    Health-related quality of life in patients with type 1 diabetes mellitus in the different geographical regions of Brazil : data from the Brazilian Type 1 Diabetes Study Group

    Background: In type 1 diabetes mellitus (T1DM) management, enhancing health-related quality of life (HRQoL) is as important as good metabolic control and prevention of secondary complications. This study aims to evaluate possible regional differences in HRQoL, demographic features and clinical characteristics of patients with T1DM in Brazil, a country of continental proportions, as well as investigate which variables could influence the HRQoL of these individuals and contribute to these regional disparities. Methods: This was a retrospective, cross-sectional, multicenter study performed by the Brazilian Type 1 Diabetes Study Group (BrazDiab1SG), by analyzing EuroQol scores from 3005 participants with T1DM, in 28 public clinics, among all geographical regions of Brazil. Data on demography, economic status, chronic complications, glycemic control and lipid profile were also collected. Results: We have found that the North-Northeast region presents a higher index in the assessment of the overall health status (EQ-VAS) compared to the Southeast (74.6 ± 30 and 70.4 ± 19, respectively; p < 0.05). In addition, North- Northeast presented a lower frequency of self-reported anxiety-depression compared to all regions of the country (North-Northeast: 1.53 ± 0.6; Southeast: 1.65 ± 0.7; South: 1.72 ± 0.7; Midwest: 1.67 ± 0.7; p < 0.05). These findings could not be entirely explained by the HbA1c levels or the other variables examined. Conclusions: Our study points to the existence of additional factors not yet evaluated that could be determinant in the HRQoL of people with T1DM and contribute to these regional disparities

    PIK3CA mutations are frequent in esophageal squamous cell carcinoma associated with chagasic megaesophagus and are associated with a worse patient outcome

    Chronic diseases such as chagasic megaesophagus (secondary to Chagas' disease) have been suggested as etiological factors for esophageal squamous cell carcinoma; however, the molecular mechanisms involved are poorly understood.Background Chronic diseases such as chagasic megaesophagus (secondary to Chagas’ disease) have been suggested as etiological factors for esophageal squamous cell carcinoma; however, the molecular mechanisms involved are poorly understood. Objective We analyzed hotspot PIK3CA gene mutations in a series of esophageal squamous cell carcinomas associated or not with chagasic megaesophagus, as well as, in chagasic megaesophagus biopsies. We also checked for correlations between the presence of PIK3CA mutations with patients’ clinical and pathological features. Methods The study included three different groups of patients: i) 23 patients with chagasic megaesophagus associated with esophageal squamous cell carcinoma (CM/ESCC); ii) 38 patients with esophageal squamous cell carcinoma not associated with chagasic megaesophagus (ESCC); and iii) 28 patients with chagasic megaesophagus without esophageal squamous cell carcinoma (CM). PIK3CA hotspot mutations in exons 9 and 20 were evaluated by PCR followed by direct sequencing technique. Results PIK3CA mutations were identified in 21.7% (5 out of 23) of CM/ESCC cases, in 10.5% (4 out of 38) of ESCC and in only 3.6% (1 case out of 28) of CM cases. In the CM/ESCC group, PIK3CA mutations were significantly associated with lower survival (mean 5 months), when compared to wild-type patients (mean 2.0 years). No other significant associations were observed between PIK3CA mutations and patients’ clinical features or TP53 mutation profile. Conclusion This is the first report on the presence of PIK3CA mutations in esophageal cancer associated with chagasic megaesophagus.     Percepção discente e docente sobre o desenvolvimento curricular na atenção primária após Diretrizes Curriculares de 2014

    RESUMO As novas diretrizes curriculares nacionais (DCN) do curso de medicina preveem uma formação com maior foco na Atenção Primária à Saúde. A Faculdade de Medicina da Universidade Federal de Goiás (UFG), buscando estas mudanças e adaptando-se às DCN de 2014, criou um novo Projeto Pedagógico de Curso (PPC). Este estudo avaliou as percepções de discentes e docentes sobre o desenvolvimento do novo currículo do curso de medicina de uma Universidade Federal da região centro-oeste do Brasil, após as novas Diretrizes Curriculares Nacionais de 2014, quanto ao ensino na Atenção Primária à Saúde (APS). Realizou-se uma pesquisa qualitativa, cuja coleta de dados ocorreu por meio de dois grupos focais: um com docentes e outro com discentes. As perguntas norteadoras foram relacionadas ao entendimento dos discentes sobre a APS e a concordância entre os assuntos teóricos e o que é visto nas atividades práticas, a avaliação do novo PPC e as críticas e sugestões ao novo currículo e a desospitalização. Com docentes, as perguntas-guia questionaram sobre a implementação do novo currículo, a avaliação do novo PPC, a percepção de apoio institucional e as críticas à formação na APS. Para análise dos dados oriundos da transcrição dos grupos focais utilizou-se a técnica de análise de conteúdo que buscou sistematizar as questões respondidas pelos participantes, facilitando sua interpretação. Quanto à modalidade, foi escolhida a análise temática fundamentada em Bardin. A percepção de discentes e docentes foi que o desenvolvimento do novo currículo ainda é frágil. Os discentes apontaram a depreciação do ensino na APS por alguns professores e a falta de longitudinalidade no ensino da APS. Os docentes destacaram a falta de apoio da gestão municipal, escassez de campos de estágio e de formadores qualificados para esta mudança. Conclui-se que o novo currículo, mesmo desenhado para contemplar o ensino na APS, apresenta algumas lacunas de temas relevantes, enfrenta resistências por parte do corpo docente e carece de professores com formação específica na área
