PROFIL EPIDEMIOCLINIQUE ET EVOLUTIF DES LYMPHOMES MALINS NON HODGKINIEN VUS AU SERVICE D’ONCOLOGIE DU CHUJRA ANTANANARIVO MADAGASCAR ” EPIDEMIOCLINIC AND EVOLUTIVE PROFILE OF NON-HODGKINIAN MALIGNANT LYMPHOMES AT THE SERVICE OF ONCOLOGY OF THE UNIVERSITY
Non-Hodgkin's malignant lymphomas (NHLH) are malignant proliferations of lymphoid tissues. The objective of our study is to describe the epidemiological, clinical and evolutionary profile of malignant non-Hodgkin lymphoma diagnosed, monitored and treated in the oncology department of the Joseph Ravoahangy Andrianavalona (CHUJRA) Antananarivo Madagascar University Hospital. This is a descriptive retrospective study of all cases of LMNH diagnosed in the Oncology Department of CHUJRA Antananarivo Madagascar over a period of 05 years, from 01 January 2013 to 31 December 2017. During our study, 96 cases were collected with an average age of 39 years. There was a male predominance with a sex ratio of 1.52. The most common clinical manifestations were lymphadenopathy and general impairment. Stage III and Stage III LMNH were the most common with 34.37% and 30.20%, respectively. More than 90% of patients received specific treatment with a complete remission of about 80%. But many patients are still under diagnosed for lack of means and for fear of consulting a doctor, which still requires a lot of effort on the part of officials at all levels
Profil épidemio-clinique et radiologique des atteintes ostéo-articulaires des hémophiles à Madagascar
Introduction: déterminer le profil épidémio-clinique et radiologique des atteintes ostéo-articulaires des hémophiles malagasy.
Méthodes: une étude prospective, descriptive portant sur 25 patients hémophiles venant de tout Madagascar a été réalisée. Des radiographies numérisées des genoux, des chevilles et des coudes en incidence de face et de profil ainsi qu'une échographie des hanches, des genoux, des chevilles et des coudes ont été réalisées chez ces patients. Le type et la sévérité de la maladie ainsi que l'aspect de la cavité articulaire, la synoviale, les noyaux épiphysaires et les surfaces articulaires ont été analysés.
Résultats: soixante-huit pourcent des patients étaient hémophiles de type A et 32 % de type B. Quarante pourcent étaient classés sévères, 28 % modérés et 32 % mineurs. Les atteintes ostéo-articulaires ont été retrouvées chez 56 % des patients. Il n'existait pas de prédominance d'atteinte selon le type ni la sévérité de la maladie. Les plus jeunes étaient les plus atteints et l'articulation du genou et de la cheville étaient les plus touchées.
Conclusion: les complications ostéo-articulaire de l'hémophilie sont graves et ne dépendent pas du type ni de la sévérité de l'affection. Elles touchent surtout les enfants d'âge scolaire. Le couple radiographie-échographie permet de diagnostiquer et de surveiller ces lésions
Doppler transcranien au cours de la drépanocytose chez l’enfant Malagasy
Introduction: Le doppler transcrânien est un outil efficace permettant de dépister les enfants drépanocytaires à risque d'AVC. Méthodes: Nous avons réalisé une étude descriptive transversale sur des enfants Malagasy âgés entre 24 mois et 15 ans (groupe 1: 57 drépanocytaires, groupe 2: 43 témoins) afin d'évaluer le profil vélocimétrique des artères cérébrales chez les drépanocytaires. Un examen Doppler transcrânien a été réalisé avec étude des flux sanguins cérébraux chez les enfants des deux groupes. Résultats: Pour les sujets drépanocytaires, la vitesse moyenne (VM) de l'artère cérébrale moyenne était de 100,9 ± 26,8 cm/s, l'indice de pulsatilité (IP) de 0,73 ± 0,20, la différence entre les artères cérébrales moyennes droite et gauche (ACMr) de 19,8 ± 21,5 cm/s, le rapport des vitesses de l'artère cérébrale antérieure/artère cérébrale moyenne (ACA/ACM) de 0,7 ± 0,2. Pour les enfants non drépanocytaires, VM: 80,6 ± 19,3 cm/s, IP: 0,79 ± 0,14, ACMr: 17 ± 20,1 cm/s, ACA/ACM: 0,8 ± 0,2. La vélocité des enfants drépanocytaires était supérieure au groupe contrôle. Les vitesses ont été corrélées avec le taux d'hémoglobine et l'âge et non pas avec le sexe et le volume globulaire moyen. Conclusion: Les vitesses circulatoires cérébrales sont élevées chez les drépanocytaires que les enfants non drépanocytaires et sont influencées par le taux d'hémoglobine et l'âge.Pan African Medical Journal 2016; 2
Troubles de l’hémostase en réanimation chirurgicale de l’Hôpital Universitaire Joseph Ravoahangy Andrianavalona Antananarivo
