (SD=0.48 kg) at the one-month visit. The relationship between birth weight and one-month weight was weak (r=-.06). Conclusion The HATLFF assisted providers in determining need for frenotomy. All infants who underwent frenotomy demonstrated adequate weight gain by one-month of age. The HATLFF instrument may be a useful tool for determining the need for frenotomy, however, maternal comfort may also need to be considered

    Heavy and Light Chain Isotypes of Immunoglobulin in Epidermolysis Bullosa Acquisita

    Epidermolysis bullosa acquisita (EBA) is a chronic acquired blistering disease with characteristic clinical, pathologic, and immunopathologic features. The disease is characterized immunopathologically by circulating and tissue-bound IgG class autoantibodies (EBA antibodies) to the basement membrane zone of stratified squamous epithelium. Previous studies have shown that circulating and tissue-bound EBA antibodies are heterogenous in their ability to activate complement and have raised the possibility that functional heterogeneity might be related to IgG subclass restriction. In this study, we have characterized the IgG subclasses of the circulating and tissue-bound EBA antibodies by immunofluorescence and have examined the relationship between IgG subclass and complement binding. The results show that EBA antibodies belonging to all IgG subclasses are present in the skin of EBA patients. The results also show that EBA antibodies belonging to all IgG subclasses are present in the sera of most patients, including sera with and without complement binding EBA antibodies

    Self-perceived Well-being Among Doctor of Physical Therapy Students in the United States

    Introduction: The purpose of this study was to describe self-perceived well-being among Doctor of Physical Therapy (DPT) students in the United States during the COVID-19 pandemic and explore factors associated with well-being. Methods: This observational study was cross-sectional, using an anonymous, self-administered, nationwide survey. The survey included questions about an array of factors theoretically related to well-being, and incorporated the WHO-5 Well-being Index, the Perceived Stress Scale-10, and the Brief Resiliency Scale. Results: A total of 1,542 responded to the survey. Data from 1,537 DPT students in the U.S. were included in the analysis. Well-being was positively correlated with resilience (r = 0.457; p \u3c 0.001), male gender (p \u3c 0.001), heterosexuality (p = 0.022), being married (p = 0.004) or living with a partner or spouse (p = 0.036), and being physical active (p \u3c 0.001). Well-being was negatively correlated with higher perceived stress (r = -0.686; p \u3c 0.001), the number of friends or family who died for non-COVID reasons during the prior year (p = 0.005), food insecurity (p \u3c 0.001), having chronic pain (p \u3c 0.001), more days absent from school (p = 0.021), and being a first-generation college student (p = 0.007). Surprisingly, COVID-19 infection status and having at least one close friend or relative die of COVID-19 were not correlated with self-perceived wellness. Regression modeling using individual factors found that being male (p \u3c 0.001), married (p = 0.046), and physically active (p \u3c 0.001) were positive predictors of well-being, while having food insecurity (p \u3c 0.001) and chronic pain (p \u3c 0.001) were negative predictors. Conclusion: Among DPT students in the U.S., self-perceived well-being is negatively correlated with higher perceived stress; while concurrently being positively correlated with resilience. Well-being is partially predicted by modifiable factors including physical activity, chronic pain and food insecurity. Our findings deepen the understanding of DPT students’ well-being and can help inform the services and resources provided by colleges and universities to better address modifiable factors that are predictive of well-being among DPT students

    MARTA in Clayton County: An Opportunity for Equitable Transit-Oriented Development

    Amidst national anxiety surrounding the disappearance of low-cost housing within American cities, affordability advocates are beginning to champion improved transit services as a way to lower household costs. As central urban neighborhoods continue to attract upwardly-mobile income earners, affordable properties are increasingly relegated to the suburban margins, which often feature only minimal transit service. The Atlanta region currently has an opportunity to intervene in these worsening patterns. With new transit service on the horizon for Clayton County, a coordinated housing and transportation plan could ensure that long-term affordable measures surround planned corridors. One potential intervention is equitable transit-oriented development, which champions locating housing near transit to address the ever-escalating costs, which keep low-income households in poverty. To investigate the viability of these strategies in Clayton County, this study considers the national context of housing and transit planning. Observing patterns and factors that contributed to the success of equitable transit-oriented development projects, one common theme arises: regionalism. Guided by this framework, my paper assesses how the opportunities associated with the expansion of MARTA into Clayton County can Ellen Ray | Option Paper 2 translate into expanded housing options for the whole Atlanta region. Reviewing planning literature, regional demographics, regional governance, and planning tools and policies, this study ultimately makes recommendations towards an equitable housing-transit strategy in Clayton County. If housing and transit planning are considered simultaneously, this public investment offers the potential to positively impact the county's current and forthcoming residents, especially as the frontier of affordability in the Atlanta region continues moving southward

    Context Matters: Intertextuality and Voice in the Early Modern English Controversy about Women

    This dissertation examines three clusters of works from the early modern English controversy about women--the debate about the merits and flaws of womankind--in order to argue that authors in the controversy took advantage of the malleability of women's voices to address issues beyond the worth of women. I depart from standard treatments of the controversy by giving priority to the intertextual contexts among works that engage with one another. Attending to the intertextual elements of this genre reveals the metapoetic concerns of the authors and the way such authors fashion their feminine apologists as discursive agents in order to express those concerns. Chapter 1 examines Edward Gosynhyll's sixteenth-century works in tandem with Geoffrey Chaucer's The Legend of Good Women and "The Wife of Bath's Prologue and Tale," arguing that Gosynhyll's revisions of Chaucer--revisions embodied by the feminine apologists in the texts--are integral to his project of establishing the controversy genre as multivalent and dialectical. The resulting metacommentary examines in a new light the age-old rhetorical tradition of exemplarity, a persuasive tool used in diverse literary genres. Chapter 2 considers the way the anonymous play Swetnam the Woman-Hater uses cross-voicing and cross-dressing to establish the performative nature of controversy conventions. In doing so, the play argues for the social benefits of abandoning essentialist logic in favor of gender performance, as such performance makes the role of apologist available to men and women alike. This cluster reconsiders the very processes by which a person--male or female--can be known to others. Finally, I trace John Taylor's use of the marginal woman in his controversy works in order to demonstrate the extent to which Taylor makes these women instrumental in establishing his own poetic and social identity. This project contributes to studies on the English controversy as well as to the field of early modern women and women's writing by arguing that authors found the genre generally and the woman's voice specifically to be fit vehicles for articulating poetic agendas beyond the immediate task of debating the nature of womankind
