Impact of Bronchoscopic Lavage and Aggressive Chest Physiotherapy on Clinical Pulmonary Infection Score of Patients of Severe Traumatic Brain Injury
Introduction: Traumatic brain injury patients present in the emergency room with a variable conscious status depending upon the severity of the injury. The process of aspiration starts from the scene of trauma as the patients with Traumatic brain injury and chest trauma have higher rates of aspiration pneumonia.
Materials and Methods: We conducted a prospective study to assess the impact of bronchoscopy lavage and aggressive chest physiotherapy on the Critical Pulmonary Infection Score of Patients with Severe Traumatic Brain Injury in the Neurocritical Care Unit. Patients with severe traumatic brain injury who met the inclusion criteria were enrolled in the study. Patients of both genders, ages ranging from 05 – 70 years were included in the study.
Results: 48 patients who met the inclusion criteria were enrolled in the study. 8 patients expired before 7 days and were excluded from the study. 88% were male and 12 % were females. Out of 40 patients, 30 were ventilated and 10 were managed without ventilation. Brain contusion was the most common CT scan finding. Out of 40 patients, 18 underwent surgical intervention, with decompressive hemicraniectomy being the most commonly performed procedure. Out of 40 patients, 32 patients underwent bronchoscopy. 8 patients out of 32 had more than two sessions of bronchoscopy.
Conclusion: As Critical Pulmonary Infection Score is an important indicator for pneumonia in the neurocritical care unit, the importance of standardized chest care with the help of bronchoscopic lavage and chest physiotherapy in keeping the Critical Pulmonary Infection Score on the lower side can prevent patients from developing life-threatening pneumonia
A Novel Single-Fed Dual-Band Dual-Circularly Polarized Dielectric Resonator Antenna for 5G Sub-6GHz Applications
In this research article, a single-fed dual-band circular polarized (CP) dielectric resonator
antenna (DRA) for dual-function communication, such as GPS and WLAN, was made. Initially, the
proposed design process was initiated by designing a linearly polarized singly fed-DRA. To attain
CP fields, the cross-shape conformal metal strip was optimized to excite the fundamental and the
high-order mode in the two frequency bands. The metallic strip (parasitic) was utilized on top of
the rectangular DRA to improve and widen the impedance and axial ratio (AR) bandwidth. This
step led to a 2.73% improvement on the lower band and an impact of 6.5% on the upper band while
on the other side a significant improvement was witnessed in the AR bandwidth in both frequency
bands. A prototype was designed and fabricated in order to validate its operations. The measurement
outcomes of the proposed antennas authenticated wideband impedance bandwidths of 6.4% and
25.26%, and 3-dB axial ratios (AR) of 21.26% and 27.82% respectively. The prototype is a decent
candidate for a global positioning system (GPS) and wireless local area network (WLAN).This project has received funding from the Universidad Carlos III de Madrid and the

Diosgenin normalization of disrupted behavioral and central neurochemical activity after single prolonged stress
Introduction: Post-traumatic stress disorder (PTSD) is a chronic mental illness triggered by traumatic experiences such as wars, natural disasters, or catastrophes, and it is characterized by anxiety, depression and cognitive impairment. Diosgenin is a steroidal sapogenin with known neuroprotective and antioxidant properties. This study aimed to assess the pharmacological potential of diosgenin in a single prolonged stress (SPS) model of PTSD, plus other behavioral models along with any consequent alterations in brain neurochemistry in male mice.Methodology: SPS was induced by restraining animals for 2Â h, followed by 20Â min of forced swim, recuperation for 15Â min, and finally, exposure to ether to induce anesthesia. The SPS-exposed animals were treated with diosgenin (20, 40, and 60Â mg/kg) and compared with the positive controls, fluoxetine or donepezil, then they were observed for any changes in anxiety/depression-like behaviors, and cognitive impairment. After behavioral screening, postmortem serotonin, noradrenaline, dopamine, vitamin C, adenosine and its metabolites inosine and hypoxanthine were quantified in the frontal cortex, hippocampus, and striatum by high-performance liquid chromatography. Additionally, animal serum was screened for changes in corticosterone levels. Results: The results showed that diosgenin reversed anxiety- and depression-like behaviors, and ameliorated cognitive impairment in a dose-dependent manner. Additionally, diosgenin restored monoamine and vitamin C levels dose-dependently and modulated adenosine and its metabolites in the brain regions. Diosgenin also reinstated otherwise increased serum corticosterone levels in SPS mice. Conclusion: The findings suggest that diosgenin may be a potential candidate for improving symptoms of PTSD