Les troubles de l'hémostase sont fréquemment observés en réanimation. L'objectif de cette étude est d'en déterminer l'incidence, les étiologies et la prise en charge pour en diminuer le risque de morbi- mortalité. Etude rétrospective sur une période de huit mois en réanimation chirurgicale du CHUA HUJRA. Les dossiers étudiés étaient ceux contenant tous les premiers bilans d'hémostase lors de l'admission du patient en réanimation, avec un âge supérieur à 15 ans. Etaient exclus les dossiers des post opérés (malade ayant subi une intervention chirurgicale). Les dossiers retenus pour l'étude étaient ceux qui ont présenté une ou plusieurs anomalies au niveau du bilan d'hémostase. Cent cinquante-cinq dossiers ont été colligés dont 20,64% ont présenté un trouble de l'hémostase. Les patients de sexe féminin (53,12%) étaient les plus concernés, avec une population âgée en moyenne de 49±19 ans. Trois patients (9,37%) ont présenté une notion de diathèse hémorragique à l'interrogatoire. Les différents troubles de l'hémostase étaient: une diminution du taux de prothrombine (100%), un taux de céphaline activé allongé (78,12%), un international normalized ratio élevé (43,75%) et des thrombopénies (40,62%). Chez 3,12% de ces cas, des accidents hémorragiques ont survenu. La plupart de ces troubles de l'hémostase étaient présents au décours d'hémorragies digestives (46,87%) dont la prise en charge était constituée par l'administration de vitamine K1 (68,75%) et de transfusion de plasma frais congelé (65,62%). D'autres pathologies ont été également incriminées. Une incidence de 15,62% de mortalité a été retrouvée. L'existence des troubles de l'hémostase en réanimation constitue un facteur pronostique imposant un dépistage et une prise en charge précoces et adéquats pour pouvoir en réduire le taux de mortalité.Key words: Coagulation, réanimation, transfusion, trouble de l´hémostas
Infections in children sickle followed in the outpatient at UPFR Hematology HJRA Antananarivo Madagascar “Infections chez les enfants drépanocytaires suivis en consultation externe à l’UPFR Hématologie HJRA Antananarivo Madagascar”
Résumé: Introduction: Les infections sont responsables d’une part importante de morbi-mortalité chez les enfants drépanocytaires. Notre objectif est d’évaluer le profil épidémio-clinique des infections chez les enfants drépanocytaires suivis en consultation externe.
Matériels et méthodes: Nous avons réalisé une étude rétrospective sur 56 enfants drépanocytaires fébriles sur une période de 6 mois (Janvier- Juin 2015). Nous avons analysé les caractéristiques cliniques et paracliniques de ces enfants drépanocytaires.
Résultats: La tranche d’âge de 0 à 5 ans était la plus touchée. Le suivi était irrégulier chez tous nos patients. Sur le plan clinique, 4 cas de Streptococcus pneumoniae, 1 cas de Mycobactérium tuberculosis et 1 cas de Plasmodium falciparum ont été diagnostiqués. L’antibiothérapie probabiliste dans la majorité des cas était basée sur les céphalosporines de 3ème génération. L’évolution était favorable dans 85,71% des cas.
Conclusion: Les complications infectieuses sont fréquentes chez les enfants drépanocytaires à Madagascar.

Abstract: Introduction: The infections are responsible for a large share of morbidity and mortality in sickle cell children. Our objective is to evaluate the epidemiological and clinical profile of infections in children with sickle cell disease outpatients.
Materials and Methods: We perfomed a retrospective study of 56 febrile affected children on a 6 month period (January – June 2015). We analyzed the clinical and laboratory characteristics of these affected children.
Results: The age groups of 0-5years were the most affected. Follow up was irregular in all our patients. Clinically, 4 cases of Streptococcus pneumonia, 1 case of Mycobacterium tuberculosis and 1 case of Plasmodium falciparum were diagnosed. The empiric antibiotic therapy in the majority of cases was based on the 3rd generation cephalosporins. The outcome was favorable in 87,5% of cases.
Conclusion: Infectious complications are common among affected children in Madagascar.

LEUCEMIE MYELOIDE CHRONIQUE CHEZ UN NOURRISSON VU AU SERVICE D’HEMATOLOGIE ONCOLOGIE DU CHUJRA ANTANANARIVO “CHRONIC MYELOID LEUKEMIA IN A INFANT SEEN AT THE SERVICE OF HEMATOLOGY ONCOLOGY OF CHUJRA ANTANANARIVO”
Introduction: Chronic myelogenous leukemia (CML) is a myeloproliferative disorder due to monoclonal myeloid proliferation without maturation blockade, predominant in the granular lineage. Our goal is to report a case of chronic myeloid leukemia in an infant to clarify the particularity of management and follow-up.
