    Pattern of antibiotic use in neonatal intensive care unit in tertiary care hospital in Southern India

    Background: Repeated and prolonged courses of antibiotics exposure have resulted in an increase in the prevalence of hospital acquired infections and antibiotic resistant profile. The objective of this study was to quantify the use of antibiotics in a neonatal intensive care unit (NICU) from rural tertiary health care centre.Methods: A hospital based cross-sectional study was conducted in the NICU of tertiary care hospital located in western Maharashtra, India during the year 2011-12. A total of 237 neonates admitted to NICU from October 2011 to March 2012 were enrolled in the study according to inclusion criteria of the study. Demographic details, data on antibiotic prescriptions (name, dose, frequency, route of administration) were recorded by utilizing pre-tested structured proforma.Results: A total of 3822 prescriptions were received by the neonates and commonly prescribed antibiotics were amikacin (75.53%), cefotaxime (43.34%) and ampicillin (31.33%) respectively. It was also noted that 50% of the drugs prescribed were in compliance with the national list of essential medicines 2011. The max, 68.75% of antibiotics prescribed were in generics forms however, 12.5% were prescribed in the form of fixed dose combinations.Conclusions: The revealed that 3rd generation cephalosporins and amikacin are most commonly used antibiotics in NICU

    Health Literacy among Medically Underserved: The Role of Demographic Factors, Social Influence, and Religious Beliefs

    The current study examined the sociodemographic and psychosocial variables that predicted being at risk for low health literacy among a population of racially and ethnically diverse patients accessing primary care services at community-based clinics. Participants (N = 416) were aged 50-75 years, currently not up-to-date with colorectal cancer (CRC) screening, at average CRC risk, and enrolled in a randomized controlled trial (RCT) aimed at promoting CRC screening. Participants completed a baseline interview that assessed health literacy as measured by Rapid Estimate of Adult Literacy in Medicine-Revised, sociodemographic factors, and psychosocial variables (e.g., health beliefs) prior to randomization and receipt of an intervention. Thirty-six percent of the participants were found to be at risk for low health literacy. Sociodemographic and psychosocial variables were assessed as predictors of being at risk for low health literacy using logistic regression. In the final model, predictors were male gender, being from a racial/ethnic minority group, being unable to work, having higher social influence scores, and having higher religious belief scores. These findings suggest several patient characteristics that may be associated with low health literacy, and highlight the importance of supporting all patients through simplified and clear communications and information to improve understanding of CRC screening information

    Insect pests are considered the major hurdle in enhancing the production and productivity of any farming system. The use of conventional synthetic pesticides has led to the emergence of pesticide-resistant insects, environmental pollution, and negative effects on natural enemies, which have caused an ecological imbalance of the predator-prey ratio and human health hazards; therefore, eco-friendly alternative strategies are required. The plant kingdom, a rich repertoire of secondary metabolites, can be tapped as an alternative for insect pest management strategies. A number of plants have been documented to have insecticidal properties against various orders of insects in vitro by acting as antifeedants, repellents, sterilant and oviposition deterrents, etc. However, only a few plant compounds are applicable at the field level or presently commercialised.     SARS-CoV-2 B.1.617.2 Delta variant replication and immune evasion

    Abstract: The B.1.617.2 (Delta) variant of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) was first identified in the state of Maharashtra in late 2020 and spread throughout India, outcompeting pre-existing lineages including B.1.617.1 (Kappa) and B.1.1.7 (Alpha)1. In vitro, B.1.617.2 is sixfold less sensitive to serum neutralizing antibodies from recovered individuals, and eightfold less sensitive to vaccine-elicited antibodies, compared with wild-type Wuhan-1 bearing D614G. Serum neutralizing titres against B.1.617.2 were lower in ChAdOx1 vaccinees than in BNT162b2 vaccinees. B.1.617.2 spike pseudotyped viruses exhibited compromised sensitivity to monoclonal antibodies to the receptor-binding domain and the amino-terminal domain. B.1.617.2 demonstrated higher replication efficiency than B.1.1.7 in both airway organoid and human airway epithelial systems, associated with B.1.617.2 spike being in a predominantly cleaved state compared with B.1.1.7 spike. The B.1.617.2 spike protein was able to mediate highly efficient syncytium formation that was less sensitive to inhibition by neutralizing antibody, compared with that of wild-type spike. We also observed that B.1.617.2 had higher replication and spike-mediated entry than B.1.617.1, potentially explaining the B.1.617.2 dominance. In an analysis of more than 130 SARS-CoV-2-infected health care workers across three centres in India during a period of mixed lineage circulation, we observed reduced ChAdOx1 vaccine effectiveness against B.1.617.2 relative to non-B.1.617.2, with the caveat of possible residual confounding.     Magnesium is intimately involved in over 300 enzymatic reactions, particularly in processes involving the formation and utilization of ATP. It is also a known fact that alcoholism is associated with hypomagnesaemia. The present study determines the significance of routine serum magnesium determination in alcoholic patients consuming illicit liquor from lower socio-economic background attending deaddiction centre. Serum magnesium, sodium and potassium concentrations in 200 chronic alcoholic patients consuming illicit liquor from lower socio-economic background admitted to the deaddiction centre were estimated and compared to 158 healthy nonalcoholic controls. Serum magnesium levels were significantly decreased whereas serum transaminases and gammaglutamyl transpeptidases were increased in alcoholic patients compared to healthy nonalcoholic controls.Hypomagnesemia could be due to the increased oxidative stress and might be a risk factor in the progression of fatty liver to steatohepatitis.The results of the present investigation support the practice of routine magnesium administration to chronic alcoholics.     Transmission of B.1.617.2 Delta variant between vaccinated healthcare workers

    AbstractBreakthrough infections with SARS-CoV-2 Delta variant have been reported in doubly-vaccinated recipients and as re-infections. Studies of viral spread within hospital settings have highlighted the potential for transmission between doubly-vaccinated patients and health care workers and have highlighted the benefits of high-grade respiratory protection for health care workers. However the extent to which vaccination is preventative of viral spread in health care settings is less well studied. Here, we analysed data from 118 vaccinated health care workers (HCW) across two hospitals in India, constructing two probable transmission networks involving six HCWs in Hospital A and eight HCWs in Hospital B from epidemiological and virus genome sequence data, using a suite of computational approaches. A maximum likelihood reconstruction of transmission involving known cases of infection suggests a high probability that doubly vaccinated HCWs transmitted SARS-CoV-2 between each other and highlights potential cases of virus transmission between individuals who had received two doses of vaccine. Our findings show firstly that vaccination may reduce rates of transmission, supporting the need for ongoing infection control measures even in highly vaccinated populations, and secondly we have described a novel approach to identifying transmissions that is scalable and rapid, without the need for an infection control infrastructure.</jats:p