Observation: This was an 18-month-old male infant who was referred for splenomegaly. The blood count showed leukocytosis at 450Giga / L with balanced myelemia. The Philadelphia chromosome was positive for cytogenetic examination of the bone marrow. The patient had received specific treatment with Imatinib. Clinical and haematological remission was obtained after 2 months of treatment. No side effects were noted.
Discussion: Chronic myelogenous leukemia is rare in infants. The therapeutic protocol in children is not yet standardized but only obtained from that of the adult. Management requires increased surveillance to try to detect early side effects, especially stunting.
Conclusion: Our patient is the first, youngest diagnosed with chronic myeloid leukemia in Madagascar. Treatment evolution was favorable, but long-term side effects remain to be monitored.

Onychomycosis: experience of the laboratory of parasitology-mycology of CHU-Joseph Ravoahangy Andrianavalona, Antananarivo, Madagascar
Introduction: onychomycosis is defined as a fungal infection of the nails. They represent the main cause of onychopathy. They constitute a public health problem because of their increased prevalence in the world ranging between 20 to 30%. However, they remain under documented in Madagascar. This study was conducted in order to determine the mycological profile of onychomycosis diagnosed in the Parasitology Mycology laboratory of CHU-JRA.
Methods: a descriptive retrospective study was taken over a 13-year period from June 2005 to December 2018. The data presenting onychomycosis on the mycological outcome register were included in the study. Results showing the presence of fungi on direct examination and / or culture were considered positive.
Results: during the study period, a prevalence of 17.75% (180/1014) was observed. The age of our patients ranged from 3 to 76 years. Women were the most affected in 68.34% (n = 123) with a sex ratio of 0.46. Onychomycosis was localized in 64.65% of the fingers (n = 128). Simultaneous involvement of the hands and feet was found in 10% of the cases (n = 18). Candida albicans (C. albicans) was the most isolated in 33.03% (n = 71) of cases, followed by other species of Candida (24.65%, n = 53) and Trichophyton spp (9.3%, n=17). Mixed infections associating dermatophytes, Yeasts and molds were found in 23.33% (n = 42) of the cases.
Conclusion: this is the first investigation dealing with onychomycosis in Madagascar. These data may be useful for future research and in the development of preventive and educational strategies
Anemia in Malagasy hospitalized patients
Background: Anemia is a public health problem with major health consequences. According to the WHO, anemia worldwide is 1.62 billion people (95% confidence interval (CI): 1.50-1.74 billion), which corresponds to 24.8% Of the population (95% CI: 22.9-26.7%).Methods: We studied the clinical and biological data of hospitalized patients who applied for a complete blood count in hematology laboratory of Joseph Ravoahangy Andrianavalona Antananarivo hospital between January and April 2017.Results: Sixty-four percent of hospitalized patients had anemia. Anemia was more common in women. Anemia was encountered mainly in intensive care unit (33.9%) and occurred in perioperative cases in the majority of cases (33.4%).Conclusions: Anemia is a common symptom in hospitals, particularly in intensive care unit. In the Malagasy hospital environment, etiologies are numerous, in particular hemorrhage, sickle cell disease or cancer
MICROBIOLOGICAL SCREENING OF OTORRHOEA FROM PEOPLE COMING TO HOSPITAL IN MAHAJANGA
In whole, 56 patients were included. Amidst identified microorganisms were fungus (4,7%) and bacteria (95,3%) to which Gram negative bacilli represented 72,1% (n=44), Gram positive cocci 6,4% (n=10), Gram positive bacilli 8,2% (n=5) and Gram negative cocci 3,3% (n=2). Among these bacterias, Pseudomonas aeruginosa and Proteus sp were predominant, with respectively 41% (n=25), 23% (n=14). However, three cases of S. aureus reported, six with negative coagulase Staphylococcus, one with Escherichia coli, one with Klebsiella sp, one with Haemophilus sp, two cases with Neisseria sp and four cases with Corynebacterium sp. Two types of cultures were noticed, one of them monomorphic (91,1%, n=51) and the other polymorphic (8,9%, n=5) to which 3 associations of P. aeruginosa-Proteus sp, 1 association of P. aeruginosa- coagulase negative Staphylococcus and 1 association of P. aeruginosa- E. coli. No resistance to ciprofloxacin was observed with Pseudomonas, Neisseria sp, Haemophilus, and enterobacteria except for E. coli. No resistance to rifampicin was observed with S. aureus. However, the sensitivity of S. aureus to ciprofloxacin decreased (one bacterium out of three).
The use of rifampicin or fluoroquinolones should be based on the type of ear infections
